[One-staged surgery for coarctation of the aorta associated with annuloaortic ectasia--a case report].

A 37-year-old man was admitted to St. Marianna University Hospital with an episode of severe anterior chest pain. Contrast enhanced CT scan and digital subtraction angiography disclosed an aneurysmal dilatation of the aortic root and almost completely obstructed aortic isthmus. Modified Bentall operation was performed with simultaneous graft bypass using a woven Dacron prosthesis between the ascending aorta and the abdominal aorta through the median thoracoabdominal incision. Histologically the aortic wall showed cystic medial necrosis, and the aortic valve showed myxomatous degeneration. Only 9 cases with coarctation of the aorta associated with annuloaortic ectasia have been reported in the Japanese literature, and this is believed to be the first report that one-staged surgery has successfully been done dealing simultaneously both the coarctation and the annuloaortic ectasia.

Does glucagon improve the viability of ischemic intestine?

The effects of glucagon, heparin, urokinase, thromboxane synthetase inhibitor (OKY-046), and the free radical scavengers, superoxide dismutase and catalase (SOD + CAT), on the viability of ischemic intestine were evaluated based on various parameters measured. The mucosal blood flow, the fluorescence pattern, and the histopathological findings in a rabbit model with 3.5 hr total vascular occlusion of a short small intestine indicated that glucagon improved the ischemic intestine. Glucagon increased, tremendously, the mucosal blood flow by 112% in the ischemic intestine compared with that of 25% in the nonischemic intestine. This indicated that vascular spasm, not reperfusion injury or thrombosis, played the initial role in the progression of transmural bowel necrosis. In addition, the outcome in the viability of the ischemic intestine was not detected by the fluorescence technique but was able to be detected through the mitochondrial morphology under the electron microscope.

Predominant obligate anaerobes invading the deep layers of root canal dentin.

This study was carried out to investigate the presence and types of bacteria invading the deep layers (0.5-2.0 mm from the surface of the root canal wall) of infected dentine of human root canals by sampling with an anaerobic glove box system the split surfaces of eight freshly extracted teeth. More bacteria were recovered after incubation in an anaerobic glove box than after aerobic incubation in air with 30 per cent CO2. Out of 256 predominant bacterial isolated, 205 isolates (80 per cent) were obligate anaerobes. These findings suggest that the environment of deep layers of endodontic dentinal lesions is anaerobic and favours the growth of anaerobes. Among the obligate anaerobic isolates, strains belonging to Lactobacillus (30 per cent) and Streptococcus (13 per cent) were predominant, followed by Propionibacterium (9 per cent). No strains of obligate anaerobic Gram-negative rods were isolated. The microflora of deep layers of infected root dentine is somewhat similar to that of the deep layers of carious lesions in coronal dentine.

[The role of eosinophils, neutrophils and lymphocytes for the development of bronchial hyperresponsiveness in a guinea pig model of asthma].

To elucidate the role of eosinophils, neutrophils and lymphocytes for the development of bronchial hyperresponsiveness (BHR) following antigen exposure, we have developed a guinea pig model of BHR. Guinea pigs immunized by repeated exposure to aerosolized ovalbumin (OA) were intravenously given metopirone, a cortisol synthesis inhibitor, 24 hrs before and 30 min before antigen challenge, and to prevent death from immediate severe bronchoconstriction, chlorpheniramine maleate was also injected. After antigen challenge with high dose of OA, LAR occurred in twelve of fifteen animals (80%) and the bronchial responsiveness to acetylcholine (Ach) was significantly increased. Histologic examination at 72 h showed a significant increase in the number of eosinophils but not neutrophils within the tracheal walls. However, there was no significant correlation between the change in bronchial responsiveness to Ach at 24 h and the number of eosinophil in the tracheal wall at 72 h. When guinea pigs were treated with Cyclosporin A or FK506, T-lymphocyte selective immunosuppressive agents, from the beginning of immunization period, both eosinophil infiltration and an increase in bronchial responsiveness were inhibited. These results suggest that eosinophils and T-lymphocytes may play an important role in the development of bronchial hyperresponsiveness.

[Right ventricular myxoma complicated by paroxysmal atrial tachycardia: a surgical case report].

A rare case of right ventricular myxoma complicated by paroxysmal atrial tachycardia (PAT) is presented. A 13-year-old boy was referred to our hospital because of the recurrent episodes of PAT. Every available medical treatment was tried to reverse the PAT, during which an abnormal mass in the right ventricle was found by echocardiography. Subsequently magnetic resonance imaging computed tomography demonstrated that the tumor developed from the anterior wall of right ventricle with a pedicle. A 10 g of reddish myxomatous mass originating from the base of anterior papillary muscle was resected including the stalk during total cardiopulmonary bypass. The transient paroxysmal atrial tachycardia developed shortly after operation, however, tachycardic episode has never recurred after that. It is noteworthy that intracardiac evaluation is necessary when there is longstanding and inveterate tachyarrhythmia in otherwise healthy children.

Purification of enzymatically active poliovirus proteinase 3C produced in Escherichia coli.

A viral protein 3C of the poliovirus (PV) Sabin 2 strain, a possible core region of the viral proteinase, was expressed in Escherichia coli using a recombinant DNA technology. The protein was recovered as a soluble protein from the insoluble protein fraction of the bacterial lysate, and was purified by a simple procedure with column chromatography. The viral capsid precursor P1 (1ABCD) of the PV Sabin 3 strain, which had been similarly produced in E. coli, was mixed with the purified or crude recombinant 3C. Immunoblotting assay with monoclonal antibodies specific to capsid proteins 1C (VP3) and 1D (VP1) of the PV Sabin 3 strain revealed that the in vitro reaction products contained 1C (VP3), 1D (VP1) and 1ABC (VP0-VP3). The data indicated that processing of the polyprotein P1 by the recombinant 3C proceeded properly in vitro, although an undigested product, 1ABC, is always detected in the reaction mixture. The results strongly suggest that, in addition to a protein 3CD, the 3C protein itself is also catalytically active in the processing of the viral capsid precursor polyprotein P1.

Prognostic significance of epidermal growth factor receptor in esophageal squamous cell carcinomas.

The prognostic value of epidermal growth factor (EGF) receptor level was studied in 32 patients with esophageal squamous cell carcinoma. The EGF receptor levels of tumors were measured by iodine 125 (125I)-EGF binding assay, and the patients subsequently were divided into two groups: a group with high EGF binding capacities (greater than or equal to 2.5% of input), and a group with low EGF binding capacities (less than 2.5% of input). The cumulative survival rates for the two groups were calculated by the Kaplan-Meier method. The generalized Wilcoxon test indicated that the survival rate of the high EGF binding group was significantly lower than that of the low EGF binding group (P less than 0.05). In tumors from two patients with the highest EGF receptor levels, EGF receptor gene amplification was observed. These patients developed mediastinal lymph node metastasis and died 4 and 11 months after surgery, respectively. These results suggest that elevated EGF receptor level is a significant prognostic indicator for esophageal squamous cell carcinoma.

Selective killing of squamous carcinoma cells by an immunotoxin that recognizes the EGF receptor.

We have conjugated a murine monoclonal antibody (B4G7) against the human epidermal growth factor (EGF) receptor to gelonin, a 60S ribosome inactivating protein, via N-succinimidyl-3-(2-pyridyldithio)propionate (SPDP) and 2-iminothiolane. The B4G7-gelonin conjugate bound to the cell surface in proportion to the number of EGF receptors and competed with B4G7 antibody for binding to EGF receptors. The conjugate killed EGF receptor-hyperproducing squamous carcinoma cells (A431, NA, Ca9-22, TE5), and to some extent, human fibroblasts (HFO). It did not kill EGF receptor-deficient small-cell lung cancer cells (H69) and mouse fibroblasts (Swiss/3T3). Free B4G7, gelonin or a mixture of B4G7 and gelonin did not kill A431 cells. The number of EGF receptors was correlated to cytotoxicity at 10(-8) M of the conjugate, and the data were fitted to the regression equation: y = -35.83 log x +233.4 (correlation coefficient = -0.9995). These results suggest that the B4G7-gelonin conjugate may be a useful weapon for targeting therapy to squamous-cell carcinomas.

Two cases of resected esophageal mucoepidermoid carcinoma.

Two cases of resected esophageal mucoepidermoid carcinoma are described herein. Case 1, a 56-year-old man, had an ulcerous lesion of 6.5 cm in length, in the lower esophagus and a small skin tumor of 0.5 cm in diameter, in the forehead. Pathologic studies of both tumors revealed mucoepidermoid carcinoma. This case was therefore considered to be a primary tumor of the esophagus with skin metastasis. The patient was alive and well when last seen, 15 months after his operation. Case 2, a 66-year-old man, had a long ulcerous lesion of 9.0 cm in length, in the mid-thoracic and lower esophagus. The tumor had invaded the aorta and the membranous portion of the left main bronchus, and therefore complete resection was impossible. The patient died of mediastinal recurrence only 3 months after his operation in spite of postoperative irradiation. A review of the literature showed that this tumor has a much greater incidence of rapid recurrence and distant metastasis, regardless of treatment, than usual squamous cell carcinoma of the esophagus. In order to establish and accurate diagnosis of mucoepidermoid carcinoma, alcian blue and/or mucicarmin staining of the endoscopic biopsy specimen should be performed if the tumor contains both glandular and squamous cell carcinoma.

Micronucleus test with procarbazine hydrochloride administered by intraperitoneal injection and oral gavage.

The difference in effect of route of administration of procarbazine hydrochloride (PCZ) in the mouse was investigated in the micronucleus test. PCZ was administered by intraperitoneal injection (i.p.) and oral administration (p.o.) to 2 strains of male mice (MS/Ae and CD-1). On the basis of a small-scale acute toxicity test and a pilot micronucleus test, bone marrow preparations were prepared 24 h after the administration by the i.p. and p.o. routes of 50-400 mg/kg and 200-1600 mg/kg, respectively. The maximum incidence of polychromatic erythrocytes with micronuclei (MNPCEs) was somewhat higher after p.o. treatment in MS/Ae mice and the same with both routes in CD-1 mice. Thus, the clastogenicity of PCZ in mouse bone marrow was revealed by both routes.

Bactericidal efficacy of metronidazole against bacteria of human carious dentin in vivo.

The bactericidal efficacy of metronidazole (MN) against bacteria of carious dentin was estimated by measuring bacterial recovery with or without application of MN to dentinal lesions in vivo. More than 10(3) colony-forming units of bacteria per milligram of sample were recovered from the samples of carious dentin. However, none were recovered from the samples of dentinal lesions which had been covered by alpha-tricalcium phosphate (TCP) cement containing MN in situ for 1 day, 1 month, 1 year and/or 2 years in vivo. Placebo experiments in which carious dentin was covered by TCP cement without MN showed that TCP itself was not bactericidal. Bactericidal efficacy of MN in situ is clearly demonstrated, indicating that MN is a valuable material in caries treatment to disinfect carious dentin.

Culture conditions for efficient recovery of bacteria from infected dental root canals.

For better bacterial recovery from endodontic lesions, several bacterial culture conditions were studied. Samples taken from endodontic lesions of 26 freshly extracted teeth, and the bacterial growth was determined in 11 kinds of growth media under aerobic (in air with or without addition of 5% CO2) or anaerobic conditions (in an anaerobic glove box). Among the media tested, bacteria of endodontic lesions were most efficiently recovered in brain heart infusion medium containing 4% sheep blood (BHI-Blood) under anaerobic conditions. For routine dental practice, serum can be substituted for blood which makes the media opaque, resulting in clear media, and thus bacterial growth can be judged more easily.

[A case of acute retinal necrosis syndrome caused by herpes simplex virus type 1].

A case of bilateral acute retinal necrosis syndrome accompanied by viral meningitis in a 46 year-old male is reported. The characteristic scattered yellowish-white retinal exudates and retinal detachment appeared in both eyes within 2 weeks, and after treatment with intravenous acyclovir and gamma-globulin, and encircling, scleral buckling, pars plana vitrectomy, 100% SF6 tamponade procedures, the retinal detachment was repaired and the yellowish-white retinal exudates disappeared. On virological examination, the complement fixation titer for herpes simplex virus (HSV) in serum increased more than eightfold in pair serum, and the quotient of antibody for HSV type 1 (HSV-1) IgG was high. HSV-1 was isolated and cultured from the subretinal fluid. From these results, it is concluded that HSV-1 infection was the cause of the meningitis and acute retinal necrosis in this case.

[An assessment of extended lymphadenectomy including cervical node dissection for cancer of the thoracic esophagus].

An extended lymphadenectomy including cervical node dissection is one of the most difficult operations, therefore its merits and demerits should be assessed in order to evaluate whether it has the significance of extended radical operation or not. Extended lymphadenectomies including cervical node dissection became to be the standard procedures of lymphadenectomy for cases with cancer of the thoracic esophagus from 1986, and were carried out in 42 cases. Survival rates, disease free survival rates, sites of recurrences and incidences of postoperative complications were compared with the of cases with conventional lymphadenectomy excluding cervical node dissection. Concerning over all survival rates, no significant differences were observed between extended and conventional groups. However only in cases of stage 0 I II, significant difference in survival rates were observed between two groups. Therefore based on our experience, the merit of extended lymphadenectomy was observed only in stage 0 I II cases which had no lymph node metastasis. Concerning the incidence of postoperative complications, no significant differences were observed between two groups, therefore the significance of proceeding extended lymphadenectomy was confirmed.

A phase II study of vindesine in the treatment of esophageal carcinoma. Japanese Esophageal Oncology Group.

A phase II study of Vindesine for esophageal carcinoma was carried out cooperatively by 10 Japanese institutions. Fifty patients were enrolled in the study over a year and ten months. Four patients could not be evaluated because less than 4 weeks had elapsed since their preceding therapy. There were one complete remission and five partial remissions among the 46 remaining patients, a response rate of 13.0%. The patient experiencing the complete remission is still alive more than 24 months after treatment. The main side effects were depilation, anorexia, stomatitis and leukopenia. Thrombocytopenia and elevations of blood urea nitrogen and creatinine were not marked. There was no mortality connected with the administration of Vindesine. It was concluded that Vindesine is useful in the treatment of esophageal carcinoma as a single agent.
