[Asymptomatic hyperparathyroidism. An indication for surgery?].

From 1965 to 1985, 412 patients were operated on for primary hyperparathyroidism (pHPT). Of these, 35 (8.7%) were without specific symptoms and findings. The follow-up investigations after six months to 14 years (median: 31 months) revealed a normalization of the preoperative pathological parameters of alkaline phosphatase, uric acid, creatinine and hypertension in the majority of patients apart from the improvement of their general condition. All patients were normocalcemic. According to these results, the indication for early surgical therapy of the asymptomatic pHPT is to be advocated in order to avoid long-term complications.

Endotracheal and endobronchial tumours in childhood.

Following a review of the literature on the most common types of endobronchial tumours (carcinoid cylindromas and mucoepidermoid tumours), four cases of such bronchial tumours in children are reported. Two of them underwent isolated bronchial resection, whereas one of the others had to undergo bilobectomy in addition and the other, pneumonectomy. The patients remained free of recurrences after a follow-up of 5-20 years. The advantage of circumferential resection and some technical aspects are discussed.

[Echinococcus cysticus of the pancreas in the clinical picture of acute pancreatitis].

A 30-year-old Italian male was admitted to the hospital showing all clinical signs of acute pancreatitis. With the help of computerized tomography, ultrasonic and angiographic methods multiple cysts in pancreatic body and tail as well as close to the hilus of the spleen could be shown. Positive serological tests made the diagnosis "echinococcus granulosus" likely. Resection of the left part of the pancreas as well as resection of the spleen eliminated the parasite. Histologically hydatid cyst was traceable in the pancreatic duct.
