Training-overtraining: performance, and hormone levels, after a defined increase in training volume versus intensity in experienced middle- and long-distance runners.

Performance and hormones were determined in eight middle- and nine long-distance runners after an increase in training volume (ITV, February 1989) or intensity (ITI, February 1990). Seven runners participated in both studies. The objective was to cause an overtraining syndrome. The mean training volume of 85.9 km week-1 increased within 3 weeks to 176.6 km week-1 during ITV and 96-98% of training volume was performed as long-distance runs at mean(s.d.) 67(8)% of maximum capacity. Speed endurance, high-speed and interval runs averaging 9 km week-1 increased within 3 weeks to 22.7 km during ITI, and the total volume increased from 61.6 to 84.7 km. A plateau in endurance performance and decrease in maximum performance occurred during ITV, probably due to overtraining, with performance incompetence over months. Nocturnal catecholamine excretion decreased markedly (47-53%), contrary to exercise-related plasma catecholamine responses, which increased. Resting and exercise-related cortisol and aldosterone levels decreased. Improvement in endurance and maximum performance occurred during ITI indicating a failure to cause an overtraining syndrome in ITI. Decrease in noctural catecholamine excretion was clearly lower (9-26%), exercise-related catecholamine responses showed a significant decrease, cortisol and aldosterone levels remained almost constant, exercise-related prolactin levels decreased slightly. There were no differences in insulin, C-peptide, free testosterone, somatotropic hormone (STH), follicle stimulating hormone (FSH), luteinizing hormone (LH), thyroid stimulating hormone (TSH), tri-iodothyronine (T3) and thyroxine (T4). The decrease in nocturnal catecholamine excretion during ITV might indicate a decrease in intrinsic sympathetic activity in exhausted sportsmen. But it remains open whether this reflected a central nervous system incompetence.

Physical training programs and their effects on aerobic capacity and coronary risk profile in sedentary individuals. Design of a long-term exercise training program.

Currently the prevalent perception is that regular exercise training, besides increasing maximal oxygen uptake (VO2max), may have a beneficial effect on blood pressure and plasma lipid fractions. This is the basis for recommending exercise training for health maintenance and as a part of any prevention program for middle aged and older individuals. The purpose of this work was first, to review whether a basis exists for these recommendations and if so, to secondly design an exercise training program adjusted to these groups. Its parameters should be efficient enough to cause improvements in VO2max and risk profile, yet prevent cardial and orthopedic problems. The structure and the results of exercise programs in 27 experimental studies (n = 1153) were reviewed. The findings indicate that significant changes in VO2max, apart from individuals having least favourable values initially, do not necessarily effect simultaneous improvements in either blood pressure or plasma lipid fractions. Results strongly indicate the permanent benefits of participating in a long-term exercise program. This program should therefore provide a very useful and natural addition to the accepted dietary and pharmacological therapies.

[Isokinetic measurements of the stretch strength of the femoral quadriceps muscle in surfers in comparison to healthy, untrained persons].

The strength of the quadriceps muscle of twelve male surfers who practice their sport for more than 10 years was compared with results of 15 male untrained persons. The concentric peak torque of knee extensors was measured every 10 degrees in a range from 90 to 20 degrees isometrically and dynamically with a constant angle velocity of 10 degrees/sec. The difference in torque between the left and the right leg is not significant, but there is a slight dominance in favour of the standing leg. The body weight related strength of the surfers was 3.2 Nm/kg indicating a higher strength characteristic compared to 2.7 Nm/kg of untrained subjects. Moreover the strength values of our group of athletes were above the value of 3 Nm/kg, which is referred to in the literature as representing a good training status. In view of these results it can be concluded that a long-lasting specific training like surfing will change the strength characteristics of the quadriceps muscles, although none of the surfers had undertaken an extensive strength training for the lower extremity.

[Principles and general practice of a goal-oriented training plan for diabetic patients participating in sports].

Due to a well known experience physical work in patients with diabetes mellitus II is a very important part of the therapy beside diet and medicament treatment. To guarantee a high training efficiency an individual training regimen has to be worked out for the patient. To influence the impaired glucose metabolism the endurance exercise should be carried out at an intensity near the anaerobic threshold. Therefore it is necessary to have a stepwise incremental exercise test including lactate determination after each work load of the patient. Basing on exercise test this paper is designed to give an example of a training regimen in various sport activities (bicycle, running, ski cross country running, swimming) which enables the physician to handle his diabetic patient with an stepwise increasing training program to an optimal training intensity.

Resting echocardiographic parameters after cessation of regular endurance training.

Resting echocardiograms were examined in nonathletic healthy young men (controls, n = 16), in highly trained endurance athletes (n = 20), and in endurance athletes who stopped regular training (n = 40). The relative muscular wall thickness (Rel. MWTd), left ventricular internal diameters both in diastole and in systole (LVIDd, LVIDs), thus also the end-diastolic and end-systolic volumes (LVEDV, LVESV), and the stroke volume index (SVI) were greater in the endurance athletes still in training than in the nonathletes. The ejection fraction (EF), heart rate (HR), cardiac index (CI), and mean circumferential shortening velocity (Vcf) were significantly lower in the athletes. During the 60 days of detraining no change was seen in the Rel. MWTd, LVEDV, LVESV, and HR. The SVI became even greater; EF and Vcf rose up to the control level while CI exceeded it. The cardiovascular regulation is therefore assumed to undergo a peculiar shift during detraining in that a persisting cardiac enlargement and bradycardia is associated with a temporarily unstable autonomous control. This imbalance often leads to a hyperkinesis-like syndrome when an athlete stops endurance training abruptly.

Endurance capacity of trained older aged athletes.

Seven older cyclists (mean age 66.9 years) still engaged in severe endurance training (mean = 8114.3 km year-1) were examined medically, echocardiographically and they underwent a maximal stress test on a bicycle. Mean maximal values were VO2,max = 44.7 ml, Wmax = 261.4 and heart rate = 157.4 beats min-1. The heart volume/max O2-pulse ratio (mean = 46.1) and the TEDD/max O2-pulse ratio (mean = 3.37) were lower compared with age related untrained persons and demonstrate therefore the strong correlation between heart size and function in healthy older aged athletes. Parameters at the anaerobic threshold (lactate concentration 4 mmol l-1) and at the individual anaerobic threshold (lactate concentration 4.08 mmol l-1) showed no significant difference: for heart rate, mean = 133.7; and for W, mean 182.7. The relative oxygen consumption at the anaerobic threshold (mean = 77%/VO2,max) reflect a high endurance capacity of trained old persons, which enables those athletes to outstanding training and competition performance. Derived from that the results show in general the importance of regular endurance training, maintaining an improvement of the cardiovascular system during aging process.

[Echocardiographic values of healthy children in relation to ergometric performance (author's transl)].

The results of echocardiographic and ergometric studies on 22 children are presented. These children were involved in a daily lesson of gymnastics, swimming or rollerskating of one hour duration. Depending on the exhaustion during ergometry, the maximal parameters showed the same results as known from the literature. There were also no different results of echocardiographic values compared with weight or body surface related data of normal subjects. Normal values for echocardiographic data are presented and discussed with parameters of the maximal performance of the children. From that it is concluded that these sport activities do not influence the dimensions of the heart and its function because of the low intensity and short duration of the additional training lessons in contrast to the high spontaneous motoric activities of children.
