Factor VII coagulant, prothrombine time, fibrinogen, prealbumin and a fetoprotein have been evaluated in a patient operated on for right hepatectomy, in order to identify the best markers in prognosing liver regeneration. Factor VII and prothrombine time progressively increased, starting from the 5th-7th day, entering normal range by the 10th and demonstrated a good correlation with both liver scintiscan and ultrasonography. Prealbumin and fibrinogen did not provide any useful information and a fetoprotein levels progressively decreased, demonstrating the complete removal of the neoplastic tissue. Factor VII and prothrombine time seem therefore to be the most reliable markers, among those investigated in this study, of liver regeneration after partial hepatectomy.
The Montgomery T-tube was used in 33 patients over a total of 100 cases undergoing tracheal surgery. Of 21 cases with inflammatory stricture the T-tube was used in 18 in association with resection and end-to-end anastomosis, while in the remaining 3 cases the T-tube was the only treatment carried out. The results were good in 48% of cases, poor in 17% and still to be assessed in 29%. In neoplastic diseases (10 cases) the use of the Montgomery T-tube ensures the patency of the tracheal lumen avoiding the possible, severe asphyxial crises. The major limits of the T-tube appear to be its ineffectiveness against extended tracheomalacia and its possible displacement which may lead to asphyxial crises.

Following a classification of the various types of congenital hernia of the diaphragm, the acute complications that may be caused by such defects of diaphragmatic morphogenesis are reviewed. Relative physiopathology is considered, particularly as regards the thoracic dimension of large hernias of the cupola. As regards visceral hernia, the most typical pictures of the two commonest complications, volvulus and strangulation, are described among other aspects. The clinical situation is outlined bearing in mind the diversity of anatomoclinical pictures in relation to the age of the patient, the type of hernia involved, and the period in which the observation was made. The surgical problems involved in acute complications concern the approach--although agreement is almost unanimous depending on the type of hernia--viscous reduction modalities and the type of plastic surgery which is required on each occasion. Postoperative complications and the prognosis for these hernias are reviewed briefly.

At the turn of the new century, liver transplant procedures can finally be considered an efficient treatment option. Technology has helped transplant intervention become a preferred treatment for patients with progressive and irreversible liver failure. New immuno-suppressive drugs have been introduced which reduce the patient's immunological reaction to the implanted organ, entail minimal side effects and improve practical applications of liver transplantation. As a result of these technological advanced and proper disease management, liver transplant procedures are no longer thought of as an elite therapy, reserved for selected patients with end stage liver disease. In our opinion, it is now a sound and valid surgical option with strictly defined characteristics, indications and well-understood limits. Throughout the past decade, we have studied and applied this type of intervention and have come to terms with its rapid expansion at both the theoretical and practical levels. The most significant obstacle remaining today is the discrepancy between the ever increasing demand and limited supply of organs. The future of liver transplant lies in overcoming this obstacle. Liver transplant practice began at our Institute on 23 November 1990 with the first surgical intervention to replace an organ. In the past 10 years, we have exceeded 200 liver transplant procedures.

Eight patients with inoperable hepatocellular carcinoma were treated by means of percutaneous alcoholization of the malignancy (11 nodular lesions less than 5 cm O). Upon treatment completion they were all given intraarterial injection of lipiodol, which was followed, a week later, by a CT scan. At angiography, during the parenchymal phase, 7 of 11 nodules appeared as avascular areas, whereas in the remaining 4 cases an intense parenchymal effect was seen within the previously treated areas. Lipiodol CT scans revealed intense uptake of oily material in the 4 hypervascular lesions as well as in 1 of those with avascular appearance. In 4 lesions, pathology of bioptic specimens obtained from the areas with contrast pooling was consistent with the persistence of viable neoplastic tissue. In these patients alcoholization had therefore to be continued. Lipiodol accumulation within previously treated nodules has proved to be related to the presence of residual neoplasm. Moreover, in 2 cases, focal retention of lipiodol was very helpful for biopsy under CT guidance. According to our experience, we believe lipiodol administration followed by CT to be very useful in evaluating and staging HCCs. We nevertheless believe that the procedure should be performed only after alcoholization has been completed: this would inform us of treatment effectiveness and subsequently enable us to decide whether treatment can be discontinued.
Eighty-two operations of hiatus hernia repair were performed at the General Surgery Department of the University of Padua from 1966 to 1975. Two cases of iatrogenic stenosis of the the terminal esophagus, whose correction required postoperative dilatations, occurred in this series. This complication should be separated from the transient dysphagia frequent occurring, without radiologia or endoscopic evidence of esophageal lesions, after hiatus hernia repair. The stenosis followed two different repair techniques but, at present time, it is impossible to establish a relationship between occurrence of the iatrogenic lesion and repair techniques used. Eventhough iatrogenic stenosis of the terminal esophagus does not occur frequently, it is a possible complication to keep in mind while performing hiatus hernia repair, because it is exclusively related to defects of the surgical technique.
Carcinoids, mucoepidermoid as well as adenoid-cystic carcinomas represent a group of uncommon neoplasm of the lung. Based on their own experiences in between 1984 and 1988, including 6 cases (4 of which were carcinoids, one was a mucoepidermoid carcinoma and One an adenoid-cystic tumor), the authors have reviewed the existing literature on this subject and related the tumors of the long with those of the trachea, pointing out the different problems associated with the site of origin, the tumor spread, and the histological pattern of these malignancies.

The authors report their experience of 30 cases of inflammatory tracheal stenosis. Of the patients, 53.4% showed postoperative improvement, while in 10% mediocre results were observed (50% reduction of the tracheal lumen). After a second operation, however, the rate of failure was reduced from 26.6% to 13.3%. Postoperative mortality was 10%. An examination of these results demonstrates that tracheal surgery is not always successful at the first attempt and that the complications involved (granulations, recurrent stenosis) require further definition.
