Cortical heterotopia in Aicardi's syndrome--CT findings.

A case of 5-month-old female infant with Aicardi's syndrome is presented. The main clinical features were severe developmental retardation and intractable epileptic seizures. Ophthalmoscopic examination revealed pathognomonic chorioretinopathy. Ultrasonic examination of the brain detected agenesis of the corpus callosum, whereas CT showed a coexisting malformation of the brain, i.e. cortical heterotopia of the gray matter. Agenesis of the corpus callosum is an entity well-recognized by sonography. However ultrasonography is an insufficient modality for the visualization of cortical heterotopia which is common to all cases of Aicardi's syndrome. Therefore, in cases of suspected Aicardi's syndrome CT is recommended, as it enables the diagnosis of cortical heterotopia.

Ultrasonography in hereditary degenerative diseases of the cerebral white matter in infancy.

Ultrasonography of the brain, performed on two female infants who were clinically suspected of having leukodystrophy, demonstrated markedly increased echoes in the periventricular regions bilaterally and indistinct visualization of sulci and gyri on sagittal and coronal scans. The same sonographic features present on subsequent US examinations corresponded to progressive low density areas and atrophy on CT scans. Increased periventricular echogenicity which shows no evolution, in a clinical context, may contribute to the early recognition of neurodegenerative disorders affecting white matter.

Infarction of FSH-secreting pituitary adenoma.

A patient with an FSH-producing adenoma is reported. Two hours after LRH administration (100 microgram iv) the clinical signs of tumour infarction developed. The patient followed a severe clinical course. FSH levels normalised but further deterioration of pituitary function was documented. Worsening of the vision without improvement after surgical resection of the adenoma was observed. The role of LRH testing in adenoma infarction is discussed.

[Computed tomography in the diagnosis of cerebral cysticercosis].

Cerebral cysticercosis was diagnosed in 16 patients by CT, both before and after contrast injection. In six patients the findings were confirmed at operation and histologically. In ten patients, the diagnosis was supported by typical findings on conventional radiographs of the thighs, calves and skull. The CT findings permitted classification of the patients into three types of disease.

Possibilities and limits of computed tomography and angiography in diagnosis of cerebral vascular malformations.

CT findings in 18 patients with cerebral vascular malformations are presented. The following findings were established: 9 angiomas, 4 spontaneous intracerebral haematomas after angioma rupture, 3 aneurysms, 1 aneurysm of vena magna Galeni and 1 megasinus. Contrast medium was administered during every CT screening. All the CT results were controlled by angiography and 10 patients were surgically treated. The authors point out the limits in possibilities of CT to diagnose cerebral vascular malformations. They also discuss the possible errors during CT procedure and deal with the problems of diagnostic differentiation of results. Discussing some cases of cerebral vascular malformations, the authors are trying to evaluate the reliability of the CT diagnostic method had compare it with angiography.

Large and giant intracranial aneurysms. A radiologico-clinical study.

The brain scans and cerebral angiograms of 13 patients with intracranial aneurysms larger than 15 mm. in diameter are analysed. The radiological findings obtained are compared with clinical symptoms and signs. Twelve patients had genuine aneurysms and one patient a posttraumatic false aneurysm. Post-contrast CT in transversal and coronal plane revealed the real size of the aneurysm in 7 cases of intraluminal thrombosis. Cerebral angiography demonstrated the presence of aneurysms, their anatomic relations and haemodynamic changes.

[Current diagnosis and treatment of intracerebral abscess].

Experiences in the diagnosis and treatment of an intracerebral abscess with the aid of computed tomography of the brain (superior to all past diagnostic procedures) have been presented; 28 patients were elaborated and some of them more extensively described, especially those of particular interest due to the abscess' localization and its differential diagnosis.

Comparative study of computed tomography and ventriculography with water-soluble contrast media.

From material of over 10,000 CTs a comparative study of computed tomography and ventriculography with contrast media was carried out. We examined 18 patients with suspected expansive lesions localized in the 3rd ventricle, aqueduct of Sylvius, and 4th ventricle. We have found that CT is the method of choice in these cases and that ventriculography should be reserved for those cases which have clear clinical indications.
