A simplified procedure for the purification of trichosanthin (a type 1 ribosome inactivating protein) from Trichosanthes kirilowii root tubers.

A one step rapid and simple purification procedure for trichosanthin, a type 1 ribosome inactivating protein, from root tubers of Trichosanthes kirilowii has been developed using cation-exchange perfusion chromatography. The identity of the protein has been confirmed by its size, immunoreactivity, and sequence information. Yields of 0.16% of electrophoretically pure trichosanthin from dried root tuber have been achieved with a single 10-min chromatographic step giving the ability to purify gram quantities of trichosanthin in 1 day.

Clinical diagnosis of pediatric obstructive sleep apnea validated by polysomnography.

The decision to perform tonsillectomy and adenoidectomy for treatment of pediatric obstructive sleep apnea syndrome is often made on a clinical basis without formal polysomnography. To examine the accuracy of the clinical diagnosis of pediatric obstructive sleep apnea syndrome, we prospectively evaluated 30 children with obstructive symptoms by a standardized history, physical examination, and review of a tape recording of breathing during sleep. On the basis of this clinical evaluation, patients were divided into three predictive groups: (1) definite obstructive sleep apnea syndrome, (2) possible obstructive sleep apnea syndrome, and (3) unlikely to have obstructive sleep apnea syndrome. Nocturnal polysomnography was used to determine the presence or absence of true sleep apnea. Ten of 18 (55.6%) patients predicted clinically to have definite obstructive sleep apnea syndrome had positive nocturnal polysomnographies. Two of six (33.3%) patients predicted to have possible obstructive sleep apnea syndrome had positive nocturnal polysomnographies. One of six (16.7%) patients predicted to be unlikely to have obstructive sleep apnea syndrome had a positive nocturnal polysomnography. Six nocturnal polysomnographies negative by conventional criteria were suspicious for apnea, but considering these positive for obstructive sleep apnea syndrome did not improve the specificity of the clinical prediction. Our results show that clinical assessment of obstructive sleep apnea syndrome in children is sensitive (92.3%) but not specific (29.4%) for making the diagnosis of obstructive sleep apnea syndrome as compared with nocturnal polysomnography and may contribute to the decision to obtain nocturnal polysomnography in specific circumstances.

Transcriptional and posttranscriptional regulation of inhibin alpha-subunit gene expression in rat Sertoli cells by 8-bromo-3',5'-cyclic-adenosine monophosphate.

FSH is a major regulator of inhibin production in the testis. FSH effects on Sertoli cell inhibin production are believed to be mediated, at least in part, via the cAMP second messenger system. Previously, it has been shown that 8-bromo-cAMP (8-Br-cAMP) stimulates inhibin-alpha mRNA levels. This study examines whether the cAMP-induced increase in inhibin-alpha mRNA levels results from increased alpha mRNA synthesis, decreased degradation of mRNA, or both. The effects of cAMP on inhibin-alpha gene transcription were examined using nuclear run-on assays. Furthermore, the ability of 8-Br-cAMP to drive the transcription of chimeric constructs containing a 2.2-kilobase (kb) segment of the 5'-regulatory region of the alpha gene placed upstream of the coding region of the luciferase reporter gene was also examined. Data from nuclear run-on assays demonstrated rapid induction of alpha gene transcription by cAMP within 2 h and maximal 4- to 5-fold increase within 4-8 h in primary Sertoli cells. Transfection of TM.4 and JEG.3 cells with an alpha (2.2 kb):luciferase chimeric construct (containing 2.2 kb of the alpha gene 5'-flanking DNA) revealed rapid time-dependent induction of luciferase activity by 8-Br-cAMP in these cell types. To examine the effects of 8-Br-cAMP on alpha mRNA stability, cells were pretreated with medium or 50 micrograms/ml 8-Br-cAMP for 24 h before addition of 5 microM actinomycin D to arrest new RNA synthesis, and the decay of alpha mRNA transcripts was assessed over 24 h by Northern analysis and nonlinear regression.(ABSTRACT TRUNCATED AT 250 WORDS)

The modified Pereyra procedure: a clinical and urodynamic review.

OBJECTIVE: To review the clinical and urodynamic outcome of treatment by the modified Pereyra procedure in 93 women with genuine stress incontinence. DESIGN: Retrospective review. SETTING: Harbor/UCLA Medical Center and Los Angeles County Women's Hospital, Los Angeles, California, USA. MAIN OUTCOME MEASURES: Clinical and urodynamic assessment one year after modified Pereyra procedure. RESULTS: Overall 82% of patients were subjectively cured while only 63% were objectively cured. Women with failed surgery had significantly lower pre-operative maximum urethral closure pressures. The procedure had a low operative and postoperative morbidity with no significant disturbance of voiding function noted at one year follow-up. CONCLUSIONS: Our results with the modified Pereyra procedure for stress incontinence showed a significantly lower success rate than has been reported from many previous studies.

Angiofibromas of the postnasal space: a critical appraisal of various therapeutic modalities.

Angiofibromas of the postnasal space are an enigma to the treating surgeon in view of their extensions around the skull base. The availability of modern investigative procedures has to some extent facilitated the decision making process in choosing a suitable surgical approach. Proper tumour staging and the earliest and widest possible exposure tailored to the needs of the specific situation together with adequate blood replacement is the key to success in minimizing the chances of the recurrence. The present study aims at an evaluation of the methodology adopted in 100 serial cases of angiofibromas of the postnasal space. After initial surgery, a further 31 procedures were needed for the management of recurrences.

A rational classification of angiofibromas of the post nasal space.

Angiofibromas of the post nasal space have the potential to extend in all directions along predetermined pathways with a definite mode and chronological pattern. In order to understand their clinical behaviour, methods of treatment and prognosis, a system of clinical staging based on conventional and tomographic observations has been found useful in this series of cases. The classification has been evolved according to the extent of the tumour involvement but also the age of onset, duration, size and area of encroachment including the surgical approach and consequent prognosis. Deficiencies in the earlier classifications are discussed.

Detection of human papillomavirus deoxyribonucleic acid from vulvar dystrophies and vulvar intraepithelial neoplastic lesions.

Vulvar biopsy specimens from 16 patients with gross lesions of the vulva consistent with vulvar dystrophy were examined histologically and for the presence of human papillomavirus deoxyribonucleic acid by southern blot hybridization. None of the nonatypical dystrophies probed positive for human papillomavirus; however, three of five atypical hyperplastic dystrophies (vulvar intraepithelial neoplasia) contained human papillomavirus deoxyribonucleic acid type 16. We conclude that human papillomavirus does not seem to be a causal factor in nonatypical chronic epithelial changes of the vulva.

The conus demyelination syndrome in multiple sclerosis.

Bowel, bladder and sexual dysfunction are common in multiple sclerosis and are generally attributed to the widespread nature of the involvement of the neuroaxis by the demyelinating plaques. Recently we encountered a specific subset of patients within this group who had characteristic clinical complaints of hesitancy, straining and incomplete voiding, perineal hypesthesia on examination, areflexia by cystometry and colonometry and electrophysiological parameters suggesting involvement of the conus medullaris. This heretofore postulated but undocumented mechanism of neurovisceral dysfunction in multiple sclerosis is detailed and discussed in 2 patients in this report.

Evaluation of a portable blood gas analyzer for pediatric interhospital transport.

We evaluated a portable blood gas analyzer for its speed, reliability, and usefulness during interhospital transportation of critically ill children in a prospective study. The accuracy of a portable blood gas analyzer (PBGA) was first established by comparing its results with values obtained from a standard blood gas analyzer. The speed, accuracy, and usefulness of the PBGA were then compared with those of standard analyzers at 10 referring hospitals during interhospital transportation of critically ill children. A highly significant linear correlation was demonstrated between values obtained using the PBGA and those derived from standard analyzers. The time required to obtain blood gas results was 2 min with the portable device, significantly less than the mean of 8.4 min +/- 6.4 min (range 1-24 min) required to get results from the laboratory facilities of the referring hospitals. Modification of treatment or adjustment to mechanical ventilation was required in 30% of transported patients based on blood gas results obtained immediately before departure from the referring hospital. We conclude that a portable blood gas analyzer provides rapid, reliable, and useful data that help to determine therapy for critically ill children awaiting interhospital transport.

Significance of free radicals in chronic tonsillitis.

The present study was carried on adolescents suffering from chronic tonsillitis. Blood (pre and post tonsillectomy) as well as tonsil samples were evaluated for -MDA, SOD and Catalase. Our results showed a decrease in level of MDA and increase in SOD and Catalase levels post tonsillectomy. Presence of MDA and SOD in tonsillar tissue reinforce involvement of oxidative stress in pathophysiology of chronic tonsillitis.
