Rapid pulmonary dissemination in mycosis fungoides simulating pneumonia. A case report and review of the literature.

Mycosis fungoides is a neoplastic disease with extranodal dissemination most frequently to the lung. The roentgenographic and clinical manifestations in these patients may simulate pneumonia and delay appropriate treatment. Disseminated disease is thought to occur in the setting of involvement of the peripheral blood with the neoplastic cells. A case of rapid pulmonary involvement with mycosis fungoides which simulated pneumonia is presented in which there was no peripheral blood manifestation of the disease.

Computerized tomography evaluation of chest wall involvement in lymphoma.

One hundred fifty-nine computed tomographic (CT) scans were performed on 123 patients (90 Hodgkin's disease and 33 non-Hodgkin's lymphoma). Seventeen patients were shown to have involvement of the chest wall. Computed tomographic scans were more sensitive in detecting chest wall lesions than conventional x-rays and clinical examination. This information was important in staging the extent of disease at presentation, guiding radiation therapy, and assessing response to treatment. The anterior chest wall was most commonly affected with or without associated anterior mediastinal disease, but other sites also were involved.

Post-transplant obliterative bronchiolitis and other late lung sequelae in human heart-lung transplantation.

Since March 1981, 19 patients have undergone heart-lung transplantation for end-stage pulmonary vascular disease, with 14 long-term survivors. In five of the survivors, obstructive airway disease has developed with the superimposition of a progressive restrictive ventilatory defect in three of them. None of these five patients showed a tendency for spontaneous improvement of flow rates. Biopsy and postmortem material was available in four of the five patients and showed obliterative bronchiolitis (OB) in three. A fourth patient showed clinical and physiologic data consistent with obliterative bronchitis, but histologic material was not available. Obstructive lung disease without restrictive features developed in a fifth patient, but no histologic evidence of OB was found at transbronchial biopsy. In addition to OB, recurrent lung infections were found in all patients, significant pleural fibrosis in two patients, and bronchiectasis in one patient. Despite these long-term sequelae of human heart-lung transplantation, ten of the 14 surviving patients are leading relatively normal lives.

Computed tomography, lymphography, and staging laparotomy: correlations in initial staging of Hodgkin disease.

One hundred twenty-one patients with newly diagnosed, previously untreated Hodgkin disease underwent abdominal and pelvic computed tomographic (CT) scanning and bipedal lymphography. These studies were followed by staging laparotomy, which included biopsy of the liver, retroperitoneal and mesenteric lymph nodes, and splenectomy. Correlation of the results of the imaging studies with the histopathologic diagnoses revealed a small--but significant--increased accuracy of lymphography compared with CT in assessing the retroperitoneal lymph nodes. The theoretical advantages of CT scanning in detecting lymphomatous deposits in lymph nodes about the celiac axis and the mesentery, or in the liver and spleen, were not confirmed. In part this was due to the relative incidence and the small size of individual lesions at these sites in patients with Hodgkin disease at the time of initial diagnosis and staging.

CT evaluation of cardiophrenic angle lymph nodes in patients with malignant lymphoma.

Two hundred seventy-four computed tomographic (CT) scans of the thorax were obtained in 209 patients with malignant lymphoma (153 Hodgkin disease and 56 non-Hodgkin lymphoma). Fourteen patients (6.6%) were shown to have adenopathy involving the cardiophrenic angle lymph nodes on CT. Of these, only three were considered positive on the basis of chest radiography alone. The anatomy of these diaphragmatic lymph nodes and CT appearance of these nodes when pathologically enlarged are discussed.

Epidemiology of infant mortality in Rio Grande do Sul, Brazil. The influence of agricultural production.

Infant mortality rates (IMRs) for the 24 districts of the State of Rio Grande do Sul, Brazil, for 1972, have been correlated to 12 variables representative of the state's agrarian structure, as well as five other socioeconomic variables, through Pearson's correlation coefficient and multiple linear regression. The IMR was found to be significantly larger in the districts characterized by large properties, cattle raising and wage earning, rather than in the districts of small properties, agriculture and large rural population. The IMR was not found to be correlated to the variables representing education, housing, medical care or availability of banks, but there was a direct association with the sanitation variable. A multiple linear regression equation was then obtained, in which the agrarian variables accounted for 70% of the variation in the IMR, while the non-agrarian ones accounted for 13%. These findings support the hypothesis that the agrarian structure of Rio Grande do Sul may influence the observed distribution of the IMR in the state, while other variables specifically related to the preservation and restoration of health do not seem to play an important role.

Etiologic diagnosis of focal pulmonary infection in immunocompromised patients by fluoroscopically guided percutaneous needle aspiration.

Fluoroscopically guided percutaneous needle aspiration of focal pulmonary lesions was performed in 108 presumed infectious episodes in 82 immunocompromised patients in whom prior diagnostic studies, including transtracheal aspiration, were negative to inconclusive. Two-thirds of the lesions were 4 cm or smaller. Single (61/79) or multiple (18/79) organisms were recovered, for a diagnostic yield of 73% (79/108). Complications were pneumothorax (26%), half of which required tube drainage, and limited hemoptysis (3%). This technique can be quickly performed and frequently repeated with existing personnel and equipment in institutions currently employing it for suspected pulmonary neoplasm, and is a productive study in the evaluation of "opportunistic pneumonia".

Routine full-lung tomography in the initial staging and treatment planning of patients with Hodgkin's disease and non-Hodgkin's lymphoma.

The yield of additional information from anteroposterior full-lung tomograms that changed stage or treatment, in comparison to that obtained from routine chest radiographs, was prospectively evaluated in 243 previously untreated patients with Hodgkin's disease and non-Hodgkin's lymphoma. Although new information was found in 21.4% of all patients, in only 1.2% did these additional data change patient staging. In 3.3% of the other patients the tomograms provided information that affected radiotherapy treatment planning as practiced in our institution.
