[Surgical treatment of lumbar foraminal disk herniation. Value and indications for an interlaminar and extra-articular combined approach].

From 62 foraminal herniations seen over 2 years, the authors present a series of 22 lumbar foraminal disc herniations operated on by a combined interlaminar and extra-articular approach. After describing the technique the results are analysed. The objective of the combined approach is to provide satisfactory foraminal liberation without damage to the articular structures. They also define the indications. These are based on precise analysis of the topography of the hernia, provided by CAT scan examination. Three major topographical types may thus be defined.

Specific selection of osteosynthetic material in the treatment of thoracic or lumbar spinal injuries by the posterior approach. A review of 165 cases.

The authors report 165 cases of thoraco-lumbar lesions with neurological dysfunction. All the patient were operated. They analyze the neurological and mechanical results and indicate the use of different osteosynthesis apparatus according to the type and level of lesions. Harrington's rods seem to give more precise repositioning while Roy Camille's plates give more stability. When the posterior wall of the spinal canal is intact, Kempf's compression rods can be used. Thoraxic spine injuries seem to be an indication for Harrington's rods, while lumbar injuries seem to call for Camille's plates.

[Liver abscess with pleuropulmonary fistula following hepatobiliary ascariasis in a 18-month-old infant. Apropos of a case with recovery for over 10 years].

This study deals solely with the problem of intra-hepatic collected suppuration due to an ascaris migration through the liver ducts. It is due to the infestation of the intestine germs carried away by the worm but not by the ascaris itself which can die calcified inside the liver without hearing of it, provided no mechanical complication has happened, or if it has returned to the intestine environment. When the illness starts with hepatic colic, a pyrexy with a painful right hypochondria and a painful liver-swelling, if the positive diagnosis, that can have been made from the cutaneous allergy test, the R.A.S.T. [Radioallergosorbent Test]. the sonographic diagnosis, the cholangiography and the C.T. scan, can be proved only by curative aiming surgery, mainly in on-endemic countries. The pre and post-operative anti-helmintics are strongly advised to avoid a reinfestation of the illness.

Anterointernal approach to the lumbar spine: technical note.

Lumbar surgery is currently benefiting from the development of anterior approaches to the spine and the possibilities of anterior arthrodesis. The transperitoneal approach is being replaced more often by an extraperitoneal approach that enables anterior access to the spine. The authors report an anterointernal approach to the lumbar spine.

Use of acrylic prosthesis in cervical spine pathology (report of 45 cases).

The authors report on their experience with the use of a reinforced acrylic prosthesis in cervical spine pathology. The study of their own 45 cases enables the advantages and disadvantages of the technique to be demonstrated. A discussion of possible improvements in the technique and its indication completes the report.

[Antero-internal approach to the lumbar spine].

Lumbar surgery is currently benefiting from the development of anterior approaches to the spine and the possibilities of body grafts. The transperitoneal approach is being replaced more a more by an extra-peritoneal approach which enables anterior access to the spine. The authors report an antero-internal approach for all the lumbar spine.

[Colo-duodenal fistula caused by cancer of the right colonic flexure treated by right extended hemicolectomy associated with a mucosal patch using a terminal ileal pedicled graft. Apropos of a case].

A considerable loss of substance of the duodenal wall makes its repair very difficult especially if this exceeds two-thirds of its circumference and is located around its entire second portion. Such a pathological or traumatic localization calls for extensive surgery-cephalic duodenopancreatectomy requiring several gastrojejunal, biliodigestive and the pancreaticodigestive shunts. If straightforward repair was possible, this could lead to stenosis and to a fistula producing complications and the death of the patient through peritonitis and hydroelectrolytic and metabolic imbalance. The mucous patch seems to us the last and only solution after other known techniques.
