Ultrasonic diagnosis of intrauterine foetal growth retardation.

The intrauterine growth retardation (IUGR) is a very frequent pathological entity. After a brief review of the literature of this problem, the AA. expose the ultrasonic techniques that put in evidence the IUGR, and the results obtained. In relation to this diagnosis, the AA. underline the importance of the abdominal diameter (AD) measurement. It is also very important to begin this control from the 28th week on.

Expression and distribution of notch protein members in human placenta throughout pregnancy.

Notch signaling is an evolutionarily conserved mechanism used by invertebrates and vertebrates to control cell fates through close-range cell interactions. Four Notch receptors have been identified in vertebrates and different ligands, divided into Delta-like and Serrate-like (Jagged). Several studies have demonstrated that Notch signaling is involved in different branches of the cell fate decision tree: differentiation, proliferation and apoptosis. These three processes are finely regulated in human placenta in order to allow a successful pregnancy and a correct fetal growth. Moreover, Notch and its ligands participate in the vascular remodelling and stabilization, other two processes much important and ticklish in human placenta. So, we decided to investigate the pattern of expression of Notch-1, Notch-4 and Jagged-1, together with two members related to Notch pathway and involved in angiogenesis: VEGF and p21, in human placenta during gestation by immunoblotting and immunohistochemistry. We showed a modulation of Notch proteins throughout the pregnancy; in particular we showed a slight decrease of Notch-1 throughout pregnancy, with a decreased cytoplasmic staining from the first to the third trimester of gestation in cytotrophoblast and syncytiotrophoblast. In contrast Jagged-1 showed an increase throughout pregnancy especially in syncytiotrophoblast and stroma during the third trimester of gestation. In addition, we found by immunoblotting an increase of VEGF expression from the first to the third trimester and an intense VEGF expression inside endothelial cells throughout the gestation as also confirmed by immunohistochemistry. We also showed a decrease of p21 expression during the pregnancy both through immunoblotting and immunohistochemistry assays. Moreover, we observed Notch localization in extravillous trophoblast cells that are able to invade the decidualized endometrium. Our results suggest an involvement of Notch signaling in regulation of placental cell fate decision and in angiogenesis that are dramatically important to maintain a normal physiology of this organ during pregnancy.

Immunology and fertilization.

The whole fertilization process can be affected by the presence of iso or auto-antibodies or even Fab fragments against specific antigens involved in fertilization like FA1 sperm surface antigen and ZP1, ZP2 and ZP3 oocyte surface antigens. These knowledges can be used to develop a natural vaccine (FA1 seems to be able to inhibit fertilization). The involvement of HLA in the fertilization process is still disputed but it is surely involved. Clinical and experimental aspects are surveyed.

Immune system and endometriosis.

Many experimental and clinical findings indicate a disimmune pathogenesis of infertility associated with endometriosis. The most important modifications of cell-mediated immunity in endometriosis concern macrophage activity, increased both quantitatively and qualitatively. Peritoneal macrophages (PM) acquire the ability of producing lytic enzymes, that would be able to enhancing and stimulating implantation and proliferation of endometrial cells in peritoneal cavity. In these last years many experimental and clinical demonstrations have showed an immunosuppressive action of Danazol similar to corticosteroids. The Authors report their study on a comparison among Danazol 600 mg/daily for 3 months plus a local immunosuppressive treatment combined with superovulation and IPI, Danazol for 3 months plus superovulation and IPI and, as a control group, superovulation and IPI.

The use of GnRH agonists depot for the treatment of dysfunctional uterine bleeding.

The Authors report their experience with the use of GnRH analogue (Goserelin) in 35 women with abnormal uterine bleeding due to low-risk hyperplasia of the endometrium. A hysteroscopic and histopathological follow-up was performed at 2 months and 6 months after treatment. The results show a significantly effectiveness of Goserelin in treating dysfunctional uterine bleeding.

Endometrial ablation: principles and technique.

Uterine bleeding may be caused either by benign organic pathology as well as different dysfunctional conditions. Medical treatment with progestins, danazol or GnRH analogues is usually used as the first choice therapy. Where the symptoms persist, hysterectomy is generally proposed; it has been calculated that from 500,000 to 700,000 such operations are performed annually in the United States. The authors review the state of the art of the endometrial ablation as an alternative to hysterectomy and other medical therapy. Endometrial ablation either by means of the YAG-laser or with the resectoscope, would seem to play a role in well-selected cases of dysfunctional uterine bleeding. In the authors opinion a multicenter study on large numbers of patients is needed in order to confirm these preliminary results.

Lipoprotein patterns in follicular fluid.

Samples of follicular fluid have been obtained from 23 women in order to evaluate the concentration of the different classes of lipoproteins. In the Authors' preliminary experience the determination of these biochemical parameters could help the diagnosis of LUF syndrome.

[Extrauterine pregnancy: epidemiology and etiology].

The aim of the present study is to determine the epidemiological characteristics and etiological factors of ectopic pregnancy. The pool of women at risk is determined by different factors such as contraceptive practices, fertility rate, sterilization, gynecological and pelvic infection. Serological evidence of previous Chlamydia infection was much more common when ectopic pregnancy occurred in abnormal tubes than in normal tubes or compared to intrauterine pregnancies. The influence of endometriosis and ovarian cyst was limited to a very few cases. The analysis of contraceptive habits shows a much higher number of extrauterine pregnancies (45%) when the woman is using progestin-only pill than when she is on combined oral tablets (20%). This higher rate of ectopic gestation can be interpreted to suggest either that the POP might be less effective in preventing ectopic than intrauterine pregnancies or that the POP can delay ovum transport by inhibiting the motility of the fallopian tube.

[Reproductive outcome of hysteroscopic lysis of intrauterine adhesions].

OBJECTIVE: To analyze the reproductive outcome in women undergoing hysteroscopic lysis of intrauterine adhesions, according to their localization and severity. STUDY DESIGN: Retrospective study. MATERIALS AND METHODS: Fifty-three patients affected by intrauterine adhesions of different degree, subdivided according to localization and morphologic aspect: three isthmic, thirteen marginal, fourteen central, moreover twenty were complex and three complete. All the interventions were performed by means of the resectoscope. RESULTS: Hysteroscopic surgery restored an acceptable menstrual cycle in almost all the patients affected by intrauterine isolated adhesions, in 52% of women affected by complex incomplete adhesions, and in none of the three patients with entirely obliterated cavity. Concerning fertility, while in isolated, isthmic, central or marginal synechias we observed a pregnancy rate of 73.3% (22 out of 30) with a pregnancy rate to term respectively of 63.3% (19 out of 30 cases) and of 86.3% (19 out of 22 total pregnancies), in case of complex but not complete adhesions we reported, on 20 cases, 5 pregnancies (25%) with only two gone to term. We had no pregnancy in three cases of complex synechias. CONCLUSIONS: The basic parameter to define the functional and reproductive prognosis of the hysteroscopic lysis of intrauterine adhesions is not the menstrual profile or the histological characteristic of the lesions, but rather their extension.
