[Reduction in mean cerebral blood flow measurements using 99mTc-ECD-SPECT during normal aging].

Mean cerebral blood flow (mCBF) was measured by SPECT using the 99mTc-ECD-Patlak-Plot method in a selected group of 61 normal non-hospitalized subjects aged 51 to 91 years. The mCBF values showed 48.4 +/- 4.7 ml/100 g/min in aged 50-59 years group, 49.9 +/- 5.9 ml/100 g/min in aged 60-69 years group, 46.4 +/- 6.5 ml/100 g/min in aged 70-79 group, 38.0 +/- 3.7 ml/100 g/min in aged 80-89 years group, 38.9 ml/100 g/min in aged 90-99 years group. There was a statistically significant reduction of mCBF with advancing age (R = -0.41; p = 0.001). Women have significantly higher mCBF values than men up to aged 70 years group. In this study, there was no significant laterality in the mCBF between right and left hemispheres in all decade groups. The history of hypertension, alcohol consumption, and cigarette smoking failed to show significant difference in the mCBF values. The present study shows that normal aging is associated with mCBF reduction.

Systemic venous aneurysms.

A case of superior vena cava aneurysm combined with aneurysm of the left innominate vein is presented. For the diagnosis, it is important not only to apply imaging techniques such as superior vena caval angiography, radioisotopic angiography, and computed tomographic scans but also to have a concept of this entity in mind.

[Serum and tissue concentrations of UFT in patients with lung cancer].

Postoperative serum and tissue concentrations of 5-FU, FT-207 and uracil were measured in 36 patients with lung cancer who were administered UFT for seven days preoperatively. The concentration of 5-FU was high in tumor tissue and lymph nodes, but very low in serum. Such differences were not observed in the FT-207 levels. Tumor concentration of 5-FU in patients administered daily doses of 600 mg was 0.151 +/- 0.099 microgram/g which was three times higher than the minimum inhibitory concentration, and higher than that seen with other doses. The histological type and T factor were not related to the tissue concentration of 5-FU. Lymph node metastasis was not related to the concentration of 5-FU in the lymph nodes. The optimal daily dose of UFT for patients with lung cancer was considered to be 600 mg.

A seven-month survival of a calf with an artificial heart designed for human use.

A Jarvik-7 type of pneumatic artificial heart, which was specifically designed to fit the anatomy and hemodynamic requirements of human patients, was implanted in a calf in an experiment to test the hemodynamic performance of the artificial heart. The experiment lasted for 221 days, longer than any animal had ever lived without its natural heart, despite the fact that the calf increased its body weight to 171 kg. The calf showed typical signs of low cardiac output before its death, but the direct cause of death was intestinal bleeding. At autopsy, it was discovered that the low cardiac output was due to severe pannus around the left inflow valve, as diagnosed earlier by changes in the pneumatic pressure wave form.

Preclinical evaluation of intra-aortic balloon pumping for pediatric use.

Small intra-aortic balloons (0.75 ml to 5.0 ml) mounted on small catheters (4.0F and 5.0F) have undergone in vitro testing with the Utah Heart Driver and in vivo evaluation with 3 different pumping modules. In small animals with reduced cardiac output from acutely produced mitral regurgitation, intra-aortic balloon pumping effectively reduced aortic peak systolic pressure, lowered aortic end-diastolic pressure and reduced left atrial pressure. In addition, the Datascope system most effectively augmented diastolic pressure and increased aortic forward and negative flow as compared with 2 other pumping modules. The total system has been adequately and experimentally evaluated and is ready for clinical use in infants and small children.

Recovery of cardiac function with total transapical left ventricular bypass.

Transapical left ventricular bypass which withdraws blood directly from the left ventricular apex through a roller pump and outflow filter with return to the femoral artery has been applied in 6 patients with cardiac failure. All patients required total TALVB (with the aortic valve closed) for periods of one to 144 hrs, and 2 patients had recovery of life-sustaining myocardial function, allowing removal of the TALVB system. One patient had complete recovery without other organ dysfunction. Total TALVB continuously decompresses and unloads the left ventricle throughout systole and diastole, and maintains the circulation in otherwise terminal circumstances.

Reoperative surgery in calves with a total artificial heart.

Two surgical techniques have been developed in our laboratory to deal with identifiable problems in long-term artificial heart experiments. A right throacotomy is used to deal with problems such as extensive bleeding, which occur in the immediate postoperative stage of the experiment, while a left thoracotomy is used in cases in which the original implantation is preceded by more than one week, since extensive adhesions complicate the right thoracotomy at that stage. Pulmonary problems have been eliminated as primary cause of difficulties after reoperation, but infection remains a serious problem.
