MRI in the detection of malignant infiltration of bone marrow.

Magnetic resonance imaging (MRI) at 0.35 T with a superconductive magnet was performed on 80 patients with known or suspected malignant disease of the bone marrow. The group comprised 50 patients with known primary malignancy and 30 with known multiple myeloma. The MRI scan was correlated with plain films and radionuclide bone scans. In 40 patients with suspected metastatic disease, areas of decreased signal intensity on T1-weighted spin-echo images were observed. Ten patients had no MRI evidence of metastasis, and the abnormalities suspected on bone scanning were shown to be due to other causes. All the myeloma patients had abnormalities demonstrated by MRI. This was significant, since most had normal bone scans. All diagnoses were confirmed by needle biopsy. MRI was shown to be a sensitive method of detecting areas of malignancy within the bone marrow toward which biopsy could be directed.

Ultrasonographic findings in acute acalculous cholecystitis.

Acute acalculous cholecystitis (AAC) is usually seen as a complication of major surgery or trauma. Although this entity is well-known in the surgical literature, little has been written about it in the radiologic literature. A review of patient records from 1975 through 1982 revealed 16 patients with pathologically confirmed AAC on whom at least 1 sonographic study had been performed. Thickening of the gallbladder wall, a subserosal "halo" of edema, pericholecystic abscess, and marked gallbladder distention were consistent findings in AAC. In the proper clinical setting, these otherwise nonspecific findings allow a prompt and accurate diagnosis.

CT appearance of omental packs.

Liver lacerations may be surgically treated by using the omentum as packing. A radiologist can differentiate potential postoperative complications, such as abscess, hematoma, and biloma, from these packs on computed tomographic scans by noting attenuation values.

MR imaging of hepatic plasmacytoma: a case report.

A case of kappa light chain multiple myeloma with space-occupying hepatic plasmacytoma is presented. Magnetic resonance (MR) imaging shows an eccentric multiple-layer pattern of the liver mass and suggests malignancy. Differential diagnosis concerning the MR appearance of this rare hepatic tumor is discussed.

Uterine rupture secondary to trauma: CT findings.

We describe a case of a ruptured gravid uterus, secondary to motor vehicle accident, in a 20-year-old woman. Maternal and fetal injuries associated with blunt trauma are described along with the use of CT in pregnant patients.
