[Value of computed medical imaging in the surveillance of neo-adjuvant chemotherapy of osteogenic osteosarcoma in children and adolescents. 51 patients seen from 1984 to 1986].

The value of quantitative bone scintigraphy, digitised angiography, CT scanning and magnetic resonance imaging in the follow-up of neo-adjuvant chemotherapy for osteogenic osteosarcoma was assessed in 51 patients between 1984 and 1986. Bone scintigraphy was a very sensitive method of detecting bone metastases but of limited value in assessing the response to preoperative chemotherapy. CT scanning was very useful in small and medium sized tumours with predominantly non-calcific involvement of the soft tissue. At present, digitised angiography seems to be the best investigation for following up these patients as shown by the close histo-angiographical correlations. However, magnetic resonance imaging is a very promising method and may in future replace the more invasive aforementioned techniques in this indication.

[Pre and postoperative chemotherapy of osteosarcoma with an adriamycin-cisplatin combination. Risks of a neoadjuvant chemotherapy which is not sufficiently effective].

The authors evaluated a new protocol of neoadjuvant chemotherapy for osteosarcoma, easier to manage and different from T10. The good results obtained with the postoperative ADR-CDDP association led us to undertake a pilot study between 1982 and 1984, using ADR-CDDP as preoperative chemotherapy. The records of sixteen patients were available for follow-up. The average age of the patients was 19.9 years. Patients received two or three preoperative courses, and a total of six identical courses. Tolerance was good. Pain usually disappeared but this was often misleading because associated with radiological and/or clinical tumor progression, low histological necrosis or poor outcome. The continuous disease-free survival actuarial rate was less than 57 and 40% at 18 months and two years respectively. The actuarial survival rate was 87% at one year and 65% at two years respectively. Disappointing results of this preoperative protocol, compared to results with the SO4 78 or T10 protocols for example, led to publish these data early in order to underline their potential dangers. As a result, we stopped our study. The charter of pilot studies justifies this publication. As well, these data point out the necessity of very close follow-up of neoadjuvant chemotherapy by sophisticated medical imaging. Neoadjuvant chemotherapy, if ineffective, must be stopped early, and should lead to surgery, followed by adequate postoperative chemotherapy.

[Magnetic resonance imaging of tumors of the knee using surface coils].

Fourteen patients presenting with a primary bone tumour of the knee have been assessed, in parallel with other imaging techniques, by examination using MRI with surface coils. The first examinations were made before any biopsy was made or chemotherapy given so as not to impair the interpretation of the images and to provide guidance as to the correct site for biopsy. Two sequences were made - a longitudinal sequence made with echo-spin, echo-time = 30 ms, repetition-time = 500 ms, with four impulses and a transverse sequence made with echo-spin, echo-time = 40 ms, repetition-time = 1300 ms with two impulses (+/- multi-echo). Thanks to the excellent spatial resolution using a surface aerial, we have been able to determine not only the intramedullary extent of the tumour and its relationship with healthy muscles and vessels but also, in a much more precise way than previously, evidence of extension to the skin, to the epiphysis, to the articular cartilage and to the synovium, thus allowing an objective assessment to be made of the possibility of a closed block-excision of the joint. A second MRI examination with cranial coil was made pre-operatively at the end of adjuvant chemotherapy to determine the precise progress of the tumour, and in particular, to discover the localisation of the tumour in the femoral and tibial shafts, the investigation being aided by systemic scintigraphy.

[NMR examination of soft tissue tumors].

Preliminary findings on NMR imaging of 30 soft tissue tumors demonstrated the indispensable value of this examination (particularly when a surface antenna is used) for preoperative investigation and diagnosis of tumoral recurrence when compared with other radiologic techniques. The possible potential of NMR imaging for characterization of tissues, apart from lipoma or liposarcoma, cannot be evaluated at the present time.

[Neonatal syphilis despite erythromycin treatment of the mother].

A one-month old infant whose mother had been treated with erythromycin during pregnancy exhibited signs of severe congenital syphilis with collapse requiring admission to an intensive care unit. Erythromycin has low placental transfer and other treatments would have probably been more adequate. Some authors advocate the use of the latest tetracyclines and doxycycline could also be administered. The WHO's recommendations that all children born of mothers who were not treated with penicillin should receive this antibiotic after birth is still valid.

[Neonatal hemolysis secondary to congenital poikilocytosis].

Hereditary pyropoikilocytosis is a congenital haemolytic anaemia recently described. A new case is reported in which the condition was diagnosed by a study of erythrocyte membrane proteins in the parents. The unusual clinical features of this case lead to a discussion of the relationship between hereditary pyropoikilocytosis and other rare forms of elliptocytosis in children.
