Long-acting local anaesthetics in oral surgery. Clinical evaluation of bupivacaine and etidocaine for mandibular nerve block.

2 long-acting local anaesthetics, bupivacaine and etidocaine, have been compared to lidocaine with regard to effect during surgical removal of impacted mandibular third molars and effect upon the immediate postoperative pain. It can be concluded from this study that both bupivacaine and etidocaine as well as lidocaine were highly effective and without significant difference with regard to pain blocking during surgery. Both long-acting solutions were significantly superior to lidocaine in providing an extended duration of postoperative pain control. Comparing the 2 long-acting solutions, patients in the bupivacaine group had a significantly longer pain-free period than patients in the etidocaine group.

Group practice in Sweden.

The Swedish design of group practice (Praktikertjänst) is described. After 25 years there is now a model which suits most dentists in Sweden. Almost 50 per cent of the private practitioners belong to the corporation. It is owned by practitioners and operates as a conventional free enterprise business. The practitioner owners of the company are at the same time employed in it. The corporation consists of 1900 dentists, 675 physicians and their assistants. The basic aim is to give the dentist more time for the provision of dental care. Thus the corporation helps the dentist with most of the financial and administrative work arising in the practice. The central office also gives a variety of services, eg continuing professional education; financial help for research; advice and discounts on purchases; private loans; and vacation rentals. Furthermore, the corporation provides a pension fund, an insurance department and a publishing house for scientific medical and dental literature. Compared with practitioners working outside the Praktikertjänst its employees have great advantages.

The gigantiform salivary calculus.

The size of salivary calculi may range from small particles to large concrements of several centimetres. One case of a gigantiform salivary calculus located in the Wharton duct of a 48-year-old man is presented. After surgical removal of the 3.6 cm long concrement, it was split into 4 parts. The different parts were used for routine histological studies, production of microradiographs of ground sections and for scanning electron microscopy. Decalcified sections disclosed a specimen made of a homogeneous central nucleus and a peripheral lamellation. The same morphological picture was seen on the microradiographs, where the nucleus showed dense mineralization, and the peripheral part alternating rings of high and low mineral content. Microorganisms, mostly thread-like organisms, were found throughout the entire calculus in specially stained sections. Scanning electron microscopy confirmed the histologic and microradiographic findings. The presence of microorganisms also in the nucleus of the calculus seems to be one important aetiological factor favouring the formation of this gigantiform concrement.
