[Revascularization in the skin loss with skull fornix bone exposure].

Experience of 69 patients treatment, suffering the soft tissues death with uncovering or partial necrosis of the skull vault bones, occurring after burns and mechanical injuries was summarized. Multiple milling (4 patients) of the uncovered bones is dangerous, lacks efficacy, leads to osteomyelitis and the skull vault defects development. The skull bones revascularization with cutaneo-subcutaneous, complex-composite flaps bringing nutrition from outside was proposed and accomplished. Revascularization methods are depicted, their results are presented in 65 patients. The revascularization conduction in term to 40 days after the injury occurrence permits to reduce the time of treatment, to avert the skull vault bones necroses, to gain good functional results.

[Diagnosis of endogenous intoxication, prognostication of the course of pathological process and principles of the establishment of a treatment program for patients with severe burns].

The experience of treatment of 300 patients with burns was summarized. The ways of development of diagnostics and treatment system for endogenous intoxication syndrome in injured persons with burns are proposed. The elaborated test-system permits to define the endogenous intoxication severity and the degree of functional possibilities suppression of antitoxic resistance cytological and humoral factors. The differentiated detoxication programs are compiled.

[Age factors in natural mechanisms of detoxication and curative effect of enterosgel in burns].

In the experiment, the age peculiarities of the detoxication system of the liver in burn disease and the effect of enterosgel on them were studied. Decrease in hydroxylase and demethylase activity of hepatic microsomes in relatively stable activity of oxidoreductase as well as increase in activity of aminotransferases, especially that of alanine aminotransferase, which was more pronounced in aged animals, was noted. Under the influence of enterosorption, the increase in functional activity of monooxygenase system of the liver and stabilization of hepatocytic membranes are more pronounced in young animals. This contributed to reduction in lethality, activation of natural mechanisms of detoxication and reparative processes.

[State, effectiveness and improvement of the quality of care for burn patients].

The data on the incidence of burns in Ukraine, provision with a specialized hospital stock, level of specialized aid, terms of transfer of the burned patients from the central district hospital, central city hospital, specific weight of the operative interventions performed for thermal injuries, general operative activity, lethality both of the adults and children with burns are presented. The ways for improving the quality of a specialized aid are outlined, new techniques of skin plasty operations are suggested. Particular attention is given to quality of rendering help in central district hospital, central city hospital. The ways for reduction of lethality in burns are shown. The importance of conservative and operative rehabilitation of burn convalescents, possibility of essential enhancing their social and vocational rehabilitation are stressed.

[Specifics of dermotensive skin reconstruction in children].

The technique of dermotension, peculiarities and expediency of its use in postburn and mechanical skin deformation in 50 children ranging in age from 2 to 14 years are described. It is expedient to perform operative elimination of the postburn alopecia, severe contracture of the neck, cicatricial bandage of the chest and abdomen by means of the dermotensive flaps. This permits to reduce a number of operative steps, create a sufficient reserve of healthy tissue identical to the lost one both in texture and in colour. Dermotension has the advantages over the pediculate plasty and can be widely used in children with the burn sequelae.

[Plastic surgery using skin and adipose tissue flaps in burns and their sequelae].

The principles of cutaneous plasty in IV degree burns with uncovering, or primary death of the deep structures (tendons, bone, joints) are substantiated. The necessity and possibility to perform early plasty with fatty-cutaneous flaps irrespective of character of the inflammatory process and degree of microbial contamination of the wounds are proved; peculiarities of its different methods (use of interpolated and sliding flaps, Filatov's graft, "Italian plasty", dermotension, plasty with the pedicle graft, free transfer of the fatty-cutaneous flaps) are noted. On the basis of the experience with plasty of more than in 250 sufferers, the frequency of performing its separate types, effectiveness, results of social and vocational rehabilitation of the patients are presented.

[Characteristics of healing of wounds after hemosorption in the complex treatment of severely burned patients].

Clinical and morphological peculiarities of healing burn wounds with the help of hemosorption in therapy of critically burned patients were studied. The data obtained have shown positive effects of the method on the course of wound processes. The formation of crust, demarcation zone, granulation tissue, processes of fibrillogenesis and collagenogenesis accelerated. Epithelization of burned surface occurred 3-4 days earlier than in patients without hemosorption and the surface area of total epithelization of the burn surface was 15-20% greater.

[Changes in the structure of postburn keloid scars before and after cryotherapy].

Biochemical and electron microscopy study of the collagen molecule from 14 biopsy specimens of the postburn scar (in 9 patients before and in 5 after cryoeffects) revealed that the postburn scar contains I, II, III and VIII collagen types changed according to the amino acid composition. It was found that after cryogenic effects on the scar there was a tendency to normalization of the collagen structure.

[The efficacy of the extracorporeal connection of a porcine spleen in early and late septic complications in patients with burns].

Extracorporeal connection of a porcine spleen (ECCPS) was used in 36 patients ranging in age from 14 to 60 years with a IIIAB-IV degree burn of from 30 to 80% of a body surface. The clinical signs of infective complications were the indication for ECCPS. The immunomodulating effect of ECCPS in burn septicotoxemia and normalizing effect on the morphofunctional state of neutrophil granulocytes without normalization of cellular immunity in toxemia was established.

[Pathogenesis and principles of directed therapy in the acute period of burns in children].

The specific character of the development and course of acute period of the burn disease in children is shown. The schemes for transfusion and medicamentous therapy, depending on the severity degree of thermal injury and age of a child, are given, the degree and role of immunologic disorders are shown, the methods for their correction are presented, the indications for different methods of detoxication (hemosorption, enterosorption, applicative sorption, intestinal lavage etc.) are defined. Of 293 children treated, 18 (6.2%) died.
