Management of common breastfeeding issues.

Breastfeeding provides ideal nutrition, growth hormones, and antibodies that change over time as growing infants' and children's needs change and provides these inexpensively, with no harm to the environment. Breastfed infants are healthier than other infants overall, and research indicates that the health benefits may continue on into adulthood. Increasingly, women are choosing to initiate breastfeeding in the hospital, but the attrition starts early and is dramatic. For women to meet their breastfeeding goals, physicians must not only give lip service to "breast is best" but also become knowledgeable in breastfeeding management and actively promote breastfeeding in their practices and in their communities.

Donor human milk for preterm infants.

As survival rates for preterm infants improve, more attention is being focused on improving the quality of survival through optimal nutritional management. The benefits of human milk for term infants are well recognized, with current research suggesting that human milk may especially benefit the preterm infant. Some mothers are unable or unwilling to provide breastmilk for their infants. Although not as well studied as mother's own milk, pasteurized donor human milk can provide many of the components and benefits of human milk while eliminating the risk of transmission of infectious agents. Pasteurization does affect some of the nutritional and immunologic components of human milk, but many immunoglobulins, enzymes, hormones, and growth factors are unchanged or minimally decreased. In California donor human milk costs approximately $3.00 per ounce to purchase. A reduction in length of stay, necrotizing enterocolitis and sepsis may result in a relative saving of approximately $11 to the NICU or healthcare plan for each $1 spent for pasteurized donor milk.

Truncus arteriosus associated with trisomy 18.

The spectrum of cardiac anomalies in trisomy 18 typically includes septal defects and polyvalvular disease and only rarely complex malformations. We report the first case of trisomy 18 with truncus arteriosus type II.

Examining the "cost" of substance abuse in pregnancy: patient outcomes and resource utilization.

OBJECTIVE: This study investigated the clinical and financial impact of self-reported maternal drug history and documented intrauterine substance exposure on maternal-neonatal morbidity and hospital costs. STUDY DESIGN: This two-part, case-controlled, retrospective study used matching control groups. RESULTS: Among women reporting a history of substance abuse during or prior to the index pregnancy, (a) maternal hospital costs were significantly higher and more variable; (b) birth weight, length, and gestational age were lower; (c) no significant differences were noted in the number of maternal risk factors or neonatal complications and hospital costs. In comparison of neonates with positive toxicology screens and a matched control group, there were no differences in neonatal outcomes or costs, but the number of complicating maternal risk factors and maternal hospital costs were significantly different. CONCLUSION: Knowledge of maternal substance abuse history may be useful in planning for maternal-neonatal care needs and reimbursement for hospital care for this at-risk patient population.
