Murine survival of lethal irradiation with the use of human umbilical cord blood.

We have found that human umbilical cord blood (HUCB) will routinely protect mice exposed to lethal levels of irradiation. At the end of 50 days, over seventy percent (70%) of mice injected with HUCB survived 900 cGy or irradiation, which produced 100% deaths in the uninjected control animals. Moreover, there was some evidence that human colony stimulating factors further improved survival. Anti-Natural Killer cell (NK) antibody was utilized along with HUCB in these studies, however, Anti-NK cell serum alone had no radioprotective effect in mice. The studies reported here suggest the possibility of utilizing HUCB for immediate protection of humans from lethal irradiation.

Unexpected changes in urinary catecholamines and vanillylmandelic acid following rape assault.

Although psychological changes are recognized to occur in rape assault survivors there is no information on the biochemical changes in these victims. This study compares urinary catecholamines and metabolites in 17 rape victims to two female control groups (one of which engaged in normal sexual intercourse and the other did not). We found, in the rape victims, unexpected changes in the excretion pattern of catecholamines and metabolites as compared to the various control groups. The most significant difference was the dramatic increase in urinary conjugated dopamine (P less than 0.01) in the rape victims which remained elevated for over 24 hr. Urinary vanillylmandelic acid (VMA) rose significantly in rape assault victims when compared to the normal control group. The VMA levels in rape victims were significantly lower, however, than in the women who had undergone (normal) sexual intercourse (P less than 0.01). Urinary free epinephrine showed a marked decline and remained depressed for over 24 hr in the rape assault victims (P less than 0.01) compared to normal controls. Some possible reasons for these patterns in catecholamines and metabolite excretion are suggested. These changes may be of importance in the poststress syndrome that occurs following the rape assault. In summary, a different profile of catecholamine and metabolite excretion patterns was found in rape compared to normal sexual intercourse. The enhanced dopamine excretion is contrary to the expected change of enhanced epinephrine secretion in severe stress.

Production of human to mouse xenografts by umbilical cord blood.

Utilizing human umbilical cord blood, it has been possible to create in irradiated animals a human to mouse xenograft. To facilitate hematopoietic reconstitution, SJL/J mice, which are functionally low in natural killer (NK) cells, were treated with anti-Asialo GM1 antibodies (anti-NK) and irradiation prior to injection of cord blood mononuclear cells. In contrast, SJL/J mice with the "beige" (bg/bg) mutation, which confers a functional NK cell deficiency, required only irradiation for successful transplantation. Human cells, detected by means of DNA probes, were demonstrated in the lungs and lymph nodes of irradiated animals up to 6 months after injection of the human cord blood cells.

Measurements of postcoital sympathetic activity in females by means of vanillylmandelic acid.

Urinary vanillylmandelic acid (VMA), the final metabolite of epinephrine and norepinephrine, and creatinine (Cr) were measured 1 hr before, within an hour after intercourse, and in a 23-hr pooled sample after intercourse in 11 females. The results were compared to a group of 8 normal females who abstained from intercourse. The control VMA/creatinine ratios were significantly lower than the preintercourse values (P less than 0.01) as well as the postintercourse and 23-hr pooled postintercourse sample. This ratio, 1 hr postintercourse increased significantly (P less than 0.01) over the preintercourse values. The pooled 23-hr postintercourse sample remained high and was significantly different from preintercourse but not from the postintercourse sample. The absolute values of VMA showed relatively similar significance. This study provides objective evidence in the female for the considerable involvement of the sympathetic system during sexual intercourse. In addition, it shows that the group that was planning to have sexual intercourse, 1 hr prior to intercourse, already had VMA/creatinine ratios double over the control group. Such objective information has hitherto not been available.

Fetal cord blood's potential for bone marrow transplantation.

Approximately 18 years ago, the authors were able to produce an apparently successful bone marrow transplant by using umbilical cord blood. In view of the Chernobyl disaster and the subsequent problems of treatment with marrow transplantation, this study undertook to explore further the potential use of umbilical cord blood as a source of marrow cells. Specimens of umbilical cord blood were collected from 13 routine obstetrical deliveries. All specimens grew erythroid and granulocytic-monocytic colonies. The formation of these various hematopoietic colonies from umbilical cord blood was at least equivalent to bone marrow, and in some instances over 5 times more effective. There appeared to be a statistically significant correlation between the numbers of colony-forming units (CFU-E) and the male infants. The weight of the infants also showed a statistically significant correlation with the burst forming units, erythroid (BFU-E) and the granulocytic-monocytic colony (CFU-GM). The BFU-E also appeared to be greater in number when the time between collection and plating was shorter.

Acquired immunodeficiency syndrome-associated renal disease in children.

Five children with acquired immunodeficiency syndrome (AIDS) and clinically significant renal disease had detailed pathologic examination of renal tissue (biopsy specimens, autopsy specimens, or both). All patients had proteinuria, hypoalbuminemia, and edema; one patient had persistent azotemia. In two cases, renal disease was the first manifestation of human immunodeficiency virus (HIV) infection. All patients had progressive renal disease, and four of the five died. Pathologic studies revealed focal glomerulosclerosis and mesangial proliferative glomerulonephritis with deposits of immunoglobulins and complement demonstrated by immunofluorescence and electron microscopy. Characteristic tubuloreticular structures were also demonstrated in the glomerular endothelial or epithelial cells in two cases. Renal disease is part of the multisystem involvement in children with AIDS. The pathogenesis of renal disease is not known, but circulating immune complexes are known to occur in children with HIV infection and may be involved.

Elevated creatinine levels in rape assault survivors.

Urine specimens were collected from alleged rape survivors. In 13 of the 29 survivors, creatinine levels were over 150 mg/dl, and nine of these were over 200 mg/dl. These elevated urine creatinine levels persisted for some time after the alleged rape attacks. Various controls yielded overall levels below those of the alleged rape assault survivors. When control values of creatinine and osmolality of pre- and postintercourse samples were plotted and compared by linear regression covariance analysis to the data on alleged rape survivors, the levels of urinary creatinine in the alleged rape survivors were higher than would have been expected from urine osmolality increases. The dissimilarity of the two sets of data has a p value less than 0.0001. These findings might serve as supplemental objective evidence of rape (or terror) that could be used medicolegally.

Sarcoid and cytotoxic lung antibodies.

Fifteen cases of Sarcoidosis were investigated for the presence of complement dependent cytotoxicity against human lung cells grown in tissue culture. Significant levels were found in eleven cases. Absorption studies revealed that these antibodies associated with sarcoidosis could be absorbed by human lung fibroblasts but not by fibroblasts derived from human foreskin or kidney. Although we found a similar antibody frequently associated with extrinsic asthma we have only rarely found these antibodies in cases of tuberculosis, carcinoma of the lung, emphysema and intrinsic asthma.

Antibodies to synovial-derived cells in patients undergoing artificial prosthesis implants.

In this preliminary study, the authors have found cytotoxic complement-dependent synovial antibodies in the serum of patients undergoing prosthesis implantation; these were particularly evident in those patients undergoing revision surgery. In order to demonstrate the synovial antibodies, it was necessary that the authors modify the method they had previously used in their studies of cytotoxic lung and kidney antibodies. While conventional trypsinization dispersion produced synovial target cells that would not react in the test system, mechanical dispersion successfully produced target cells sensitive to complement-dependent cytotoxic antibodies. In this study, synovial antibodies reacted similarly to cells derived from the synovium of different individuals, but they did not react to cells derived from tissues other than synovium. This tissue specificity was confirmed by absorption studies that indicated the synovial antibodies to be organ specific. Whether these cytotoxic synovial antibodies can be predictive of the impending loss of an artificial prosthesis, or somehow be directly involved in the failure mechanism of that prosthesis, will require additional studies.

The HLA-A, -B, and -DR phenotypes and tuberculosis.

The HLA-A, -B, and -DR antigens were studied in black Americans to analyze deviation in phenotype distribution between ATS Class 3 patients with tuberculosis disease and ATS Class 2 healthy persons with significant tuberculin reaction, but no tuberculosis. Statistical analyses with Fisher's exact test revealed a significant difference in frequency distribution of HLA-B5 (p = 0.046), HLA-DR5 (p = 0.0276), and HLA-DR6y (p = 0.0095) between ATS Class 2 and ATS Class 3 patients. There was a significant decrease in HLA-DR6y (p = 0.01) but increase in HLA-B5 and HLA-DR5 phenotypes in ATS Class 3 patients.

Unusual vascular findings in transplanted kidneys.

Of 189 unsuccessful renal transplant cases, eight were selected for retrospective analysis because they had been classified histologically as "chronic rejection with marked vascular changes" even though they had been removed less than 60 days after transplantation. Review of these cases revealed that only one of the eight transplants had postoperative function and that this transplant had only marginal functions. All eight kidneys had intimal damage that was unlike the usual lesions of rejection. Five kidneys showed evidence of thrombosis with recanalization, and five had what appeared to be a double layer of the intima. The findings in eight cases suggest that severe intimal damage occurred within 60 days of implantation and may not represent the usual rejection processes; it may reflect, in part, damage to the intima prior to or during implantation.

Antilung antibodies in asthma.

Twenty-four asthmatic patients were evaluated for the presence of circulating cytotoxic lung antibodies. These patients were further classified as either extrinsic or intrinsic asthmatics on the basis of skin testing, age of onset, and atopic history. Of the 12 patients considered to have extrinsic asthma, 10 had positive titers, one borderline, and one negative for cytotoxic lung antibodies. In the group of 12 patients classified as intrinsic asthmatics, eight had negative titers, two borderline, and two positive for cytotoxic lung antibodies. Adsorption studies indicated that these antibodies were organ-specific. Twenty normal non-smoking controls were also tested, all of whom were negative. Five patients with allergic rhinitis, positive intradermal skin tests, and no history of asthma were found to be negative for cytotoxic lung antibodies.

Restrospective correlation of clinical and histologic findings of 189 exchanged kidneys.

Tissues samples from 189 unsuccessful renal allografts, 47 recovered at autopsy and the others removed surgically, were examined histologically by light microscopy. Tissues samples were obtained from cadaver kidneys that had been exchanged regionally for transplantation. Each allograft tissue sample was rated as to extent of pathologic changes denoting rejection and was classified accordingly. Surgical and autopsy reports, as well as clinical data, were then obtained and these were compared with the retrospective pathologic findings of this study. Our pathologic findings agreed with the original pathologic diagnosis as to presence or absence of rejection changes in 180 cases, but disagreed with the clinical diagnosis of rejection in 28 of the 63 cases with minimal or no histologic evidence of rejection. There was less disagreement with the clinical diagnosis for the 87 cases with histologic evidence of rejection which had been judged as sufficient to cause allograft loss, 70 having been clinically diagnosed as rejected. Disagreement occurred most often where the allograft had never functioned or had been lost within 3 months. Retrospective analysis did not disclose any association between rejection histology and preformed antibodies or length of kidney perfusion time. Sufficient allografts appeared to have been lost for reasons other than rejection to cast doubt on the validity of interpreting renal allograft data only by graft survival statistics.

Properties of a cytotoxic kidney antibody associated with human renal transplantation.

A technic for demonstrating a complement-dependent cytotoxic response specific for human kidney cells was developed. Positive and negative sera from kidney-transplant recipients were subjected to several assay and adsorption procedures. The cytotoxic antibody reaction appeared to be specific against human kidney in tests with a variety of target cells. Manifestation of the typical cytotoxic effect depended on kidney explant target-cell surface changes following attachment, outgrowth, and dispersion of the first passage monolayer. A comparison of serum responses to lymphocytes and to kidney cells indicated that the cytotoxic response by human kidney cells is not related to the lymphocytotoxic antibodies associated with the HLA system.

Anti-lung antibodies associated with human lung allografts.

Serum specimens from 4 patients who had had a lung allograft were examined for anti-lung antibodies. Two patients developed antibodies after transplantation, and in 2, the antibodies increased in titer after an allograft. The absorption of the anti-lung antibody by fetal lung cell cultures, in contrast to the failure of absorption by kidney cell cultures from the same fetus, indicates that the antibody is lung specific.
