Nonfunctioning islet cell carcinoma--a twenty-one years survival case after distal pancreatectomy.

A case of nonfunctioning islet cell carcinoma was reported. A 40-year-old woman was admitted with epigastralgia. Abdominal angiography and other diagnostic modalities suggested pancreatic malignancy. After distal pancreatectomy, histopathological study revealed her pancreatic tumor to be a nonfunctioning islet cell carcinoma. Fourteen years later, postoperative computed tomographic examination (CT) detected the recurrence of para-aortic lymph node metastases. Five years later, distal gastrectomy was performed to control bleeding from a gastric ulcer. Twenty-one years after the original operation, she died because of underlying metastatic carcinoma. In this case, slow growth and a low grade malignancy were characteristic. Operative removal of the tumor would be the treatment of choice even if metastatic lesions existed.

Prosthetic valve thrombosis after abdominal surgical procedures: a case report.

A 63-year-old woman with a prosthetic mitral valve who developed valve thrombosis after abdominal surgical procedures is reported. She had undergone mitral valve replacement with a Sorin-Bicarbon valve 3 years previously, and was referred for an operation due to rectal cancer with anal bleeding. On admission, the patient was on oral anticoagulant therapy with warfarin, and the intensity of anticoagulation was around 40% of Thrombotest. Doppler echocardiography showed that the prosthetic mitral valve function was normal. Low anterior resection of the rectum was uneventful. Management of the perioperative anticoagulation was peformed with heparin, however, the postoperative anticoagulation using intravenous infusion of heparin was imperfectly achieved. Consequently, thrombosis of the prosthetic mitral valve occurred 8 days after the operation. Replacement of the thrombosed prosthetic valve was performed with successful results. The imperfect postoperative anticoagulation with heparin may have been the cause of the valve thrombosis. The management of perioperative anticoagulation during noncardiac operations is discussed.

Toxicological study on antineoplastons A-10 and AS2-1 in cancer patients.

Antineoplastons, which were firstly described by Burzynski, are naturally occurring peptides and amino acid derivatives which control neoplastic growth. We conducted a toxicological study of the Antineoplastons A-10 and AS2-1 in combination with other anticancer agents or radiation in 42 patients, 46 tumors with terminal stage cancer. Antineoplaston A-10 oral formulation and A-10 injectable formulation was administered in 14 and 25 patients respectively. The maximum daily dose was 10 g and 40 g, respectively and the longest term of administration was 610 days and 67 days, respectively. Antineoplaston AS2-1 oral formulation and AS2-1 injectable formulation was administered in 33 and 10 patients, respectively, the maximum daily dose was 12 g and 30 g, respectively, and the longest term was 1070 days and 25 days, respectively. The major adverse effects that may have been related to these agents as used in combination with other conventional chemotherapeutic agents or radiation were general weakness, myelosuppression, and liver dysfunction, but these effects were not seen when either Antineoplaston was administered alone. The minor adverse effects observed in single use of either Antineoplaston A-10 or AS2-1 were excess gas, maculopapullar rash, fingers rigidity, reduced cholesterol, reduced albumin, increased amylase, eosinophilia, increased alkaline phosphatase, headache, hypertension, palpitation, peripheral edema but these adverse effects did not limit to continuation of either agent. The evaluation of the usefulness of the Antineoplastons in combination therapy based on the imaging findings during the course of treatment revealed disappearance or measurable shrinkage of the tumor lasting more than one months as visualized by magnetic resonance imaging or computed tomography was seen in 15 tumors (32.6%). No increase in size of tumor for more than 3 months was observed in 8 (17.4%). The mean survival time of these patients was significantly longer than that in patients with tumors showing progressive increasing (17.52 + 3.31 months vs 4.80 + 0.65 months, p < 0.005). Antineoplaston A-10 and AS2-1 are less toxic than conventional chemotherapeutics and they were useful in maintenance therapy for cancer patients.

The inhibitory effect of the combination of antineoplaston A-10 injection with a small dose of cis-diamminedichloroplatinum on cell and tumor growth of human hepatocellular carcinoma.

The inhibitory effects of a combination of Antineoplaston A-10 Injection with a small dose of cis-diamminedichloroplatinum (CDDP) on cell and tumor growth was tested in vitro and in vivo settings. A human hepatocellular carcinoma cell line (KIM-1) was used for the cell growth and transplanted tumor growth studies. In the cell growth study, one-hour exposure of KIM-1 cells to CDDP in the medium at concentrations of 0.5, 1.0, and 2.0 micrograms/ml inhibited cell growth dose-dependently. Continuous exposure of cultured cells to Antineoplaston A-10 Injection at concentrations of 4, 6, and 8 mg/ml also inhibited tumor growth dose-dependently. The combination of 0.5 microgram/ml CDDP and 6 mg/ml A-10 Injection inhibited cell growth more than did each agent individually. Electron microscopic study showed well-maintained organelle structures in Antineoplaston A-10 Injection-treated cells compared to CDDP-treated cells. alpha-Fetoprotein (AFP) production by 10(4) cells in 48 h increased in the A-10 Injection-treated and A-10 Injection+CDDP-treated groups as the concentration of these agents increased. In the tumor growth study, daily administration of Antineoplaston A-10 Injection 75 mg with once a week administration of 20 micrograms of CDDP for 5 weeks inhibited transplanted tumor growth in athymic mice after 33 days of treatment, while administration of 75 mg of A-10 Injection or 20 or 60 micrograms of CDDP alone showed no significant inhibition of tumor growth.

[A device for hepatic arterial catheterization using saphenous vein graft].

We performed regional arterial infusion chemotherapy for hepatocellular carcinoma by retaining a hepatic arterial reservoir. The catheters were placed via gastroduodenal artery and positioned at the junction with common hepatic artery during operation. But we encountered many complications and did not perform regional arterial infusion chemotherapy. This time we cannulated through the saphenous vein graft by which hepatic perfusion was maintained by the superior mesenteric artery; we did not cannulate through the common femoral artery using a subcutaneously implanted reservoir. Venous arterial anastomosis is performed with interrupted (7-0 Prolene) vascular suture. Postoperative angiography by the reservoir showed total hepatic perfusion. There have been no instances of hemorrhage, thrombosis, or catheter dislodgement with this technique. Duration of follow-up varied from 2 to 7 months.

Changes in lymphocyte subsets, mitogen responsiveness, and interleukin-2 production after cardiac operations.

We studied 104 patients after cardiac operations to search for changes in peripheral blood lymphocyte subsets, mitogen responsiveness, and interleukin-2 production. Mononuclear cells obtained from 10 patients with cholecystectomy were examined as control samples. Cultures for phytohemagglutinin and interleukin-2 production were incubated in microtiter plates for 24 hours with cells labeled with tritium thymidine used as target cells. Lymphocyte subsets were measured by using monoclonal antibodies. The results obtained with preoperative blood samples were compared with those from the first, third, and seventh days after operation. In patients having cardiac operations, mitogen responsiveness was low on both the first and the seventh days after operation and interleukin-2 production was markedly depressed (greater than 90% depressed) on the first postoperative day. However, in control samples from patients having cholecystectomy, lymphocyte subsets, mitogen responsiveness, and interleukin-2 production showed no significant changes. Improvement in interleukin-2 production occurred immediately in patients without blood transfusion from random donors and reached normal levels by postoperative day 3. However, interleukin-2 production remained depressed on day 3 in all patients with transfusion from random donors and remained significantly diminished even on day 7 in such patients in New York Heart Association classes III and IV. We conclude that heart operations for which cardiopulmonary bypass is used are associated with depression of cellular immunity, including interleukin-2 production. Furthermore, it appears that both blood transfusion from random donors and the general preoperative condition of the patients contribute to suppression of measures of cell-mediated immunity.

[Immunocytochemical localization of carcinoembryonic antigen (CEA) and transport of CEA into the blood vessels in gastric cancer].

CEA was localized in the luminal border, cytoplasm, but not in mucus, in Signet ring cell carcinoma (sig) and mucinous carcinoma (muc). Electronmicroscopically, CEA was localized in the glycocalyx of the microvilli and microvesicles of the cytoplasm. The histologically different cancer types showed no difference in the localization of T-CEA. We also studied P-CEA elevating factors in 38 CEA-positive (++) patients manifesting subserosal (ss) or deeper invasion. No remarkable findings were obtained. When P-CEA elevating factors were studied in 20 patients with (+) CEA reaction and ss or deeper invasion, we found that the incidence of por was high in P-CEA negative cases. In particular, the por incidence was significantly low (p less than 0.01) in patients with scirrhous type. High P-CEA levels were detected in blood adjacent to the cancer. Among 6 cases with negative P-CEA in the vessels adjacent to the cancer whose T-CEA reactions were (++) or (+), and who manifested ss or deeper invasion, there was a high incidence of por (5 cases) and scirrhous type (4 cases), histologically.

Rhabdomyosarcoma of the right ventricle.

A case of a 66-year-old male with rhabdomyosarcoma of the right ventricle who presented first with a pleuritic chest pain and later developed congestive heart failure with A-V block is described. The total period of the disease was about 9 months. At autopsy the right ventricle and wall were found to be replaced by tumor tissue which was diagnosed histologically as rhabdomyosarcoma of the alveolar type. We described one case and 52 rhabdomyosarcomas of the heart from the literature; the ages ranged from 3 months to 92 years with 29 men and 23 women (unknown 1). The primary lesions were right ventricle 29%, right atrium 26%, left atrium 24%, left ventricle 6%, and others 15%.

Quick response of advanced cancer to chemoradiation therapy with antineoplastons.

Antineoplastons A10 and AS2-1 exhibit growth inhibition of cancer cells by diverse modes of action. We observed antitumor responses within 2-3 weeks of a combination treatment of chemoradiation therapy and antineoplastons A10 and AS2-1 in phase I clinical study being conducted in Kurume University Hospital. We reviewed 3 clinical cases of advanced cancer (multiple metastatic lung cancer, thalamic glioma and primary lung cancer) in which we believed antineoplaston A10 and AS2-1 may be contributing to the rapid antitumor response. The possible use of this combination for induction therapy in advanced cancer is discussed.

Hepatic arterial infusion chemotherapy with continuous low dose administration of cisplatin and 5-fluorouracil for multiple recurrence of hepatocellular carcinoma after surgical treatment.

To date, conventional treatments for multiple intrahepatic recurrence of hepatocellular carcinoma (HCC) after surgery are unsuccessful. The aim of this retrospective study was to evaluate the prognostic effectiveness of a new infusion chemotherapy of cisplatin (CDDP) and 5-fluorouracil (5-FU) via hepatic artery for HCC with multiple intrahepatic recurrence. Fifty-two patients, who had postoperative multiple recurrence of HCC (more than 3 tumors), were enrolled in this study. Thirty-one patients were treated by hepatic arterial infusion chemotherapy via a subcutaneously implanted injection port. A one-week course of this treatment consisted of daily administration of cisplatin (10 mg for 1 h on days 1-5) and subsequent daily administration of 5-fluorouracil (250 mg for 5 h on days 1-5). Three to six sequential one-week courses were performed (the CDDP,5-FU group). Twenty-one patients underwent conventional interventional therapies including transcatheter arterial chemoembolization, lipiodolization (the conventional group). The complete response rate and the effective response rate in the CDDP,5-FU group were 29.0% and 71.0%, respectively. The 5-year survival rate in this group was 45.7%, which was significantly better than that in the conventional group. Based on multivariate analysis, CDDP,5-FU hepatic arterial infusion chemotherapy was found to be significant in prolonging survival, and this treatment achieved favorable therapeutic results for multiple recurrence of HCC. As part of a multidisciplinary approach this treatment is expected to improve the prognosis of patients with advanced HCC.
