[Reconstruction of the anterior eye segment in children based on penetrating rotary autokeratoplasty].

The paper describes the technique of reconstruction of the anterior segment of the eye used in 46 children, aged from 3 to 14 years, with coarse adherent corneal leukomas, adhesions and obliterations of the pupil, cataracts of mainly (89.2%) posttraumatic etiology. Besides rotatory penetrating autokeratoplasty, the complex surgical intervention included iridoplasty in 91.3%, cataract extraction in 82.6%, anterior vitrectomy in 17.4%, intraocular lens implantation in 2.2% of cases. The complex surgical intervention allowed to achieve transparency of the central zone of the cornea thanks to avoidance of tissue incompatibility, harmless manipulations on the tissues of the anterior segment of the eye, prevention of recurrent adhesions in the anterior chamber. A rise of visual acuity was recorded in 33 persons (71.7%), of them to 0.3-0.7 with correction in 30.8% of cases in remote terms of follow-up. Improvement of cosmetic appearance was achieved in all of the patients operated on.

[Methods to improve the delivery of hospital ophthalmological care to children in the Ukrainian SSR].

On the material of Children Eye Department the Filatov Institute, changes in the structure of eye pathology for the period of 1947-1989 are analysed. It is shown that for the last years, among all eye diseases, the incidence of inflammatory eye diseases fell to 7.5%. The incidence of eye traumas and their outcomes reaching 26.2% remained stable. The incidence of congenital pathology (35.4%) and new-formations in the eye (10.8%) with predominance of malignant tumors, complex pathology of oculomotor apparatus (23.5%) reached high values. Organization of centers, of ophthalmic microsurgery in children allowed to introduce into ophthalmological practice complex surgical interventions requiring special devices and instruments. Surgical activity of the Center of Ophthalmic microsurgery in Children on the basis of the Filatov Institute made up 84.5% in the last five years. In order to optimize the rendering of stationary ophthalmological aid to children, the authors consider it necessary to improve continuity between a microsurgical center and regional children eye hospitals, to increase qualification of children ophthalmologists, to create a Republic Center for Rehabilitation of Children with eye diseases on the basis of a sanatorium with a good ophthalmological equipment.

[The clinical characteristics and emergency microsurgical care in eye injuries in children].

The paper describes results after primary surgical treatment in 131 children, aged from 2 to 14 years, with severe combined trauma of the anterior segment of the eye complicated by lens injury in 84.4%, prolapse of eye coats in 34.9%, injury of the iris in 12.8%, extensive hemorrhages in 12.8%, intraocular foreign bodies in 6.4%. Besides sutural fixation of corneal and scleral wounds, there were simultaneously made iridoplasty, aspiration of a swelling lens, anterior vitrectomy, removal of a foreign body, organized hyphema and exudate tectonic corneoscleroplasty, and, if indicated, IOL implantation. The functional outcomes (a cure with visual acuity 0.3-1.0 was recorded in 45% of patients) speak in favor of such a method of rendering urgent aid and allow to recommend it into a wide ophthalmological practice.

[An improvement in the technic and results of phacoemulsification of congenital cataracts in children].

In 163 children (256 eyes), aged from 3 months to 14 years, with different clinical forms of bilateral and monolateral congenital cataracts surgical interventions were used. A new method of extracapsular cataract extraction is described: as a retractor and protector of the iris in the performance of the operation, a formed flap of the anterior lens capsule is used. The proposed method provides conditions for maximum and atraumatic removal of masses from the upper segment of the lens capsule, lowers the risk of the opening of the posterior capsule, prolapse of the vitreous body and prevents traumatizing of the iris. Optical results depended on the kind of cataract and the presence of associated pathology. Visual acuity of 0.3-1.0 was achieved in 37.1%-46.4% in zonular cataracts and in 21.7%-22.2% in total cataract. In case of atypical congenital forms of cataract it was, as a rule, not higher than 0.2.

[The state-of-the-art of surgical treatment of congenital cataract in prematures].

Described in the paper are results of a clinical treatment of 127 children (174 eyes), aged 2 to 15 years, with different-type congenital cataract; the treatment was based on modem technologies including tunnel limbic incision, continuous anterior round capsulorhexis, aspiration-irrigation of the contents of the capsular sac, implantation of soft Acrysof SA30A1 IOLs by forceps or "Monarch" injector and suturing of incision. The anterior continuous capsulotomy was found to be the most complicated surgery stage in removal of congenital cataract in children it could be made without errors in 75.3% of cases. Intracapsular implantation of IOL in different-type cataract in children was possible in 97.7% of cases. The single-unit Acrysof IOL stays stable inside the capsular sac for as long as 24 months. A combination of primary capsulorhexis with "dry" anterior vitrectomy is most effective in the prevention of secondary cataract.
