[Systemic chemotherapy with cisplatin, methotrexate, and vinblastine, and bladder preservation in T2/T3a bladder cancer].

We are currently witnessing a lively controversy over which should be the optimal treatment for patients with infiltrant urothelial carcinoma with muscle location (stage B Jewett's Grading or T2-T3 on the TNM Rating Scale, UICC 1992). This paper reports a randomized, prospective study in 37 patients with T2-T3a tumours. Arm A (n = 20) received systemic chemotherapy with cisplatin, methotrexate and vinblastine (CMV) after TUR, and was carefully followed aiming for vesical preservation. Arm B (n = 17) was given chemotherapy with CMV after TUR and then underwent cystectomy. Disease-free survival at 5 years for the overall series is 69%, 47% for group A and 88% for group B (log-rank, p = 0.14). No statistically significant differences in survival were detected relative to age (under or over 65 years), growth patterns (papillary or plain), histological grade (G2 or G3), and size (smaller or greater than 5 cm). Survival was different depending on the clinical suspicion of nodular involvement (log-rank, p < 0.001). In summary, although the number of patients in the series does not allow to draw conclusions in terms of statistical significance, there is enough evidence to consider that vesical preservation based on TUR and systemic chemotherapy as definitive treatment is a dangerous option without the good survival rates achieved by T2/T3a patients undergoing cystectomy.

[Suburethral sling in the treatment of urinary incontinence: our experience].

Review of 17 female patients who underwent placing of Suburethral Sling as anti-incontinence procedure with the purpose of assessing the results obtained. The most frequent complications were urinary retention and post-surgical urgency. A larger proportion of patients with Stress Urinary Incontinence had retention and during more days than those with Mixed Urinary Incontinence. In most cases of mixed urinary incontinence, asymptomatic prior to surgery, the placing of a suburethral sling provokes signs and symptoms of urgency, and even of post-surgical urgency-incontinence. Both complications were attenuated in all cases using measures such as conservation of urinary by-pass or administration of anti-cholinergics. Results at 2 years have been successful in 68.75% cases, thus encouraging us to reduce the number of indications for this technique.

[Renal hemangiopericytoma and secondary hypertension].

OBJECTIVES: Tumors arising from pericytes are rare; even more so are those located in the kidney. A case of renal hemangiopericytoma is described herein. The literature is reviewed with special reference to the unclear prognosis of the foregoing condition and the therapeutic approach. METHODS: A 51-year-old male with renal hemangiopericytoma who underwent radical nephrectomy is described. RESULTS/CONCLUSIONS: The arterial hypertension that had led to the diagnosis of this neoplasm remitted after surgery and can be considered to be a paraneoplastic manifestation.

[Renal-cell adenocarcinoma as an incidental finding. Its diagnosis and tumor evolution].

OBJECTIVES: We attempted to determine whether or not incidental renal cell carcinoma had a better prognosis. METHODS: We reviewed 53 cases of renal cell carcinoma that had been surgically treated over the periods 1985-1988 and 1989-1993. The form of presentation, stage of the incidental tumors, sex, compromised side and the diagnostic imaging technique utilized were analyzed. RESULTS: GI pathology most frequently led to the diagnosis of the tumor. Tumor stages were A or B with a slightly higher prevalence for the incidental tumors. However, the prognosis was not better than that of clinically suspected renal cell carcinoma. The male to female ratio was 1.17:1, the mean age was 63 years, the tumors were frequently right-sided and ultrasound was superior in the diagnosis of incidental tumors. CONCLUSIONS: No prognostic difference was observed between clinically suspected and incidental renal cell carcinoma of the same tumor stage.

[Trigono-cervico-prostatotomy (TCP) versus transurethral resection of the prostate (TURP). Our experience].

OBJECTIVE: Trigonocervicoprostatotomy (TCP) has been considered up to now an alternative treatment to transurethral resection of the prostate (TURP) when evaluating the size of the prostate. This paper demonstrates that endoscopic visualization is more important when choosing the surgical procedure and that both techniques can achieve satisfactory results. METHODS: The prostate volume is evaluated by ultrasound and according to the endoscopic findings, TURP or TCP is performed. The prostatic lobes are routinely biopsied if the latter procedure is chosen. RESULTS: We compared the results of 34 TCP and 30 TURP procedures using the values of I-PSS and uroflowmetry before and after surgery. In 86.2% of the TCPs and 80% of the TURPs, the patients are asymptomatic with I-PSS less than 7 and significantly improved uroflowmetry data. Biopsy disclosed adenocarcinoma in one patient submitted to TCP. CONCLUSIONS: TCP represents an alternative to TURP not only for small prostates (30 gm) but also for medium-sized prostates (50-60 gms), depending on the endoscopic findings. TCP is not indicated for lateral lobes that fall on the prostate floor. Performing a biopsy routinely in TCP does not prolong the operating time significantly and permits detecting subclinical adenocarcinoma of the prostate.

[Squamous cell carcinoma of the urinary bladder. Clinico-pathologic study of 7 cases].

A retrospective review of 552 patients with primary bladder cancer yielded 7 cases (1.2%) of pure squamous cell carcinoma. Their clinical presentations did not differ from conventional transitional cell carcinomas, gross hematuria being the most frequent complaint (6 cases). All the cases presented as flat tumors. They were large and frequently affected more than one topographic area. The trigone was almost always affected (6/7). Three cases were considered low and 4, high grade. There were no dysplastic changes on the peripheral mucosa in any case. At the time of diagnosis, all of them were deeply invasive (pT3 and pT4). Two cases were Stage III and 5 Stage IV, with involvement of lymph nodes and metastases to the lung (2 cases), liver (1 case), and bone (1 case). All the patients died of tumor and the overall survival was very poor (range 6-37 months; mean 17.4).

[Bladder leiomyoma: report of a case].

We report a case of leiomyoma of the bladder in a male patient who presented with cystitic syndrome and hematuria. The literature is briefly reviewed and the benign nature of this condition is underscored. The prognosis is excellent with correct treatment.

[Inverted papillary carcinoma].

Some authors consider the papilloma to be a very low grade papillary carcinoma limited to the urothelium. Similarly, inverted papilloma can be interpreted to be a papillary carcinoma with an inverted growth pattern. Furthermore, the association of transitional cell carcinoma and inverted papilloma supports this concept. We investigated the frequency of transitional cell carcinoma associated with inverted growth--synchronous or metachronous--in our setting. Our results show that this association is present in 1.7% of the pure transitional cell carcinomas and in 2.6% of the superficial pure transitional cell carcinomas (pTa/pT1). We underscore the possibility of misinterpreting these lesions to be benign, while in fact they are potentially aggressive.

[The use of computerized tomography in the localization of impalpable testes].

Computed tomography is the most reliable method in the localization of the impalpable testis. This imaging technique permits precise preoperative localization where ultrasound cannot. Moreover, it can provide very useful information in the complicated cases of cryptorchidism (torsion, trauma, tumor) and can disclose other previously undetected coexisting anomalies.

[Percutaneous transluminal angioplasty on the solitary kidney. Report of a complicated case of renovascular hypertension].

We report a complicated case of renovascular hypertension from bilateral atherosclerotic renal artery stenosis in a solitary functioning kidney. Percutaneous transluminal angioplasty (PTA) was performed after diagnostic arteriography. Hypertension remitted only after surgical removal of the nonfunctioning kidney. In our view, PTA is the procedure of choice even in the solitary kidney.

[Chilaiditi syndrome in the differential diagnosis of renal colic].

We report on a case of hepatodiaphragmatic interposition of the colon in a 45-year-old patient who presented at the emergency department with a clinical picture of renal colic. When making the differential diagnosis, we should take into account that the clinical picture known as Chilaiditi syndrome may mimic renal colic.

[Blind-ending bifid ureter associated with adult congenital hydronephrosis].

We present a case of cephalad blind-ending bifid ureter coexisting with adult congenital hydronephrosis that had not clinically manifested previously and had been incidentally detected during surgery. The possibility that both anomalies of development may be related etiopathogenically is discussed. This and other malformations should be sought during the preoperative evaluation of a patient with congenital hydronephrosis.

[Practical precautions in urology against the human immunodeficiency virus].

As infection from HIV spreads, the features of the population at risk change. In fact, we have been treating more cases with different features. Numerous studies have been published in the medical literature aimed at making the health staff aware of the need to take precautions while caring for these patients, most of whom are HIV infected but apparently healthy. However, the information on prevention of infection for staff in the different specialist settings who are less accustomed to treating these patients seem to be relatively scant. We believe that it is necessary to provide and make known the simple, yet important preventive measures in the urological setting.

[Adriamycin versus ftorafur in the prevention of recurrence of superficial carcinoma of the bladder].

We performed a randomized prospective study in 40 patients with primary superficial carcinoma of the bladder who had been submitted to TUR. These patients were divided into two groups; one received intravesical adriamycin and the other oral ftorafur. We analyzed tumor size, number and grade and determined the recurrence rate (IR), recurrence rate/month (RR) recurrence rate/cystoscopy (RIC), disease-free interval, survival curves, and drug toxicity. After a mean follow-up of 19.7 months for the patient group that received adriamycin and 20.2 months for the group on oral ftorafur, the IR, RR, and RIC for the group on adriamycin were 40, 2.20 and 0.08%, respectively, and 33.3, 1.64 and 0.07% for the patient group on ftorafur (difference: NS). Drug toxicity was practically absent for the group on adriamycin whereas treatment had to be discontinued in two of the patients on ftorafur due to gastric intolerance. Furthermore, ftorafur was shown to cause a transient increase of transaminase levels in 4 patients, cutaneous changes and alterations in fingernails in 1 patient, and intense dermatitis during the last month of treatment in another patient. However, it was not necessary to discontinue treatment.

[Paratesticular rhabdomyosarcoma].

We report a case of paratesticular rhabdomyosarcoma in a 5-year-old boy who was submitted to radical orchidectomy via the inguinal approach and high ligation of the spermatic cord. Clinical and retroperitoneal staging classify this case under group I of the Intergroup Rhabdomyoma Study. Adjuvan chemotherapy with 12 courses of VAC have achieved a good clinical course to date (14 months after surgery) and a good outcome is predicted. We review the literature and discuss the features of this mesenchymal tumor.

[The immotile cilia syndrome as a cause of sterility].

We investigated the relationship between ciliary dyskinesia--commonly referred to as inmotile cilia syndrome--and sterility. In the past two years, we have accurately diagnosed 6 new cases of a total of 20 suspected as having this condition. To make the diagnosis, complete clinical, radiological, ultrastructural and spermatic work up was performed. All males in the fertile age were found to be sterile with spermatozoides with no motility. Sterility may initially go undetected in these patients due to early intense ORL and respiratory symptoms they present. However, mild forms of this disease entity may be asymptomatic and patients may consult for sterility with a clinical picture of scantily florid chronic bronchitis like that of smokers. Coincidental situs inversus may be useful in making the diagnosis.

[Vesicovaginal fistula in an 8-year-old girl].

We report a case of vesicovaginal fistula in an 8-year-old girl caused by a foreign body in the vagina. The foregoing condition was associated with vesical lithiasis. Correction of the fistula was performed during the same surgical procedure for stone removal, without waiting for 3-6 months to elapse as advocated by most of the authors. A review of the literature revealed only one such case has been reported earlier.
