Loosening of a noncemented porous-coated anatomic femoral component in Gaucher's disease. A case report and review of literature.

There are few reports of experiences with hip arthroplasty in patients with Gaucher's disease. Review of the literature shows a high rate of loosening after any type of arthroplasty. The clinical, roentgenographic and histopathological findings of a porous-coated hip arthroplasty in Gaucher's disease necessitating revision after 5 years are reported. Because we found typical lipid-laden Gaucher cells at the bone prosthesis interface we support the hypothesis that loosening in Gaucher's disease is due to continued Gaucher cell proliferation and erosion of bone. Loosening seems to be related to the disease and not to a specific type of prosthetic component.

Sudden hypotonic paraparesis caused by tophaceous gout of the lumbar spine.

Gouty arthritis of the axial skeleton is rare, and neurological complications due to spinal cord or nerve root compression secondary to tophaceous deposits have been seldom reported. We describe a patient with chronic gouty arthritis who developed an acute paraparesis caused by tophaceous cauda equina compression. Surgical removal of the mass resulted in complete recovery.

Atypical ossification in bronchial carcinoid.

A patient is described with bronchial carcinoid and ossification in the surrounding bronchial wall. The osseous metaplasia was the only histologic abnormality discovered in bronchial biopsy specimens taken pre-operatively. We propose that underlying bronchial carcinoid tumour should be considered in isolated bronchial or bronchopulmonary ossification.

Some remarks on nasal polyposis.

After a short historical and literature review the authors present their definition of nasal polyposis. The importance of CT scanning is stressed and the results of a CT-scan study of 350 patients with nasal complaints is presented. The authors discuss the incidence of sinusitis, presence of polypi and nasal anomalies in this population. Furthermore 111 biopsies of nasal polypi were studied and the cellular content, the ducts, glands, veins, aspect of the basal membrane and epithelial layer were described. As an oral ASA provocation test can be hazardous, the authors tested a nasal provocation test with acetylsalicylic acid. Although the test showed some interesting results, the reproducibility was poor. Finally the authors conclude that endoscopy and CT-scanning enable the diagnosis of nasal polyposis in an early stage of the disease.
