Biosynthesis of glycosylphosphatidylinositols of Plasmodium falciparum in a cell-free incubation system: inositol acylation is needed for mannosylation of glycosylphosphatidylinositols.

The structures of glycosylphosphatidylinositols (GPIs) in Plasmodium have been described [Gerold, Schuppert and Schwarz (1994) J. Biol. Chem. 269, 2597-2606]. A detailed understanding of GPI synthesis in Plasmodium is a prerequisite for identifying differences present in biosynthetic pathways of parasites and host cells. A comparison of the biosynthetic pathway of GPIs has revealed differences between mammalian cells and parasitic protozoans. A cell-free incubation system prepared from asexual erythrocytic stages of Plasmodium falciparum, the causative agent of malaria in humans, is capable of synthesizing the same spectrum of GPIs as that found in metabolically labelled parasites. The formation of mannosylated GPIs in the cell-free system is shown to be inhibited by GTP and, unexpectedly, micromolar concentrations of GDP-Man. Lower concentrations of GDP-Man affect the spectrum of GPIs synthesized. The inositol ring of GPIs of P. falciparum is modified by an acyl group. The preferred donor of this fatty acid at the inositol ring is myristoyl-CoA. Inositol acylation has to precede the mannosylation of GPIs because, in the absence of acyl-CoA or CoA, mannosylated GPIs were not detected. Inositol myristoylation is a unique feature of plasmodial GPIs and thus might provide a potential target for drug therapy.

Osseointegrated oral implants. A Swedish multicenter study of 8139 consecutively inserted Nobelpharma implants.

Fourteen Swedish teams outside the University of Gothenburg, each with minimally three years' experience in the Nobelpharma osseointegrated implant participated in a retrospective multiclinic study. The total number of consecutively inserted implants at the 14 clinics was 8139. The outcome of every implant was reported and all implant failures, irrespective of when they occurred, were published. The success criteria included absence of implant mobility, absence of radiolucent zones on x-rays, and an annual bone loss after the first year of less than 0.2 mm. In the mandible 334 implants were followed for five to eight years, with only three failures, for a success rate of 99.1%. In the maxilla 106 implants were followed for five to seven years, with a success rate of 84.9%. In irradiated and grafted mandibles, 56 implants were inserted and none was lost during a follow-up of up to five years. In the irradiated maxilla there were 16 implants inserted with three reported failures and in the grafted upper jaw 71 implants were inserted with 12 failures. The proportions of mandibular and maxillary sleeping implants were 0.8 and 0.3%, of patient drop-out implants 0.3 and 0.6%, and of patient death implants 0.9 and 1.2%, respectively. It was concluded that the osseointegrated implant, if inserted according to the guidelines of Brånemark, results in a very high degree of clinical success, thereby meeting any published oral implant success criteria.

A model for evaluating the analgesic effect of a new fixed ratio combination analgesic in patients undergoing oral surgery.

A special model designed for evaluating the analgesic effect of oral analgesics was based on a short-time registration period of immediate postoperative pain. One-hour intervals in pain registration and a minimum of 2 h between the tablet intake allowed a good estimation of changes in pain levels. The patient material consisted of 112 patients and from each patient a lower impacted wisdom tooth was removed. The test model was used to compare two analgesic drugs with placebo. The two pharmacologically active preparations were Doleron (dextropropoxyphene, acetylsalicylic acid, phenazone, caffeine and Transergan) and Astra 2167 (dextropropoxyphene and acetylsalicylic acid). The trial was double blind and the tablets were administered according to a crossover design. There was no statistically significant difference in analgesic effect between Astra 2167 and Doleron, and both drugs were superior to placebo. Finally, the trial showed that a reduction of the number of components of a compound analgesic to some degree reduced the pain relieving effect on this particular postoperative pain. This observed reduction was however, not statistically significant.

Oral acantholytic itching disease responding to dapsone. Dermatitis herpetiformis, pemphigus, or a new disease?

A patient had a blistering and severely itching disease confined solely to the oral cavity. The histopathologic findings had the features of both pemphigus and dermatitis herpetiformis. There were no gastrointestinal symptoms, no IgA could be found in uninvolved skin, and no intercellular or basement membrane antibodies were present in the serum or epidermis. The disease responded favorable to dapsone (diaminodiphenylsulfone), which could be discontinued after 2 1/2 years. The literature is briefly reviewed, and there is a discussion of whether the patient was suffering from pemphigus or dermatitis herpetiformis. The conclusion is drawn that the diagnosis cannot be established with certainty and that we are perhaps dealing with a new disease. It is stressed that in such aberrant cases a therapeutic trial with dapsone should be made.
