Lymphocyte transformation test with nickel in hard metal asthma: another sensitizing component of hard metal.

To determine cell-mediated immunity to nickel, another matrix in hard metal besides cobalt, lymphocyte transformation tests (LTT) with nickel were carried out in seven hard metal asthma patients all of who had reacted to cobalt chloride in the bronchial provocation tests (BPT). Immunoallergic studies prior to the present study revealed that three of the seven generated a simultaneous positive reaction in the BPT with nickel and the allergosorbent test with nickel-conjugated human serum albumin (Ni-HSA). A stimulation index in LTT indicating a positive response was defined on the basis of results from the studies in the controls. Data revealed that two of the three who showed a combination of positive bronchial and immunological reactions with nickel had a positive LTT with nickel. In the other five, peripheral lymphocytes did not proliferate in response to nickel. Thus it is suggested that cell-mediated immunity to nickel as well as cobalt is implicated in some cases associated with hard metal asthma.

[A rare case of allergic granulomatous angitis (Churg Strauss syndrome) with positive anti-glomerular basement membrane (GBM) antibody in serum].

A 33-year-old man with a 6-month history of rhinitis and bronchial asthma was referred to our hospital with polyarthralgia, severe anemia, hypoxemia, mononeuropathy multiplex, and renal insufficiency with hematuria. Marked eosinophilia was observed in his sputum, peripheral blood, and bronchoalveolar lavage fluid (BALF). In addition, his sputum contained many hemosiderin-laden macrophages, indicative of pulmonary hemorrhage. His chest roentgenogram on admission showed diffuse ground grass appearance. High resolution computed tomography (HRCT) demonstrated diffuse high density areas throughout the lung fields and characteristic irregularity and enlargement of the peripheral pulmonary arteries. His general condition rapidly deteriorated, but dramatically improved with oral steroid administration, and his major symptoms disappeared within a few days. Examination of the biopsied lung tissue revealed unequivocal evidence of pulmonary angitis with marked eosinophilic infiltration and perivascular granulomas. Bone marrow biopsy showed hyperplasia of eosinophilic leukocytes in contrast to the low cellularity. Suppression of erythroid hemopoiesis was thought to be the primary cause for his rapidly progressive anemia. Serum anti-GBM antibody titer returned to within the normal range soon after the initiation of steroid therapy.

Grafting of the buccal fat pad into palatal defects.

Grafting of the buccal fat pad was performed in combination with lyophilized porcine dermis in six cases of palatal defects. The use of lyophilized porcine dermis not only simplified the surgical procedure but also permitted a proper prosthetic rehabilitation. This technique proved to have more advantages for palatal defects.

Hard metal asthma: cross immunological and respiratory reactivity between cobalt and nickel?

Eight asthmatic patients with hard metal asthma due to cobalt underwent bronchial provocation challenge with nickel sulphate. Seven patients developed a fall in FEV1 of 20% or more after inhaling nickel sulphate, four showing an immediate response and three a late response. Eight control subjects, including six asthmatic patients, with no history of hard metal exposure, showed no bronchoconstriction in response to a provocation challenge with nickel sulphate. Specific antibodies to nickel conjugated human serum albumin were present in four of the eight patients with sensitivity to cobalt conjugated human serum albumin but were absent from the serum of 60 unexposed asthmatic patients and 25 exposed symptom free workers. These results suggest that nickel as well as cobalt sensitivity plays a part in hard metal asthma.

[Constructions of compensating screen of chest radiography and its clinical applications--evaluation of normal anatomical structures].

We have constructed compensating screens which have different sensitivities for the pulmonary hilum and mediastinum in contrast to the lung. With these screens the regions of the mediastinal and retrocardiac areas are simultaneously displayed with optimal contrast on postero-anterior (PA) chest radiographs. The potential clinical use of compensating screens was evaluated in 67 patients for visibility of the anatomical structures in comparison with conventional screens. Confidence test was used to compare the detectability of the various anatomical structures of the chest. The chest radiographs obtained with compensating screens improved visibility of the anatomical structures including the tracheal bifurcation, right and left main bronchus, and paraesophageal line.

Occupational asthma from cobalt sensitivity in workers exposed to hard metal dust.

Eight asthmatic patients who had no history of asthma before starting work in a hard-metal plant and eight control subjects (three atopic, three nonatopic asthmatic, and two normal volunteers) without a history of exposure to hard metal dust were subjected to provocation tests, skin tests, radioallergosorbent tests (RAST) and Farr test with cobalt. Four of the eight patients were atopic, and seven showed bronchial hyperresponsiveness to methacholine (BHR). Patch and intradermal skin tests with cobalt chloride (CoCl2) could not discriminate the patients from control subjects. All patients had positive reactions to CoCl2 in the provocation tests; two developed immediate asthmatic reaction (IAR), four late asthmatic reaction (LAR), and two dual asthmatic reaction (DAR), while the control subjects showed no reaction. Evidence of specific IgE antibodies to cobalt-conjugated human serum albumin (Co-HSA) was presented by four patients (RAST score greater than 2) based on comparison of serum samples from 60 asthmatic patients and 25 asymptomatic workers in the same plant. Positive serum samples selectively bound 57Co, and the test was blocked by nonlabled cobalt sulfate (CoSO4). These findings suggest the development of hard metal-induced asthma from cobalt sensitivity.

The existence of specific antibodies to cobalt in hard metal asthma.

Twelve workers with hard metal asthma diagnosed on the basis of peak flow diaries and positive bronchial reactions to cobalt chloride (CoCl2) were studied for sensitization by detection of specific antibodies to radioactive cobalt (57Co), cobalt-conjugated human serum albumin (Co-HSA) and cobalt-conjugated exchange resin (Co-resin). Their IgE titres ranged from 73 to 1500 IU/ml and eight were atopic individuals. Sixty serum samples from asthmatic patients with IgE titres of 14-4300 IU/ml were studied as controls in all tests. Eleven of twelve subject sera that selectively bound to 57Co after incubation with saturated ammonium sulphate (greater than 232 c.p.m., P less than 0.01) were divided into three groups: (1) six sera showing evidence of specific IgE antibodies to Co-HSA (greater than 673 c.p.m., P less than 0.01) without those to Co-resin; (2) one serum giving a positive radio-allergosorbent test (RAST) only to Co-resin (greater than 417 c.p.m., P less than 0.01), and (3) four sera that were negative for two antigenic agents (Co-HSA, Co-resin). These results suggest that the subjects had occupational asthma due to hard metal exposure from cobalt sensitivity. An immuno-allergic mechanism mediated by specific IgE antibodies to cobalt was confirmed to be responsible for the development of hard metal asthma, with the possibility of some role of the reaction without reagins.

Computed tomography demonstration of renal dysplasia with a vaginal ectopic ureter.

The diagnosis of renal dysplasia associated with a single vaginal ectopic ureter is often difficult, because the dysplastic kidney is usually not seen on excretory urography. A 17-year-old girl, whose affected kidney was accurately identified by computed tomography, is reported. Computed tomography is a useful technique for the diagnosis of renal dysplasia with a single vaginal ectopic ureter.

Differentiation between idiopathic pulmonary fibrosis and interstitial pneumonia associated with collagen vascular diseases by comparison of the ratio of OKT4+ cells and OKT8+ cells in BALF T lymphocytes.

Idiopathic pulmonary fibrosis (IPF) and interstitial pneumonia associated with collagen vascular disease have been shown to be difficult to differentiate by clinical, roentgenological, pathological and immunological findings. We have applied the analysis of BALF T cell phenotypes to these two groups by enumerating the cell populations of OKT4+ and OKT8+ cells in BALF and, as a result, have been able to discern a clear and meaningful differentiating point between the two groups. In all 9 patients with IPF were examined in whom OKT4+ cells outnumbered OKT8+ cells in BALF T cell subsets, while 13 patients with collagen vascular disease were tested (3 SLE, 6 RA, 2 Sjögren's syndrom and 2 MCTD) in whom OKT8+ cells were more numerous than OKT4+ cells. No difference was observed in blood T lymphocytes between these two groups. These results provide us with a means of differentiation between these groups, especially in clinically borderline cases.

[Clinical studies on combination therapy of cephem antibiotics in chronic complicated urinary tract polymicrobial infections due to Pseudomonas aeruginosa and other bacteria].

Patients with chronic complicated urinary tract polymicrobial infections due to Pseudomonas aeruginosa and other bacteria were treated with cefsulodin using cefotiam or cefmenoxime as a concomitant drug. Each drug was given to the patients in daily dose of 1 or 2 g by drip infusion twice a day for 5 to 16 days. The results were as follows: Overall clinical efficacy was excellent in 7 cases (44%), moderate in 6 cases (38%) and poor in 3 cases (19%), the effectiveness rate being 81.3%. Bacteriologically, 38 strains (88.4%) were eradicated, 1 strain decreased and 4 strains unchanged out of the 43 clinical isolates from the patients. No side effects and no abnormalities of laboratory findings were observed.
