Sima de los Huesos (Sierra de Atapuerca, Spain). The site.

In this article a topographical description of the Cueva Mayor Cueva de Silo cave system is provided, including a more detailed topography of the Sala de los Ciclopes Sala de las Oseras-Sima de los Huesos sector. The history of the excavations and discoveries of human and carnivore fossils in Sima de los Huesos and adjacent passages is briefly reported, as well as the increase, throughout the succeeding field seasons, of the human collection and changes in the relative representation of the different skeletal elements and major biases. The carnivore assemblage structure is also considered. Examining the characteristics of the bone breccia, and the current and ancient karst topography, different alternative accesses are discussed for the accumulation of carnivores and humans in the Sima de los Huesos. Taking into account all the available information, an anthropic origin for the accumulation of human fossils seems to us to be the most likely explanation.

The carnivore remains from the Sima de los Huesos Middle Pleistocene site (Sierra de Atapuerca, Spain).

Remains of carnivores from the Sima de los Huesos site representing at least 158 adult individuals of a primitive (i.e., not very speleoid) form of Ursus deningeri Von Reichenau 1906, have been recovered through the 1995 field season. These new finds extend our knowledge of this group in the Sierra de Atapuerca Middle Pleistocene. Material previously classified as Cuoninae indet, is now assigned to Canis lupus and a third metatarsal assigned in 1987 to Panthera of gombaszoegensis, is in our opinion only attributable to Panthera sp. The family Mustelidae is added to the faunal list and includes Martes sp. and a smaller species. The presence of Panthera leo cf. fossilis, Lynx pardina spelaea and Felis silvestris, is confirmed. The presence of a not very speloid Ursus deningeri, together with the rest of the carnivore assemblage, points to a not very late Middle Pleistocene age, i.e., oxygen isotope stage 7 or older. Relative frequencies of skeletal elements for the bear and fox samples are without major biases. The age structure of the bear sample, based on dental wear stages, does not follow the typical hibernation mortality profile and resembles a catastrophic profile. The site was not a natal or refuge den. The hypothesis that the site was a natural trap is the most plausible. If the Sima de los Huesos functioned as a natural trap (without an egress out), the human accumulation cannot be attributed to carnivore: activities and must be explained differently.

Carnivores from the Early Pleistocene hominid-bearing Trinchera Dolina 6 (Sierra de Atapuerca, Spain).

The carnivores from Trinchera Dolina level 6 (TD6) in Sierra de Atapuerca include Ursus sp., Crocuta crocuta, Mustela palerminea, Lynx sp., Canis mosbachensis and Vulpes praeglacialis. Approximately 80 human remains belonging to Homo antecessor were found in the Aurora Stratum (AS): located in the upper part of the TD6 level (TD6, T36-43). The carnivores from AS are taxonomically similar to the rest of the TD6 level, differing only in the occurrence of Lynx sp. The spotted hyaena (Crocuta crocuta) inhabited the Atapuerca Sierra during the Early Pleistocene and up to the Middle Pleistocene, after which it is absent. According to palaeomagnetic, U/Th and ESR results, the fossils from TD6 are dated to the Early Pleistocene, which is the earliest certain occurrence in Europe of Crocuta crocuta. It is associated with Mimomys savini, the arrival of which in Europe can be correlated with the beginning of the Early Biharian. Crocuta crocuta may have arrived during the late Early Pleistocene ("Bavelian complex"), and subsequently invaded the rest of the continent during the Middle and Late Pleistocene. Atapuerca TD6-AS represents the most ancient deposits in Europe where Homo and spotted hyaenas coexisted and, they probably competed ecologically. The carnivores from TD6 are biochronologically consistent with the end of the Early Pleistocene or early Cromerian (corresponding with the Biharian biochron). The timespan could correspond with oxygen isotope stages 19, 20 or 21.

Successful intra-arterial thrombolysis for acute ischemic stroke in the immediate postpartum period: case report.

Stroke in pregnancy and the puerperium is a rare but potentially devastating event. We present the case of a previously healthy woman who underwent a cesarean delivery and experienced a middle cerebral artery thrombosis in the immediate postpartum period that was subsequently lysed with intra-arterial urokinase. The patient made a complete neurologic recovery. To the best of our knowledge, this is the first reported case of successful intra-arterial thrombolysis for ischemic stroke in the postpartum period.

Determination of surface topography of biological specimens at high resolution by scanning tunnelling microscopy.

Although techniques are available for the determination of the three-dimensional structure of biological specimens, for example scanning electron microscopy, they all have some serious drawback, such as low resolution, the requirement for crystals or for the sample to be analysed in a high vacuum. In an attempt to develop a technique for high-resolution three-dimensional structure analysis of non-crystalline biological material, we have tested the applicability of scanning tunnelling microscopy (STM), a method that has been used successfully in the analysis of metal and semiconductor surface structures. We report here that scanning tunnelling electron microscopy can be used to determine the surface topography of biological specimens at atmospheric pressure and room temperature, giving a vertical resolution of the order of 1 A. Our results show that quantum mechanical tunnelling of electrons through biological material is possible provided that the specimen is deposited on a conducting surface.

[Acute pancreatitis: the value of ultrasonography in its prognosis].

As acute pancreatitis is a disease of unpredictable outcome, we wanted to use an available diagnostic method such as ultrasonography, in order to establish the prognosis of this problem. There are some published papers using CT scan as prognostic value of this disease but none about ultrasonography. We evaluated 34 patients with clinical diagnosis of acute pancreatitis based on abdominal pain, nausea, vomits and absence of abdominal sounds plus an elevation of amylases of at least three times above the normal value. An abdominal ultrasound was performed at admission and every 4 days or even sooner if complications were suspected. Acute pancreatitis was classified by clinic and ultrasound in mild, moderate and severe. From 34 patients studied, 27 were clinically and by ultrasound correlated (79%) giving a sensibility of 100% and a specificity of 85%, without false positives. There were some cases in which ultrasound detected abnormalities before clinical manifestations, so a close follow up was done and this allowed us to be ready if any complications showed up. Follow up with ultrasound examination was very useful, permitted us to detect early complications with or without clinical manifestations and also emphasized that a single ultrasound examination is not enough as a prognostic or diagnostic method.

[Simplified method for the early detection of post potentials in patients with acute myocardial infarct].

The risk of suddenly presenting ventricular tachyarrhythmias or death was analyzed in a prospective way in 71 patients with acute myocardial infarction and in 21 patients without any apparent cardiopathy. The average surface signal was measured when the patient was admitted in the hospital and on the following days of the acute phase of the infarction. Holter 24 hours monitoring and determination of the ejection fraction with radionuclide angiocardiography were also performed. Patients were divided into 4 groups, depending on the existence of arrhythmias and if late electric activity was recorded or not. Fifty-five patients (group I), presented an abnormal signal analysis (voltage in the 40 msec of the QRS period higher than 20 microvolts), and ventricular arrhythmias. Group II was formed by 8 patients who presented arrhythmogenic activity but did not present late electric activity. Group III was formed by 6 patients who did not present arrhythmias but presented late electric activity. Twenty-three patients who did not present late electric activity or ventricular arrhythmias formed group IV. Late electric activity that was registered at 41 +/- 1 msec with a QRS complex lasting 115 +/- 4 msec and with a voltage lower than 20 microvolts in group I showed meaningful differences with the other groups: Group II: (24 +/- 5 msec, QRS 111 +/- 4 msec, P less than 0.001); Group III: (41 +/- 2 msec, QRS 109 +/- 5 msec, P less than 0.01); Group IV: (26 +/- 5 msec, QRS 80 +/- 7 msec, P less than 0.001).(ABSTRACT TRUNCATED AT 250 WORDS)

[Upper digestive hemorrhage in a patient with von Recklinghausen neurofibromatosis].

Neurofibromatosis (Von Recklinghausen's disease) is uninherited as an autosomal dominant trait. It is characterised by the development of tumors in diverse sites, which may be benign or malignant. The case of a 39 year old woman with Von Recklinghausen's disease is discussed. She presented with a 3 year history of episodes of melaena and iron deficient anaemia. Mesenteric angiography demonstrated a hypervascular tumour in the jejunum. It was surgically excised with an anatomopathological diagnosis of neurofibroma.

[Pancreatic insufficiency: an infrequent manifestation of primary hyperparathyroidism].

A case of malabsorption of pancreatic etiology secondary to primary hyperparathyroidism (HPT) is reported. Normalization of calcemia was achieved with parathyroidectomy and oral administration of pancreatic enzymes was initiated. HPT is an infrequent cause of pancreatic inflammatory disease. Although its association is controversial, its inclusion among the causes of exocrine pancreatic failure allows correct diagnosis and therapeutic management with a favorable prognosis.
