Adventitial cystic disease in the common femoral artery.

The rare condition of adventitial cystic disease with massive cysts in the common femoral artery was treated in a 49-year old man. The symptoms were calf and thigh claudication. Angiography showed occlusion of the common femoral artery and the deep femoral artery. The condition was obvious peroperatively, and after clearing the deep artery, a bypass (ePTFE) to the two femoral run off vessels was needed. After an uneventful postoperative period, it was observed at three months' follow-up, that the patient had foot pulses.

Arterial Occlusive Diseases↗

[Primary retroperitoneal seminoma].

A case of primary retroperitoneal seminoma is presented and 73 cases from the literature are reviewed. The mean age was 41 years. The presenting symptoms were abdominal pain in 53% of the cases, low back pain in 43% and in 7% the only symptom was a palpable mass. It is concluded that seminomata exist as primary tumors in the retroperitoneum, but thorough examinations of the patients is necessary in order to arrange correct treatment and to exclude a primary testicular focus or CIS. Ultrasound sonography to discover non-palpable testicular tumors is recommended as well as surgical biopsies of the testis to reveal CIS. In patients treated with combined surgical resection and radiotherapy, the success rate was 92%. The overall success rate was 68%. It is expected that the curability for all patients is significantly improved with the introduction of combination of radiation and chemotherapy, as it has been seen in advanced testicular cancers.

Malignant hemangiopericytoma of the ovary.

Tumors originating from the blood vessels seldom occur in the ovaries, and primary hemangiopericytoma localized to the ovaries has not previously been described. In the case reported here a bilateral ovarian tumor was found, with one metastasis to the terminal ileum. It was not possible to perform a radical operation, and the patient was after-treated with external irradiation therapy and chemotherapy. For 9 1/2 months the tumor receded, whereafter it appeared to be stationary for 13 months, to the death of the patient. At autopsy the tumor was in evident regression and there were no signs of metastases.

Pulmonary lymphangioleiomyomatosis: a case of progesterone receptor positive lymphangioleiomyomatosis treated with medroxyprogesterone, oophorectomy and tamoxifen.

A 28-year-old woman with lymphangioleiomyomatosis had severe changes in the chest radiograph and recurrent pneumothoraces. Pathological tissue from the lung was progesterone receptor positive. After treatment with medroxyprogesterone her condition improved, but did not become clinically stable until after bilateral oophorectomy. Even after oophorectomy her condition deteriorated, when the dose of medroxyprogesterone was decreased. For one year after oophorectomy she has had no episodes of pneumothorax or breathlessness. In order to depress oestrogen activity further the anti-oestrogen drug tamoxifen was added to her usual treatment with medroxyprogesterone. During 2 weeks of treatment with tamoxifen and medroxyprogesterone marked increase of the changes in the chest radiograph were seen and a pneumothorax occurred. After discontinuation of tamoxifen the condition improved and has since been stable on medroxyprogesterone treatment.

Granular cell tumor in the perineum. A review and case report.

Granular cell tumor is a lesion which clinically often resembles a fibroma. The tumor is, however, frequently ill-defined, and it may be mistaken for a malignant growth. The granular cell tumor is only rarely located to the perineum. A total of 55 cases have been reported. The diagnosis can be obtained by frozen section microscopy. Recurrence is prevented by excising the whole lesion. The development of malignancy is extremely rare.

Thromboangiitis obliterans affecting the upper extremity in a woman. Case report.

A rare case of thromboangiitis obliterans is reported, with involvement of the left brachial and axillary artery and signs of median-nerve denervation in a 42-year-old woman following long affection of the right popliteal artery. Arterial reconstruction of the upper extremity was beneficial when the patient stopped smoking. The histologic changes distinguished the condition from atherosclerosis.
