Activation by protein synthesis inhibitors of glucose transport into L6 muscle cells.

The effects of protein synthesis inhibitors on glucose transport into L6 myotubes was examined. Similar to the acute effects of insulin, short-term treatment with anisomycin or cycloheximide increased transport 2-fold. Kinetic studies demonstrated that this activation was due to a doubling in the Vmax values. The effects of anisomycin or cycloheximide were not additive to that of insulin. Both protein synthesis inhibitors caused only small increases in GLUT4 expression and negligible effects on GLUT1 expression. Anisomycin, as well as insulin, induced the translocation of both transporters from intracellular vesicles to the plasma membrane through not in quantities sufficient to entirely account for the activation of transport. The results indicate that protein synthesis inhibitors stimulate hexose transport in L6 myotubes by increasing the number of transporters in the plasma membrane and augmenting the intrinsic catalytic activity of the transporters.

Caring for a chronically ill infant: a paradigm case of maternal rehearsal in the neonatal intensive care unit.

This study examines a mother's perspective of caring for a chronically ill infant coming home from the neonatal intensive care unit. Understanding the meaning of the situation for the parent can foster excellence in clinical practice that is based on understanding derived from descriptions of their experience. This paper presents a paradigm case extracted from a larger phenomenological study that is examining the high-risk perinatal experience. Hermeneutic methods used for analysis led to the discovery of five meanings in the experience for the parent: (a) uncertainty; (b) experiencing the baby as powerful; (c) striving to gain acceptance from the baby; (d) blurred territory in taking up the practices of care; and (e) being alone and vulnerable.

Cutaneous responses to vasoactive intestinal polypeptide in chronic idiopathic urticaria.

Cutaneous wheal and flare responses to increasing concentrations of calcitonin gene-related peptide, substance P, neurokinin A, vasoactive intestinal polypeptide (VIP), compound 48/80, and phosphate-buffered saline were measured in 10 patients with chronic idiopathic urticaria and 10 healthy controls. A significant increase in VIP-induced wheal, but not flare or cutaneous blood flow, was seen in urticarial patients compared with controls (p less than 0.001). No significant differences in responses to other tested compounds were found between these groups. These data point to an increased sensitivity of microvasculature to VIP in patients with chronic idiopathic urticaria.

The specificity of technetium-labelled-leucocyte imaging of aortic grafts in the early postoperative period.

Radio-labelled-leucocyte imaging is becoming accepted as the investigation of choice when infection of prosthetic arterial bypass grafts is suspected. However, alternative sources of inflammation, such as haematoma and, perhaps, the healing and incorporation of newly inserted bypass grafts, are potential causes of false positive results. This study examines the use of technetium-labelled-leucocyte imaging during the postoperative in-hospital period following aortic bypass surgery. Two labelled-leucocyte scans were carried out serially in 20 patients randomly allocated to receive either of two types of Dacron aortic graft in regular use. The earlier scans were undertaken at 2-5 days after surgery and only two of these scans were positive. None of the later scans, undertaken at 7-10 days after surgery, was positive. None of the patients had a graft injection as evidenced by clinical features, leucocyte counts, and C-reactive protein measurements. These results suggest that recent surgery does not in itself significantly reduce the specificity of technetium-labelled-leucocyte imaging.

Home infusion therapy options for patients with AIDS.

With the advent of new catheter technologies and techniques, IV therapy options for the AIDS patient are increasing. To meet the needs of individual patients and achieve the best results with home infusion therapy, both the practitioner and the patient should be well informed about the range of available catheters.

Ribavirin small-particle aerosol treatment of infections caused by influenza virus strains A/Victoria/7/83 (H1N1) and B/Texas/1/84.

In a double-blind study of influenza in a population of college students in 1984, ribavirin small-particle aerosol treatment of 38 patients (18 treated, 20 control) infected with a new antigenic variant, influenza virus strain A/Victoria/7/83 (H1N1), was associated with statistically significant reductions in the height and duration of fever, systemic symptoms, and virus shedding. Patients received a total of 2.4 g of ribavirin over 42 h during 68 h of hospitalization without any side effects. In addition, in a study of patients infected with influenza virus strain B/Texas/1/84 (seven treated, eight control) treated with ribavirin aerosol showed a trend of more rapid recovery than control patients.

Estimation of gestational and skeletal age in Macaca mulatta.

Results of qualitative and quantitative studies of prenatal skeletal development in Macaca mulatta are presented. Longitudinal radiographic observations were carried out on 20 monkeys of known gestational age, beginning on 120 days of gestation until the neonatal stage of skeletal development. These studies were based on multiple uterotomies on each pregnant female. The technique described provides accurate data on prenatal bone ossification, and permits an accurate estimation of fetal age in pregnant rhesus monkeys with unknown conception dates.

Air gap magnification techniques in upper extremity fractures.

Air gap magnification is useful in the diagnosis of upper extremity fractures specifically as it applies to nondisplaced fractures of the radial head, carpals and metacarpals. These injuries often prove difficult if not impossible to detect on routine roentgenographs. Gap magnification is technically simple to perform, requires no special equipment, and has been useful in evaluating subtle roentgenographic changes.
