Extracorporeal piezoelectric shockwave lithotripsy of multiple pancreatic duct stones under ultrasonographic control.

The first ultrasonographically controlled fragmentation of multiple pancreatic duct stones of up to 14 mm size by means of extracorporeal, piezoelectric shockwave lithotripsy is reported. On account of the ultrasound localization and continuous control during therapy a nasopancreatic tube for instillation of contrast medium and frequent x-ray checks were not necessary. The 48-year-old patient did not experience any pain during the four treatment sessions and during the follow-up period. No complications were noted.

Tissue reactions under piezoelectric shockwave application for the fragmentation of biliary calculi.

The tissue reactions that occurred during piezoelectric shockwaves for the fragmentation of biliary calculi were investigated in 10 surgically removed stone containing human gall bladders and in acute (six dogs) and chronic (six dogs) animal experiments. Before and after shockwave (500, 1500 or 3000) in the anaesthetised dogs, computed tomography (CT), magnetic imaging (MRI) and laboratory tests were done; treatment was carried out under continuous ultrasonographic control. Shockwave applications to the human gall bladders resulted in disintegration of the stones with no macroscopically or microscopically detectable tissue changes. In acute animal experiments, small haematomas were observed in all six animals at surfaces, but also inside the liver and gall bladder (max diameter 25 mm). Perforation or intra-abdominal or pleural bleeding did not occur. In chronic experiments, no macroscopic, and only slight microscopic residual lesions (haemosiderin deposits) were seen three weeks after shockwave. In almost all instances, the lesions were detected by CT, MRI, and ultrasonography, while laboratory tests were negative.

[Piezoelectric lithotripsy of gallstones. Initial clinical experiences].

Piezoelectric lithotripsy was undertaken on 50 patients with gallbladder stones, none of them requiring anaesthesia, analgetics or sedatives. Stone fragmentation was achieved in all patients during the first treatment. In 44 patients the maximum fragment size was less than 50% of the initial stone diameter. The mean maximum fragment size after the first treatment was 4.3 mm (+/- 3.3 mm). After a follow-up of 0-2 months in 14 of the 50 patients and of 2-4 months in 6 of 13 patients, no more stones could be seen by ultrasonography. After an average period of 8 weeks, 17 of 50 patients were free of stones. Piezoelectric lithotripsy did not have any severe side effects besides a mild pancreatitis in one patient.

Catheter drainage of pancreatic pseudocysts into the stomach.

This paper reports on the technique and results of cystogastric catheter drainage, as described by Hancke, in eight patients with post-acute pseudocysts, and in one patient with acute necrotising pancreatitis. With this procedure, the catheter is passed, under gastroscopic and ultrasonographic control, percutaneously through the stomach into the pseudocyst in such a manner that the contents of the cyst drain into the stomach. Too small a residual stomach following surgery, and too small, or immature, cysts, represent the limitations of the technique. In five patients, complete, lasting emptying of the cyst was accomplished without any complications. The intervention can be carried out under local anesthesia.

Intralymphatic interleukin-2 treatment of a hemophiliac AIDS patient with defective interleukin-2 production.

To improve immune functions in an interleukin-2 (IL-2) deficient hemophiliac AIDS patient suffering from severe Pneumocystis carinii pneumonia, treatment with IL-2 was started in addition to standard antimicrobial therapy. Highly purified IL-2 was administered subcutaneously and then repeatedly intralymphatically in a manner similar to pedal lymphography. No toxicity was observed. The patient temporarily improved clinically as well as with regard to immunological functions. Particularly the in vitro response to phytohemagglutinin (PHA) could partly be restored, and skin tests revealed improved response to recall antigens. These findings indicate that IL-2 can be administered safely and effectively by the intralymphatic route and may--in addition to antibiotics--be of value in AIDS patients with severe opportunistic infections.

[Percutaneous pancreatico-gastric drainage of a pseudocyst].

Pancreatico-gastric drainage, guided by ultrasound and endoscopy, of pancreatic pseudocysts was performed in two patients. The cysts emptied into the stomach within 12 and 24 hours, respectively. After the drainage both patients developed leucocytosis and fever, which quickly subsided after antibiotic treatment. There were no further complications or recurrences.

Endoscopic ultrasonography of tumours of the oesophagus and the stomach.

Endoscopic ultrasonography is one of the intracavitary ultrasound procedures, all of which have as their objectives the attainment - through the use on high frequencies - of high resolution, and the avoidance of disturbing air and bone. In the technique of endoscopic ultrasonography, a high-resolution ultrasonic transducer attached to the tip of an endoscope is introduced into the gastrointestinal tract, from where it becomes possible to represent the wall of the tract, and also its neighbouring organs. The last few years have shown that the procedure is capable, in principle, of meeting the expectations placed in it.

Experimental investigations into the possibility of differentiating early from invasive carcinoma of the stomach by means of ultrasound.

Using high-resolution ultrasound such as is employed, for example, in endoscopic ultrasonography, the walls of hollow organs are visualized as layered structures. The correlation of these "acoustic layers" with the histological layers, is of particular interest for the staging of early malignant lesions. Both clinically and experimentally, we made an attempt to determine the extent to which, at a transducer frequency of 7 MHz, such a correlation is possible. Our investigations show that, although reflection phenomena can mimic and "overlap" anatomical layers, the important boundary between the submucosa and the muscularis propria is principially identifiable.

[Ultrasonic endoscopy. An assessment after 3 years].

A high-resolution ultrasound head at the tip of an endoscope, used in 259 examinations of 232 patients, has been demonstrated in general to fulfill the expectations placed on it as an important diagnostic tool.

Intralymphatic interleukin 2 treatment in patients with acquired immunodeficiency syndrome: preliminary experience in three cases.

Patients with opportunistic infections during the course of acquired immunodeficiency syndrome (AIDS) were analyzed for cellular immune functions and found to be severely immunocompromised. In particular, interleukin 2 (IL 2) production appeared to be defect not only qualitatively but also quantitatively. In some of these patients, exogenous IL 2 improved immune response in vitro. Intralymphatically administered highly purified natural IL 2 was given repeatedly (over a time period of ten days) to three of these patients. In two cases, such a treatment course was repeated later. Clinical response - at least in some patients - appeared to be of temporary benefit. Shortly after termination of IL 2 application in two patients an increase of lectin responsiveness as well as improved reactivity in skin testing was noted, encouraging further exploration of IL 2 as an immunostimulatory drug in AIDS patients.

Malignant lesions of the upper gastrointestinal tract.

For malignant tumours of the upper gastrointestinal tract, endoscopic ultrasonography is used to assess their depth of penetration, infiltration of neighbouring organs, and metastatic spread to regional lymph nodes. Our experience so far in 124 echo-endoscopic examinations shows that the superior resolution of endoscopic ultrasonography makes possible a very accurate assessment of the depth of penetration of the tumour, and of lymphatic spread to the regional lymph nodes in oesophageal carcinoma. An assessment of infiltrative growth and differentiation from early carcinoma remains problematic.

Endoscopic ultrasonography of the pancreas. Technical aspects.

Endoscopic ultrasonography was mainly developed to improve the diagnostic possibilities in pancreatic diseases. To standardize the technique and to evaluate the diagnostic possibilities of endoscopic ultrasonography, 76 examinations were performed in patients with suspected pancreatic disorders, using two types of ultrasonic device, a sector scanner and a linear array. The pancreatic region was visualized with good resolution, and a definite diagnosis was possible in most patients. Up to now endoscopic sonography has been used specially to supplement upper gastrointestinal tract endoscopy and external ultrasonography.
