Hereditary progressive mucinous histiocytosis in women. Report of three members in a family.

We describe three female patients in a family of two generations, who suffered from generalized and maximally pea-sized histiocytic tumors beginning in early adolescence. The disease ran a uniform and slowly progressive course and was confined to the skin. There were no signs of spontaneous tumor regression. Histologic, immunohistochemical, and ultrastructural examination revealed the histiocytic nature of the tumors. An outstanding finding was a marked production of mucinous material, predominantly in long-standing tumors. This nonlangerhansian syndrome differs from other benign normolipemic histiocytic diseases with generalized histiocytic tumors by inheritance, which is most likely autosomal dominant, and by other clinical and histologic features. Since a storage disease could be excluded, the syndrome presented here seems to be a hitherto unknown entity among the benign generalized histiocytoses.

[Flea bites caused by Archaeopsylla erinacei, the hedgehog flea].

A hedgehog flea was the cause of multiple flea bites in a 48-year-old patient. The main host of the hedgehog flea is the European hedgehog, but the flea was also found in different furry mammals, such as polecats, brown rats and foxes. It was not previously known that Archaeopsylla erinacei attacks man.

[Paraffin granuloma of the penis].

Granulomatous foreign-body reactions following paraffin injections can produce diagnostic difficulties, especially if the patient denies the injections. The paraffin injections serve to enlarge the penis and are mostly performed by psychiatrically patients. Undesired side effects that can occur are solid granulomatous infiltrations or penis deviation with impotentia coeundi. On the basis of a case report, the different motives of the patients and the therapeutic possibilities are presented.

[Subungual keratoacanthoma].

Subungual keratoacanthoma is a very rare condition that varies in its clinical presentation from keratoacanthoma in other localizations. It is a painful lesion that fails to regress spontaneously and shows more vertical growth. On X-ray examination destruction of the underlying bone, caused by pressure, can be seen. It may be difficult to distinguish subungual keratoacanthoma from other subungual tumors, especially squamous cell carcinoma; therefore clinical and histological data should be taken into consideration.

[Paraneoplastic multicentric reticulohistiocytosis: induced and inductable by iodine containing x-ray contrast media].

Following lymphography, a 41-year-old woman developed arthritis and papules of multicentric reticulohistiocytosis three years after the beginning of a carcinoma of the cervix, now inoperable and with vulvar metastases. Two further inevitable x-ray investigations with different contrast media led to exacerbation of the disease within 24 hours. 12 month after onset, all symptoms of multicentric reticulohistiocytosis receded spontaneously, and further investigations with contrast media were well tolerated. We assume that iodine compounds may be the cause--directly or indirectly--for provocation and enhancement of histiocytic proliferation.

Epitheloid hemangioendothelioma of the scalp. A case report with immunohistochemical and ultrastructural study.

A case of an epitheloid hemangioendothelioma of the skin and subcutaneous tissue of the scalp is described. Clinically and histologically a (metastatic) carcinoma had been considered in differential diagnosis. Of great help in recognition of the endothelial nature of this tumor was the immunohistochemical reaction with Factor-VIII associated antigen and Ulex europaeus antigen. Not only tumor cells in solid areas, but also intracytoplasmic vacuoles revealed positive staining. Thereby it could be shown, that this paranuclear vacuoles are actually compatible with developing capillary lumina. Only few tumor cells showed positive reaction with alpha-1-antichymotrypsin, pointing to a possible small histiocytic component of this tumor. Endothelial features of tumor cells and developing capillary lumina also could be demonstrated by electronmicroscopy.

[Undifferentiated cutaneous angiosarcoma of the head: identification by the endothelial marker Ulex europaeus agglutinin I].

Cutaneous angiosarcoma of the head is a rare tumor of the elderly and can occur in an undifferentiated form without any clinical or histological signs of the vascular origin of this tumor. In these cases, the tumor can be identified by using endothelial cell markers, such as factor-VIII-related antigen and ulex europaeus agglutinin I, in an immunofluorescence technique or a peroxidase-antiperoxidase method. A 78-year-old patient is described who died within 18 months from such a tumor, which was diagnosed using the endothelial cell marker, ulex europaeus agglutinin I.

First experiences with a high-resolution ultrasonic scanner in the diagnosis of malignant melanomas.

The use of high-resolution ultrasonic instruments with transducer frequencies above 8 MHz makes it possible to visualize malignant melanomas of a thickness of more than 0.7 mm. While the sonolucent structure has not permitted the dignity of a lesion to be judged to date, it is easy to determine the maximal tumor thickness as well as infiltration of the subcutis. Thus "low-risk" and "high-risk" melanomas may be differentiated preoperatively according to these parameters. The results are of great use for the operator to plan the extent of his intervention, as well as to select patients for prophylactic regional lymphonodectomy.

Lichen planus: histologic involvement of an epidermoid cyst.

Only little is known about the involvement of the epithelium of epidermoid cysts in dermatoses with keratinocytic changes because these cysts are excised only rarely. Histological findings on an epidermoid cyst, excised from a patient with wide-spread lichen planus are reported. These changes exactly agree with the epidermal histological features of lichen planus and cannot be interpreted as a foreign body reaction.

[Simulation of malignant tumors by undeclared polyvinylpyrrolidone in drugs].

Numerous drugs are available containing polyvinylpyrrolidone as a carrier and retard substance. Repeated intramuscular or subcutaneous injections of polyvinylpyrrolidone over a long time can lead to tumor-like foreign body reactions mimicking true malignant tumors by clinical and histological features. Additionally, a storage of polyvinylpyrrolidone in visceral organs is possible in such patients.

[Bone findings in psoriasis].

Three female patients out of 40 psoriatics examined with scintigraphical methods (Tc-diphosphonate) were found to have a diffuse and circumscribed pathological increase of nuclide at the skull. These abarticular signes of proliferation at the bone-area indicate an early affection of skeleton of patients with psoriasis which is similar to the joint-scan that is mostly obtained in positive bone-scan.
