[Prophylaxis of thrombosis in a pregnant woman with congenital antithrombin III deficiency: changes in hemostatic molecular markers before the onset of thrombosis].

A pregnant woman with congenital antithrombin III (AT III) deficiency was given AT III concentrate and warfarin at the first and after 36th week of gestation periods and at the other gestation period, respectively. The patient developed thrombosis in the left leg, but fibrinopeptide A (FPA) and thrombin-AT III complex (TAT) had already shown high values one week before, suggesting the possibility of their being forecast markers of thrombosis. The administration of AT III concentrate caused an improvement in thrombosis. Therefore, in case of high FPA and TAT values as determined one or two times a week, the administration of AT III concentrate was thought necessary. In case of warfarin, however, non-administration during the 6th-9th weeks of gestation for the purpose of avoiding teratogenicity and frequent blood coagulation tests taking heed of overdosage for the purpose of avoiding fetal central nervous system abnormalities were suggested necessary. Delivery was uneventful, both mother and child being doing very well; umbilical blood AT III activity was 18%. It is generally difficult for us to form the diagnosis as AT III deficiency only from AT III activity at neonatal stage, but in the present study, we analyzed the restriction fragment length polymorphism of the AT III gene using umbilical blood, and succeeded in diagnosing the neonatal child as AT III deficiency.

A 7-34 analog of the parathyroid hormone-related protein has potent antagonist and partial agonist activity in vivo.

The biological properties of a new synthetic analog of parathyroid hormone-related protein [PTHrP-(7-34)NH2] were examined in vivo using a well characterized thyroparathyroidectomized (TPTX) rat model. The phosphaturic and urine cyclic AMP response induced by infusion of PTHrP-(1-34)NH2 (0.16 nmol/h) was inhibited by 70% (P less than 0.01, n = 6) by co-infusion of PTHrP-(7-34)NH2 at a 10-fold molar excess (1.6 nmol/h). The 7-34 PTHrP analog also antagonized the PTHrP-(1-34)NH2-induced hypercalcemia and rises in blood 1,25-dihydroxyvitamin D concentrations. However, when infused alone at a higher dose rate (8 nmol/h), PTHrP-(7-34)NH2 displayed significant PTH agonist activity. This profile contrasts to that of [Tyr-34]bPTH-(7-34)NH2 which is comparatively less potent (10-20-fold) with respect to its antagonist activity but has no appreciable agonist activity in vivo.

[Neurological deterioration due to cerebral hyperperfusion following carotid endarterectomy. Case report].

Recently attention has been drawn to postoperative cerebral hyperperfusion after carotid endarterectomy (CEA) associated with a preoperative state of impaired cerebral hemodynamics. Rarely postoperative neurological deficits are caused by cerebral edema due to hyperperfusion. The patient was a 65-year-old male with dysarthria and right hemiparesis. Because of the presence of severe stenosis of the left carotid artery, CEA was performed. On the 6th postoperative day, he developed severe right hemiparesis and aphasia due to cerebral edema in the subcortical region of the left cerebral hemisphere. Left carotid angiography showed normal circulation without evidence of the carotid stenosis. Later the cerebral edema and the neurological deficits gradually disappeared.

[Comparison of screening methods for diabetes mellitus, based on fasting glucose, HbA1C and fructosamine levels].

Screening methods for diabetes mellitus, based on fasting glucose (FPG), HbA1C and fructosamine (FRA) levels were compared with regard to their screening power. The subjects studied were 699 health examinees. A significant elevation of the mean level of each screening index was observed in diabetic subjects, but not in borderline cases compared with that of normal subjects. The FPG, HbA1C and FRA levels in diabetic subjects distributed over a wide range overlapping largely with the distributions of non-diabetic subjects. No appreciable difference in the screening power was observed between FPG and HbA1C but specificity was low in FRG for the comparable sensitivity level. In the screening methods based on the combination of two or more indices, elevation of the sensitivity was noted, but the specificity declined, resulting in an increase of re-examination rate. Among them, the combination of FPG and HbA1C indicated the highest sensitivity.

A case-control study of hepatocellular carcinoma in Osaka, Japan.

A case-control study was undertaken to evaluate the roles of hepatitis B virus (HBV), blood transfusion, alcohol drinking and cigarette smoking in the etiology of hepatocellular carcinoma (HCC) in Osaka, Japan. A total of 229 cases and 266 hospital controls were included in our study. The relative risks of HCC obtained after adjustment for age, sex and other important variables were 14.3 (95% confidence interval (CI): 5.7-36.3) for HBsAg positives, 4.3 (95% CI: 1.9-9.6) for blood recipients and 3.2 (95% CI: 2.0-5.1) for heavy drinkers. A statistically significant dose-response relationship was observed between the risk of HCC and total alcohol consumption. The overall risk for HCC was also significantly elevated among smokers; however, there was no consistent dose-response relationship between the risk and cigarette consumption. We conclude that HBV, blood transfusion and excessive alcohol drinking play important roles in the etiology of HCC in Osaka, Japan. Further investigation is needed to clarify the possible etiological role of smoking.

The synthetic human parathyroid hormone-related protein is inhibited by a parathyroid hormone antagonist in rats in vivo.

The structure of a novel protein, parathyroid hormone-related protein (PTHrP), secreted by human tumors associated with hypercalcemia has recently been determined. Administration of a synthetic fragment of this protein in vivo reproduces features of the clinical paraneoplastic syndrome of humoral hypercalcemia of malignancy and produces biologic responses closely similar to those obtained with parathyroid hormone (PTH). A PTH antagonist designed to reversibly occupy PTH receptors inhibited major actions of the tumor peptide in vivo, including phosphaturia, urinary cAMP excretion, and increased serum ionized calcium. These studies indicate that PTHrP and PTH mediate their bioactivities through shared receptors in vivo and establish a potential specific mechanism-based approach utilizing PTH antagonists for the therapy of tumor-associated hypercalcemia.

Persistent albuminuria as an index of diabetic nephropathy in type 2 diabetic patients in Osaka, Japan-incidence, risk factors, prognosis and causes of death.

A group of 1196 type 2 (non-insulin-dependent) diabetic patients was followed for a mean of 10 years to determine the incidence of persistent albuminuria, its associated risk factors and prognosis, as well as causes of death in patients. None of the patients studied had albuminuria on entry. The mean annual incidence rate of persistent albuminuria per 1000 person-years in the patients was higher in males than in females (18.42 and 12.57, respectively). Development of persistent albuminuria was associated with age at entry, duration of known diabetes, systolic blood pressure, fasting glucose level, presence of diabetic retinopathy and type of treatment. Among 193 patients who developed persistent albuminuria during the observation period, 66 (34.2%) died before the end of the observation period, with a mean survival period (+/- SD) of 3.0 +/- 3.1 years after the onset of persistent albuminuria, indicating an extremely poor prognosis. Renal disease was the predominant cause of death in patients who developed persistent albuminuria, followed by heart disease and cerebrovascular disease.

Mortality and causes of death in type 2 diabetic patients. A long-term follow-up study in Osaka District, Japan.

A follow-up study of 1939 diabetic patients with a mean observation period of 9.4 years was carried out in Osaka, Japan. The mortality rates per 1000 person-years were 31.35 for males and 21.99 for females, and the ratios of observed to expected number of deaths were 1.69 for males and 1.74 for females, indicating an excess mortality for diabetic patients of both sexes and higher mortality in males than in females in Japan. Factors related to the prognosis of the patients were age, elevated fasting glucose level, lower obesity index, hypertension, diabetic retinopathy, and albuminuria at entry to the study. Insulin treatment was also associated with poor prognosis. Cerebro-cardiovascular and renal disease were the major causes of death in diabetic patients; heart disease killed 19.5%, cerebrovascular disease 16.7% and renal disease 13.1%. The relatively high frequency of renal disease as a cause of death in type 2 diabetes, especially in patients with a lower age of onset, was noteworthy, suggesting some difference in the clinical manifestations of diabetes between Japan and Western countries. Malignant neoplasms accounted for 25% of deaths, and cirrhosis of the liver for 6.4%.

[An odontological study on Down's syndrome. Part 3: Dental caries of the deciduous teeth].

The dental caries experience of a sample of 97 children with Down's syndrome who had deciduous dentition was contrasted with that of 57 non affected healthy children. The children with Down's syndrome had a caries experience less than one-third that of the control. However between the two groups, there was no difference in the relative caries pattern between occlusal, surficial and proximal tooth caries. We investigated the relationship between two factors, the delayed eruption pattern and any other environmental factors (tooth bursting, sugar control, nocturnal bottle feeding and caries activity) and the low dental caries experience in children with Down's syndrome. For the eruption factor of the deciduous teeth, we calculated the post eruptive age and excepted the delayed eruption pattern of children with Down's syndrome. It was apparent that two factors did not influence the low dental caries experience of the deciduous teeth in children with Down's syndrome.

A cluster of human T-lymphotropic virus type-I carriers found in the southern district of Tokushima Prefecture.

Since we had experience of eight patients with adult T-cell leukemia lymphoma from 130 cases of non-Hodgkin's lymphoma between 1978 and 1986, a sero-epidemiological survey of anti human lymphotropic virus type-I (HTLV-I) antibody was performed using a gelatin particle agglutination test, an enzyme-linked immunosorbent assay and an indirect immunofluorescence assay for 2,190 adults (768 men and 1,422 women) aged between 21 and 86 years, in the southern district of Tokushima Prefecture. The inconclusive data obtained using the above mentioned methods have been re-examined by western blotting analysis using enzyme-labelled anti IgM and anti IgG. There was an overall prevalence rate of 6.0% (132/2, 190) but higher rate were found in village C (10.9%) and village D (14.2%). These two villages together with town K (6.1% seropositive) form one community unit with a small population in a mountain area, which could be prone to producing a cluster of HTLV-I carriers. Town G with 8.0% sero-positivity is on the Pacific Ocean coast neighboring one of the endemic areas in the eastern section of Kohchi Prefecture. Interestingly, only IgM antibody was detected in 17 of 137 carriers (13%), all females who had never had a blood transfusion, suggesting them to h ave been in a sort of immunodeficient state, as reported in cases of HTLV-I infection. In 13 of 29 HTLV-I carriers (44.8%) from villages C and D, the viral antigen was detected in 1-9% (0.41 +/- 0.19%) of the peripheral blood mononuclear cells after being cultured with phytohemagglutinin for three days. The data indicate that those carriers had evidently been infected with the HTLV-I virus.

Tissue-specific regulation of avian vitamin D-dependent calcium-binding protein 28-kDa mRNA by 1,25-dihydroxyvitamin D3.

We have studied the regulation, by 1,25-dihydroxyvitamin D3 (1,25-(OH)2D3), of vitamin D-dependent calcium-binding protein (28-kDa CaBP) mRNA in chick tissues in vivo. Northern analysis of poly(A)+ RNA was carried out using, as hybridization probes, synthetic oligonucleotides complementary to chick 28-kDa CaBP mRNA. In vitamin D-deficient chicks, 28-kDa CaBP mRNA was virtually undetectable in intestine, was clearly detectable in kidney, and present at the highest levels in cerebellum. After a single intravenous dose of 500 ng of 1,25-(OH)2D3, intestinal 28-kDa CaBP mRNA levels were increased 50-fold, kidney levels were increased 4-fold, and cerebellum levels were unchanged. Increased levels of 28-kDa CaBP mRNA were appreciated 2 h after induction and were maximal at 12 h. Pretreatment of vitamin D-deficient chicks with actinomycin D had little effect on the acute phase of the 1,25-(OH)2D3 induction of 28-kDa CaBP mRNA in intestine but blunted the induction in kidney. Pretreatment with cycloheximide caused a delayed response to 1,25-(OH)2D3 in the intestine, although control (noninhibition) levels of 28-kDa CaBP mRNA were present 12 h after hormone administration. By contrast, in the kidney, cycloheximide pretreatment resulted in an increased steady-state (vitamin D-deficient) level of 28-kDa CaBP mRNA, but completely abolished the induction of 1,25-(OH)2D3. Our studies indicate that, whereas 1,25-(OH)2D3 does not regulate 28-kDa CaBP mRNA levels in the brain, the hormone modulates 28-kDa CaBP gene expression in intestine and kidney in a tissue-specific manner, by acting through both transcriptional and post-transcriptional mechanisms.

Elevated erythrocyte adenosine deaminase activity in a patient with primary acquired sideroblastic anemia.

We report a case of primary acquired sideroblastic anemia (PASA) associated with elevated erythrocyte adenosine deaminase (ADA) activity. The patient was an 85-year-old Japanese male. Analysis of the peripheral blood revealed pancytopenia, and the bone marrow findings showed marked ringed sideroblasts and chromosomal deletion (46XY, 11q-). The erythrocyte ADA activity was 17 times higher than that of normal control, the leukocyte ADA activity was within the normal range, and the plasma ADA activity was 2 times higher than the normal mean. The adenine nucleotides in the patient's erythrocytes were within normal range. According to starch gel electrophoresis, ADA isozyme of the patient was ADA 1. Western blotting showed an increased amount of ADA protein in the patient's erythrocytes. Southern blotting revealed no gene amplification or large structural change. Dot blot analysis of the reticulocyte mRNA showed no increase in the amount of ADA mRNA in the patient's reticulocytes compared with those of reticulocyte-rich controls. We considered that the mechanism of elevated ADA activity in this acquired defect was similar to that found in hereditary hemolytic anemia associated with ADA overproduction.

Genetic analysis of adenosine deaminase expression in adult T-cell leukemia.

We investigated the genetic basis for the increased adenosine deaminase (ADA) expression in adult T-cell leukemia (ATL). We found a correlation between the levels of ADA-specific mRNA and ADA immunoreactive protein in ATL. Southern blot analysis revealed no gene amplification or rearrangement of the ADA gene. These findings indicate that increased ADA expression in ATL cells reflects increased transcriptional activity for the ADA gene or increased stability of ADA mRNA.

Effects of oxidation of human parathyroid hormone on its biological activity in continuously infused, thyroparathyroidectomized rats.

The effect of oxidation of human parathyroid hormone 1-34 (hPTH 1-34) on the hormone's biological activity was assessed in vivo using a multiparameter, thyroparathyroidectomized (TPTX) rat model. The peptide was oxidized by treatment with hydrogen peroxide, and the oxidized form (8,18-methionine sulfoxide) was isolated by reverse-phase HPLC. Vitamin D-deficient rats were infused with either intact or oxidized hormone along with a 5 mM calcium chloride solution for 4 or 18 hr. Infusion of nonoxidized hormone (0.1-0.8 nmoles/hr) resulted in dose-dependent increases in serum calcium, decreases in serum phosphate, decreases in urine calcium, increases in urine phosphate and cAMP, and increased renal 1,25-dihydroxyvitamin D3 (1,25 (OH)2D3) production. Oxidized PTH infused at doses up to 0.8 nmole/hr had no effect on any of these parameters. To assess the effect of oxidation on the ability of PTH to inhibit the production of the 24,25-dihydroxyvitamin D3 (24,25(OH)2D3), the infusion protocol was performed in vitamin D-deficient rats repleted with 1,25(OH)2D3 by injection. In these experiments, intact hormone markedly suppressed 24,25(OH)2D3 production, whereas the oxidized form was without effect. We conclude that intact methionine residues at positions 8 and 18 of hPTH 1-34 are necessary for all its major biological actions, including its effect on the renal metabolism of 25-hydroxyvitamin D3(25(OH)D3).

In vitro antitumor activity of mitomycin C derivative (RM-49) and new anticancer antibiotics (FK973) against lung cancer cell lines determined by tetrazolium dye (MTT) assay.

Seven small- (SCLC) and four non-small-cell (NSCLC) lung cancer cell lines were used to examine the in vitro cytotoxicity of cytotoxic drugs such as (1aS-(1a alpha,8 beta,8a alpha,8b alpha]-8-[aminocarbonyl)oxy)methyl)-4,8a- dimethoxy-1,1a,2,8,8a,8b-hexahydro-7-hydroxy-5-methyl-6- nitrosoazirino(2',3':3,4)-pyrrolo-(1,2-a)indole (RM-49) and 11-acetyl-8-carboxymethyl-4-formyl-14oxa-1,11-diaze- tetracyclo(7.4.1.0(2,7),0(10,12]tetradeca-2-4-6-trien-6,9-++ +diyl-diacetate (FK973). In vitro cytotoxicities of RM-49 and FK973 were compared with those of mitomycin C (MMC), cisplatin (CDDP), carboplatin (CBDCA), etoposide (VP16), adriamycin (ADM) and vindesin (VDS). Drug sensitivity was determined using a tetrazolium (MTT)-based assay. Average IC50 values of these two drugs were not statistically different compared with that of MMC, although FK973 showed strong antitumor activity against SCLC cell lines such as LT3, N857, and H69 at the same concentration. The predicted peak plasma concentration (predicted PPC) calculated by the formula proposed by Scheithauer, log (predicted PPC) = -0.788 + (0.755 x log(LD50], and relative antitumor activity, RAA (PPC/IC50), of RM-49 were higher than those of other drugs such as MMC, CDDP, CBDCA, and ADM against SCLC cell lines (P less than or equal to 0.05), and those of FK973 were also higher than those of other drugs such as MMC, CDDP, CBDCA, and ADM against SCLC cell lines (P less than or equal to 0.05). Based on these promising in vitro studies, the clinical trials of RM-49 and FK973 were warranted.
