[Intrarenal small tumor of the kidney].

From 1992 to 2002 four hundred and thirty-six patients with kidney tumors were treated. Tumor up to 4 cm ("small" tumor of the kidney) was diagnosed in 143 patients. Intrarenal localization of the tumor was in 35 (24.5%) of them. Two groups of patients with intrarenal tumor were are distinguished: tumor is localized intrasinusly whole (18 patients) or closer to fibrous capsule of the kidney (17 patients). In the majority of cases (31 patients) of small intrarenal tumor radical nephrectomy was performed. Four patients with absolute and relative indications to organ-saving surgery underwent nephrotomy and enucleoresection of an intrasinus tumor. Renal-cell cancer was in 3 of these patients, angiomyolipoma--in 1. All 3 patients with renal cancer are alive in follow-up for 7, 2 and 1.5 years.

Angiomyolipoma↗

[A "small" kidney tumor].

The authors think it necessary to distinguish small tumors of the kidney--a neoplasm 4 cm in size maximum. The number of such small tumors account for 1/3 of all renal newgrowths. An important role in detection of small tumors belongs to ultrasonography. Ultrasonic finding of the tumor should be followed by spiral computer tomography with 3-dimensional reconstruction of the image of the tumor, calyceal-pelvic system and renal vessels. Small tumors are asymptomatic in 71.4 cases, in many cases benign, in renal cell carcinoma their cells are well differentiated, they do not invade paranephral fat. Morphological verification is effective and safe in puncture biopsy of the tumor under ultrasound control. Adequate treatment depends on correct diagnosis of the tumor nature. Surgery of choice in small tumor of the kidney is resection.

[Scope of lymphadenectomy in organ-sparing operations in kidney cancer patients].

Organ-saving operations (extended lymphadenectomy) were performed in 125 patients with renal carcinoma (RC) in the urological clinic of I. M. Sechenov Moscow Medical Academy between 1992 and 2002. In view of specific segment-by-segment lymph flow off the right and left kidneys, it is thought valid to remove paracaval and aortocaval lymph nodes in saving operations on the right kidney but in cancer of the left kidney it is recommended to remove paraaortic lymph nodes. Basing on the anatomical features, lympho-dissection should be made from the crus of the diaphragm to bifurcation of the aorta. Metastases to the regional lymph nodes were diagnosed in 12 (9.6%) patients. In 8 of 12 patients with pN+ lymph node metastases were microscopic. Frequency of lymphogenic metastases directly correlated with the size and stage of the tumor, cancer cells differentiation degree. Metastatic involvement of the regional lymph nodes in renal cancer is an unfavourable prognostic factor as 5-year survival for the overall group of patients was 71.7% while for patients with pN+ it was 41.7%.
