Etiology of nonspecific ulcerous colitis (NSUC) as well as its pathogenetic mechanisms remain obscure. Cytokines are presently shown to play an essential role in development of inflammation. Cytokines are produced by the cells in the inflammatory infiltrate in NSUC, therefore, changes of their level in the blood and biopsies from colon mucosa is naturally determined. Current investigations register various changes in the levels of both proinflammatory and antiinflammatory cytokines. Further studies of unknown mechanisms of cytokines action may contribute to design of new methods of NSUC management.
AIM: To examine validity of using interferon inductors (amixin and cycloferon) in combined treatment of patients with non-specific ulcerous colitis (NUC). MATERIAL AND METHODS: 113 NUC patients received basic antiinflammatory therapy (glucocorticoids and preparations of 5-aminosalycylic acid). Patients of groups 1 and 2 received interferon inductors (amixin and cycloferon in tables), respectively. Patients of groups 1a and 2a received placebo. All the patients were examined clinically with evaluation of interferon status and cellular immunity before and after the treatment. Colon mucosa biopsies obtained endoscopically were studied histologically. RESULTS: Alpha- and gamma-interferon production by leukocytes was substantially suppressed in all the examinees against normal levels of serum and spontaneous interferon. The interferon inductors stimulated relief of clinical symptoms and eliminated endoscopic signs of the disease. Amixin and cycloferon normalized interferon status in 36.3 and 33.3% of the treated patients, respectively. No response was registered in 9.1 and 8.4%, respectively. Improvement was seen in 54.6 and 58.3%, respectively. CONCLUSION: Cycloferon and amixin, interferon inductors, are effective in the treatment of NUC.
24-hour monitoring of gastric juice pH and esophagogastroduodenoscopy with biopsy and test for H. pylori (HP) were conducted in 117 patients with ulcer before and after treatment with famotidine (quamatel, Hungary) and device sound stimulation (DSS). Combined eradication therapy (quamatel 40 mg/day, metronidazole 500 mg 4 times a day, amoxicilline 750 mg 3 times a day, DSS) eradicated HP in 100% of patients with gastric ulcer and 91.7% of patients with duodenal ulcer. Scarring of the ulcer defect occurred for 11.8 +/- 1.1 and 15.3 +/- 1.4 days, respectively. These results are better than those of monotherapy with quamatel or DSS.
The author analyzes results of treatment of 248 patients. The method which allows the duration of temporary invalidity to become 2-3 times shorter is described.
Under analysis are results of treatment of 150 patients with different forms of acute and chronic thrombophlebitis under conditions of polyclinic. Methods of treatment are described which allowed the time of invalidism to become 2-3 times shorter.
