Butyl-2-cyanoacrylate fixation of mandibular osteotomies.

Butyl-2-cyanoacrylate is an easily applied, bioresorbable, noninfective glue that may provide an alternative to conventional craniofacial fixation techniques. Recent studies have demonstrated that the healing and strength of glue fixation of bone fragments in the craniofacial skeleton is equivalent to that of plate and screw fixation. We sought to determine if this glue could also provide strength and healing in a more stressed environment, that of the mandible. Fourteen New Zealand White rabbits underwent mandibular osteotomies at the angle. The osteotomy was fixed with either a microplate (n = 4) or glue (n = 10). Mandibles were assessed 10 weeks postoperatively and classified according to bone healing, condylar morphology, and dental morphology. Callus and nonunion were more prevalent in the glued group, whereas condylar resorption was similar in both experimental groups. Despite the poor bony healing in the glued group, the teeth did not decay but modified their standard form to meet the demands of the anatomic deformity. Butyl-2-cyanoacrylate does not offer the biomechanical stability afforded by plates and screws in bone subject to large forces.

Monoclonal antibody against Babesia equi: characterization and potential application of antigen for serodiagnosis.

Monoclonal antibody (MAb) BEG3 was produced against Babesia equi parasites to define a species-specific antigen for diagnostic use. The MAb reacted with single, paired, and Maltese cross forms of B. equi, and no reaction was observed with this MAb on acetone-fixed Babesia caballi, Babesia ovata, or Babesia microti parasites in the indirect immunofluorescent antibody test. Confocal laser and immunoelectron microscopic studies showed that the antigen which was recognized by this MAb was located on the surface of B. equi parasites. This MAb recognized a 19-kDa protein of B. equi antigen and did not react with B. caballi antigen or normal horse erythrocytes in immunoblot analysis. This MAb also significantly inhibited the in vitro growth of the B. equi parasite. Preliminary studies using partially purified antigen, which was separated by high-pressure liquid chromatography and recognized by the MAb, suggested that it is a suitable antigen for enzyme-linked immunosorbent assay detection of anti-B. equi antibodies in naturally infected horse sera.

Packaging cells based on inducible gene amplification for the production of adeno-associated virus vectors.

Although vectors based on adeno-associated virus (AAV) offer several unique advantages, their usage has been hampered by the difficulties encountered in vector production. In this report, we describe a new AAV packaging system based on inducible amplification of integrated helper and vector constructs containing the simian virus 40 (SV40) replication origin. The packaging and producer cell lines developed express SV40 T antigen under the control of the reverse tetracycline transactivator system, which allows inducible amplification of chromosomal loci linked to the SV40 origin. Culturing these cells in the presence of doxycycline followed by adenovirus infection resulted in helper and vector gene amplification as well as higher vector titers. Clonal producer cell lines generated vector titers that were 10 times higher than those obtained by standard methods, with approximately 10(4) vector particles produced per cell. These stocks were free of detectable replication-competent virus. The lack of a transfection step combined with the reproducibility of stable producer lines makes this packaging method ideally suited for the large-scale production of vector stocks for human gene therapy.

Stretch force on vascular smooth muscle cells enhances oxidation of LDL via superoxide production.

Hemodynamic forces on vasculature profoundly influence atherogenesis. We examined the effect of stretch force on the oxidation of low-density lipoprotein (LDL) by rat aortic smooth muscle cells (RASM) and superoxide production. Stretch force was imposed on RASM cultured on deformable dishes by stretching the dishes. Incubation of native LDL with static RASM for 24 h resulted in LDL oxidation as indicated by increases in thiobarbituric acid-reacting substances from 9.5 +/- 2.3 to 24.5 +/- 2.3 nmol malondialdehyde/mg. Stretch force on RASM augmented cell-mediated LDL oxidation to 149.3 +/- 17.1% concomitantly with increase in superoxide production. LDL oxidation was inhibited by superoxide dismutase or depletion of the metal ion in the culture medium, indicating that it was a metal ion-dependent and superoxide-mediated process. The enhancement of LDL oxidation by stretch force was inhibited by diphenyliodonium, indicating the involvement of the NADH/NADPH oxidase system. Our findings suggest that the increased oxidant stress induced by stretch force is one of the potential mechanisms whereby hypertension facilitates atherosclerosis.

Detection of human papillomavirus type 2 related sequence in oral papilloma.

Oral papilloma is a benign tumourous lesion. Part of this lesion is associated with human papillomavirus (HPV) infection. We analysed the genetical and histopathological evidence for HPV type 2 infection in three oral papillomas. Southern blot hybridization showed HPV 2a sequence in one lesion. Cells of the positive specimen appeared to contain high copy numbers of the viral DNA in an episomal state. In situ staining demonstrated virus capsid antigen in koilocytotic cells and surrounding cells in the hyperplastic epithelial layer. Two other specimens contained no HPV sequences by labeled probe of full length linear HPVs 2a, 6b, 11, 16, 18, 31 and 33 DNA under low stringency hybridization conditions. These results showed the possibility that HPV 2 plays a role in oral papilloma.

Expression of neuropsin mRNA in the mouse embryo and the pregnant uterus.

Neuropsin is a novel serine protease whose mRNA is expressed in the mouse central nervous system. We examined the expression of neuropsin mRNA during embryonic development using Northern and in situ hybridization in non-neural tissues. The pregnant uterus showed strong expression of neuropsin mRNA, whereas the nonpregnant uterus did not express this mRNA. Expression was first detected in the primary decidual zone at 5.5 days post coitum and was maximized at 10 days post coitum, decreasing remarkably thereafter. During mouse organogenesis, neuropsin expression was observed in the developing heart, lung, thymus, pituitary, choroid plexus, and epithelial linings of the skin, oral cavity, tongue, esophagus, and forestomach. In adult mouse organs, neuropsin mRNA was expressed in epithelial tissues covered by keratinocytes with moderate density, whereas low expression was observed in lung, thymus, and spleen. Neuropsin mRNA expression in developing organs and adult keratinocytes suggests that neuropsin is associated with extracellular matrix modifications and cell migrations.

Quinone-dependent tertiary amine N-oxide reduction in rat blood.

Rat blood exhibited a significant quinone-dependent N-oxide reductase activity towards imipramine N-oxide. The reduction mediated by the blood proceeded in the presence of both NAD(P)H and menadione under anaerobic conditions. When menadione was replaced with 1,4-naphthoquinone or 9,10-phenanthrenequinone, similar results were obtained. The reduction was also mediated by the combination of rat erythrocytes and plasma. The reducing activity was inhibited by dicumarol and carbon monoxide. When boiled plasma was combined with untreated erythrocytes, the N-oxide reducing activity was abolished. In contrast, when boiled erythrocytes were combined with untreated plasma, the activity was unchanged. These results suggest that the activity is caused by the heme of hemoglobin in erythrocytes and quinone reductase in plasma. In fact, erythrocytes and hemoglobin have the ability to reduce the N-oxide when supplemented with DT-diaphorase purified from rat liver in the presence of both NAD(P)H and menadione. Hemoglobin also exhibits N-oxide reductase activity with reduced menadione (menadiol). Furthermore, hematin exhibits a significant reducing activity in the presence of menadiol. The reduction appears to proceed in two steps. The first step is enzymatic reduction of quinones to dihydroquinones by quinone reductase(s) with NADPH or NADH in plasma. The second step is nonenzymatic reduction of imipramine N-oxide to imipramine by the dihydroquinones, catalyzed by the heme group of hemoglobin in erythrocytes. Cyclobenzaprine N-oxide and brucine N-oxide are similarly transformed to the corresponding amines by the above reducing system in blood. These results suggest that blood plays an important role in the reduction of tertiary amine N-oxides to tertiary amines.

Effects of peptidase inhibitors, [D-Ala2, Met5]-enkephalinamide and antiserum to methionine-enkephalin microinjected into the caudal periaqueductal gray on morphine withdrawal in rats.

We examined the involvement of enkephalins in the caudal periaqueductal gray (cPAG) in morphine withdrawal in rats. Rats were treated with increasing doses of morphine (20-30 mg/kg/day, s.c., for 5 days) to develop morphine dependence. Morphine withdrawal was induced by naloxone (5 mg/kg, s.c.) 24 hr after the final morphine injection. The level of preproenkephalin (PPE) mRNA in the cPAG was estimated by quantitative in situ hybridization. PPE mRNA in the cPAG was increased 4-24 hr after naloxone in morphine-treated rats. A mixture of peptidase inhibitors (0.5 microl of a solution of amastatin, captopril and phosphoramidon, 3 x 10(-3) M each) microinjected into the cPAG suppressed morphine withdrawal (a decrease in the number of jumping, chin rubbing, paw rubbing and teeth chattering). Antiserum to methionine-enkephalin (1:10 dilution) microinjected into the cPAG did not significantly aggravate morphine withdrawal with or without the mixture of peptidase inhibitors. However, [D-Ala2, Met5]-enkephalinamide (20 nmol), an enkephalin analog, injected into the cPAG decreased the number of jumping without any influence on the other withdrawal signs. These results suggest that the increase in enkephalins in the cPAG may participate in the alleviation of morphine withdrawal (jumping behavior).

Expression of inhibin alpha-subunit in horse testis.

Inhibin is believed to play roles in the pituitary secretion of FSH and in the paracrine regulation of testicular function. Although it has been generally accepted that inhibin is produced in Sertoli cells, there was a recent evidence for the localization of inhibin in Leydig cells of primates, rat and sheep. However, there is no report on the expression of inhibin in the adult horse testis. Therefore, using immunohistochemistry, western blotting and in situ hybridization techniques, the present study examined inhibin alpha-subunit (Ih-alpha) expression in the adult horse testis. For the detection of Ih-alpha protein, we used anti-porcine Ih-alpha antibody in immunohistochemistry and western blotting. Furthermore, digoxigenin-labeled complementary RNA probes were prepared to detect intracellular messenger RNA (mRNA) of Ih-alpha. Immunostainings for Ih-alpha were found not only in Leydig cells but also in Sertoli cells. The intensity in Leydig cells was stronger than in Sertoli cells. Immunoreactivities for Ih-alpha were found at approximately 46 kDa, 56 kDa and 90 kDa in the homogenates from testicular interstitial tissues. The bands at 56 kDa and 90 kDa agree with previous report, but not at 46 kDa. Signals for mRNA of Ih-alpha by in situ hybridization were detected in Leydig cells and in the basal region of seminiferous epithelium including Sertoli cells. These results suggest that Ih-alpha is expressed in Leydig cells and Sertoli cells of horse testis, and the expression level should be higher in Leydig cells than Sertoli cells.

The application of molecular biological tools to epidemiology of African trypanosomosis.

Difficulties have often been encountered in the field surveys due to a lack of definitive morphological characters, particularly where mixed infections are expected. To address this problem, some molecular biological techniques such as DNA probe hybridization, restriction fragment length polymorphism (RFLP) analysis, the polymerase chain reaction (PCR), analyses of ribosomal DNA, and pulsed-field gel electrophoresis (PFGE), have been applied to the analysis of field samples collected during epidemiological surveys of African trypanosomosis. Concurrent natural infection of different individual tsetse flies and mammalian hosts with different species of the trypanosomes have been demonstrated, through the use of a combination of specific DNA probe hybridization and the PCR. Molecular karyotypes of Trypanosoma brucei species were analyzed by PFGE in 45 - 2,000 kb range. There are distinctive differences in intermediate and mini-chromosomes among the strains. We have compared the nucleotide sequences of ribosomal DNAs of the parasites by PCR techniques. From this data new phylogenetic tree can be inferred. It is apparent that these technologies can provide powerful tools for identification and diagnosis of trypanosomes in their hosts and vectors, and for their more accurate phylogenetic classification.

[Molecular biology of human herpesvirus 6: DNA replication and trans-activator genes].

HHV-6 genes and their functions that are unique and different from those of HCMV are summarized. HHV-6 encodes a HSV-1 UL9 homolog that is an origin-binding protein (OBP) essential for HSV-1 DNA replication, although HCMV has no clear homolog. The HHV-6 OBP binds to sequences with similarity to alpha-herpesvirus replication origin that lie within a genomic segment serving as an origin in transient assays. Consensus sequence for OBP binding, protein domain analyses and a model for OBP-origin interactions are described. HHV-6 encodes an AAV-2 rep homolog. Its transcript is expressed at low mRNA levels as a spliced transcript. The gene product suppresses both transformation by H.ras and transcription by HIV-1 promoters. HHV-6 specific transactivator gene DR7 exhibits malignant transforming activity and its protein binds to p53. Studies on IE-A locus and an HCMV UL69 homolog are also discussed.

[A case of mediastinal neurilemmoma originating from the intrathoracic vagal nerve].

A case of mediastinal neurilemmoma originating from the left intrathoracic vagal nerve was reported. A 42-year-old female was admitted to our hospital because of evaluation of an abnormal shadow on chest X-ray. Both CT and MRI showed that the tumor was located in middle mediastinum between the aortic arch and the left pulmonary artery, and the diagnosis was suspected to be neurilemmoma originating from the intrathoracic vagal nerve. Left standard thoracotomy was performed and the tumor was resected by amputating the vagal trunk. Pathological diagnosis was benign Antoni A and B type neurilemmoma. The postoperative course was uneventful with transient hoarseness. The patient is doing well with no relapse at 4 years 6 months after the operation. Mediastinal neurilemmoma originating from the vagal nerve is rare and we discussed both the diagnosis and the surgical treatment for this disease.

[Comparative studies of video-assisted thoracoscopic surgery versus thoracotomy for primary spontaneous pneumothorax].

Between April 1992 and March 1997, 32 patients with primary spontaneous pneumothorax were operated in our institution. In the first three years, 9 patients underwent thoracotomy which was classified into two groups axillary thoracotomy group (AX group) in 5 cases and postero-lateral thoracotomy group (PL group) in 4 cases. In the recent two years, 23 consecutive patients underwent video-assisted thoracoscopic surgery (VATS group). The operating time, intraoperative blood loss, serum CRP and CK level, the duration of the indwelling chest tube, postoperative hospital stay, the duration of epidural anesthesia, complications and recurrences were compared among the three groups. The CRP level was significantly lower in VATS group than in both AX group and PL group. The duration of indwelling chest tube and postoperative hospital stay were significantly less in VATS group than in both AX group and PL group. There was no significant difference in complications and recurrences among the three groups We conclude from findings that VATS is superior to thoracotomy in the treatment of primary spontaneous pneumothorax. However more careful intraoperative manipulation is required to prevent postoperative recurrence.

Biomechanical comparison of antegrade and retrograde nailing of humeral shaft fracture.

A pair-controlled study was conducted to compare biomechanical properties of antegrade and retrograde nailing of humeral fractures. First, six paired fresh anatomic specimen humeri were used to compare the properties of humeri fractured at the middle to distal diaphyses junction that were nailed from the retrograde approach with the Humeral Locked nail with those of contralateral intact humeri. An 18 additional pairs were divided into three equal groups by distal, proximal, or mid-diaphysis location of a standardized 5-mm bone defect to simulate unstable fractures. The retrograde and antegrade nailings were performed in each pair in a random manner. Nail and bone constructs were tested for bending stiffness by nondestructive three-point bending and for torsional stiffness by destructive torsional tests. Compared with intact humeri, fractured humeri fixed with nails had 28.6% posteroanterior and 31.4% mediolateral bending stiffness, 22.5% torsional stiffness, and 43.3% failure torque. For distal fractures, retrograde nailing showed significantly more initial stability and higher bending and torsional stiffness; for proximal fractures, antegrade nailing showed similar properties. For middle to distal diaphyses junction fractures, retrograde and antegrade nailing were indistinguishable. The defect created as an entry portal for retrograde nailing reduced the bone strength only 11.1%. These results suggest that retrograde nailing, which is less detrimental to shoulder function than is antegrade nailing, is an acceptable alternative treatment for humeral shaft fractures. In addition, nailing from the short to the long bone segments can improve mechanical properties of the fixation construct because of better nail and bone interface purchase.

[A case report of video-assisted thoracoscopic resection for a pericardial cyst].

We report here a case of a pericardial cyst resected by video-assisted thoracoscopic surgery. The patient was a 38-year-old male who was incidentally pointed out as having an abnormal shadow close to the superior vena cava on a chest X-ray film. A pericardial cyst was suspected after magnetic resonance imaging showed low intensity in T1 weighted image and high intensity in T2 weighted image. Video-assisted thoracoscopic surgery was carried out under a general anesthesia. The cyst, sized 2 x 2 cm, was recognized between the superior vena cava and right atrium. The cyst was resected successfully and surgery took only 30 minutes. Postoperative course was uneventful and the pathological diagnosis was pericardial cyst. We think video-assisted thoracoscopic surgery is a useful method for the final diagnosis and treatment of pericardial cyst.

[Electron microscopic observations of the conjunctiva in epidermolysis bullosa hereditaria].

A 35-year-old female had been clinically diagnosed as having epidermolysis bullosa hereditaria since her skin formed blisters from the time of her birth. Her left and right corneas become clouded at the age of 17 and 31 years, respectively. She was diagnosed as having bilateral secondary glaucoma at the age of 31 years and was medicated with antiglaucoma drugs. When examined by us, both corneas were invaded by conjunctival tissues and intraocular pressure in both eyes was 38 mmHg. Visual acuity was 0.09 (n.c.) in the right eye and she had no light perception in the left eye. A trabeculectomy OD was performed at our hospital. For confirmatory diagnosis, a pathological examination of the conjunctival and dermal tissues was done. Electron microscopic study demonstrated blister formation beneath the basal lamina and lost anchoring fibrils of both tissues. This finding confirmed that she had a receive form of dystrophic epidermolisis bullosa. This is a rare case of dystrophic epidermolisis bullosa with severe corneal changes and glaucoma diagnosed from electron microscopic observations.

[Successful video-assisted thoracoscopic bullectomy and Nd-YAG laser ablation].

We report here two cases of giant/multiple emphysematous bullae treated with video-assisted thoracoscopy. The first case was a 35-year-old male who was referred to our hospital because an abnormal shadow was casually pointed out in a chest roentgenogram. Chest computed tomographic scan showed giant bulla in the left upper lobe. The second patient was a 37-year-old male who had a symptom of shortness of breath on exertion. Chest roentgenogram and computed tomographic scan revealed multiple bullae at bilateral apical regions. They were performed resection of bulla with stapler and Nd-YAG laser ablation to the emphysematous surroundings using video-assisted thoracoscopy. They had recovered sufficiently to be discharged from our hospital. We think Nd-YAG laser ablation is effective to prevent postoperative air leakage and recurrence of bullae because it makes surrounding tissue tight and continuous.

[CABG after patch angioplasty of the left main coronary: artery report of a case].

A 57-year-old man was admitted to our hospital because of restenosis of the left main coronary trunk (LMT) after patch angioplasty for the LMT lesion. PTCA was repeated four times during three years after patch angioplasty, but recent coronary angiogram still demonstrated 75% restenosis of the LMT lesion. Double-CABG was performed to LAD and LCX using the left internal thoracic artery and saphenous vein graft. Postoperative coronary angiogram revealed an excellent result. A careful consideration must be given to the indication of the patch angioplasty of the LMT lesion.
