Effect of beta blockade and beta stimulation on stage fright.

Stage fright, physiologically the "fight or flight" reaction, is a disabling condition to the professional musician. Because it is mediated by the sympathetic nervous system, we have investigated the effects of beta blockade on musical performance with propranolol in a double blind fashion and the effects of beta stimulation using terbutaline. Stage fright symptoms were evaluated in two trials, which included a total of 29 subjects, by questionnaire and by the State Trai Anxiety Inventory. Quality of musical performance was evaluated by experienced music critics. Beta blockade eliminates the physical impediments to performance caused by stage fright and even eliminates the dry mouth so frequently encountered. The quality of musical performance as judged by experienced music critics is significantly improved. This effect is achieved without tranquilization. Beta stimulating drugs increase stage fright problems, and should be used in performing musicians only after consideration of the detrimental effects which they may have on musical performance.

Staining clinical specimens for acid-fast bacilli by means of a mechanical conveyor system.

A Cyto-Tek staining machine designed for Papanicolau staining of cervical smears has been adapted for auramine staining of smears of sputum and other clinical specimens for acid-fast bacilli. The results compared favourably with those obtained by a manual method of staining. Contamination of negative smears by acid-fast bacilli from positive smears was not found. Since this study the machine has continued to be in routine use with a considerable saving in labour.

Bilateral cataract extraction in one session: report on five years' experience.

Six hundred and seventy-six patients had both their cataracts removed in one operating session because of the rural and economic conditions of Malawi. These operations were performed over a period of 5 years. The final visual results were satisfactory and the number of complications acceptable in relation to the environmental conditions and medical facilities available. However, the incidence of infection is higher than would be acceptable in a sophisticated medical environment. Three eyes were lost owing to infection, but fortunately in all 3 cases the fellow eye was not affected. Furthermore, broken capsule and vitreous loss occurred more frequently than expected in a sophisticated facility in the eye first operated on. This allowed measures to be taken by the surgeon in many cases to prevent these complications from recurring in the second eye.

Computed tomography of retroperitoneal castleman disease (plasma cell type) with sonographic and angiographic correlation.

The CT and sonographic appearances of a case of retroperitoneal Castleman disease of the plasma cell type are reported along with angiographic correlation. In this case CT demonstrated a minimally calcified homogeneously enhancing retroperitoneal mass. Sonography showed the mass to be hypoechoic with good through transmission. The lesion was hypervascular with prominent early venous drainage on inferior mesenteric arteriography.

[Local anesthesia for children: a critical step].

To realise a good child's dentistry, an excellent analgesia is primordial. Local anaesthesia is most useful for reliable and complete dental care on one tooth during one single appointment. But the young patient is afraid by the "ting", making an association with disagreeable or sore souvenirs as a vaccination. First contact with the dentist is the very important; from psychological point of view, situation has to be dedramatized; from technical point of view, anaesthesia has to be not painful and efficacious. In pedodontics, the best technical procedure is an infiltration anaesthesia.

[The deciduous tooth: therapeutic choices].

Deciduous tooth diagnostic and treatment plan put a specific problem down. Therapeutical choice depends, not only of the child general health, but also of the deciduous tooth physiological period, its pathology, and the young child cooperation. Complexity of deciduous tooth biological cycle gives necessary, for the pedodontist, accuracy and a good clinical sense.
