External fixation of complex femoral shaft fractures.

Conversion of temporary external fixation to an intramedullary nail within the first 2 weeks after a femoral shaft fracture is standard practice. However, due to financial constraints, in large parts of the world external fixation of femoral shaft fractures is often the definitive treatment. Out of 60 fractures, 47 were followed-up for a minimum period of 39 weeks. The average follow-up time was 75 weeks. Fourteen fractures were closed, and 33 open. Forty-four fractures united at an average of 31 weeks. There were four non-unions, three of them infected. Secondary surgical procedures were performed for four non-unions and in eight cases of delayed union. One re-fracture occurred, which was successfully treated with repeat external fixation. Only six patients regained full range of motion. The average flexion was 72 degrees . Pin tract infections occurred in 26 patients, leading to loosening of four pins. Satisfactory results can be obtained with definitive external fixation of femoral shaft fractures. Pin tract infections, although a common occurrence, are not a major problem and can be treated by local wound care and antibiotic therapy. The most common problem is significant decrease in the range of motion of the knee.

Accidental ligature strangulation deaths in East Delhi (India).

Ligature strangulations are usually homicidal. Accidental cases are quite uncommon, especially among adults. A retrospective ten-year study conducted in East Delhi revealed that in a total of 98 cases of death due to strangulation, five cases (approximately 5%) among all medico-legal deaths were of fatal ligature strangulation, involving adults who were neither intoxicated nor indulging in paraphiliac activity. The causative agent in three cases was a cycle rickshaw, in one case it was a rotatory part of a machine, and the fifth case occurred due to entanglement while tying up a cow. The cases and their autopsy findings have been described in this paper, together with the review and preventive aspects of such cases.

Performance and persistence of phosphate solubilizing Azotobacter chroococcum in wheat rhizosphere.

Survival and establishment of inoculant strains of Azotobacter chroococcum NV 11 and its mutant NV 43 were assessed in sterilized soil and rhizosphere soil of wheat plants at 7 and 15 d interval, respectively, after sowing, i.e., up to 45 d under pot-house conditions. There was an apparent decrease in population of both strains in bulk soil but a steady increase was observed in root zones of the wheat plant. 10(3) to 10(4) introduced bacteria per g root were found sticking to roots. Further studies indicated that inoculant strains of Azotobacter could survive, proliferate and establish well in root zones.

Influence of analogue-resistant mutants of Azotobacter chroococcum solubilizing phosphate on yield and quality of sunflower (Helianthus annuus).

Two phosphate-solubilizing analogue-resistant mutants of Azotobacter chroococcum showed a positive interactive effect on sunflower (Helianthus annuus). Plant height, leaf area, seed mass, seed yield, protein and oil content increased significantly by inoculation with A. chroococcum mutants. Both levels of fertilizers applied (80N + 30P2O5 and 80N + 60P2O5 kg/hm2 showed increased seed yield. Likewise, inoculation resulted in a significantly higher yield. Application of P-solubilizing A. chroococcum strains with the lower fertilizer level showed no significant difference on plant growth parameters when compared with the higher fertilizer level, indicating that inoculation of P-solubilizing Azotobacter strains can reduce the necessary levels of fertilization from 60P2O5 to 30P2O5 kg/hm2.

Gibberellic acid (GA3) induced changes in proanthocyanidins and malt quality of two- and six-row husked barleys.

Analysis of husked barleys for proanthocyanidins and malt quality attributes has shown that not a single variety is free of proanthocyanidins. The proanthocyanidins in barley grains varied from 3.85 to 4.94 mg/g as catechin equivalent. The concentration of proanthocyanidins decreased, while total soluble sugars, reducing sugars, diastatic power and beta-amylase activity increased during maltings as well as with exogenous gibberellic acid (GA3) application. Alfa 93 (two-row) and RD2560 (six-row) varieties appeared to be superior for malting and brewing purposes on the basis of proanthocyanidins, total phenols, diastatic power and beta-amylase activity. It is suggested that exogenous application of GA3 at 15 ppm may be useful for producing good quality malt from barley grains.

Psychosocial profile of suicide ideators, attempters and completers in India.

The study aims to determine the psychological profile of suicide ideators, attempters and completers in a tertiary care teaching hospital. A total of 260 suicidal ideators, 58 attempters and 55 completers were studied. The majority of ideators, attempters and completers were 26-35 years of age, males (except attempters who were predominantly females), married, literate up to high school, employed (ideators) or housewives (attempters and completers). The suicide ideators, attempters and completers who had a past history of attempt were 6.9%, 24.1% and 18.2% respectively. Family history of attempted suicide or completed suicide was also common among patients suffering from depression. In suicidal ideators, mixed anxiety and depressive disorder was the most common psychiatric diagnosis followed by major depression and schizophrenia. Among suicide attempters, adjustment disorder with depression was the most common diagnosis. The most common method of suicide attempt was organophosphorus compound intake whereas in suicide completers, the most common method in use was hanging. The patients with suicidal ideation or attempt need careful evaluation, early intervention and long term follow up.

Hanging-delayed death (a rare phenomenon).

Hanging is one of the most common methods of suicide in India in which death of the individual occurs instantaneously. However, a few cases have been reported in literature in which death has occurred after a certain period of time or the patient has survived after prolonged resuscitative measures. A case of a 20-year-old female is described who survived for nine days after hanging, remaining unconscious throughout in the hospital. She died due to cerebral damage caused by cerebral anoxia.

Accidental strangulation in a cycle rickshaw.

A 50-year-old female was accidently strangulated when her chunni (a piece of cloth worn around the neck) was caught in the moving wheel of a rickshaw in which she was travelling. She was brought to the nearest hospital and died within 16 hours, remaining unconscious throughout the hospital stay. Cause of death was asphyxia as a result of constriction of the neck by ligature strangulation.

Study of head injury victims in fatal road traffic accidents in Delhi.

The study revealed 31% were the victims of Head injury injury in this part of Delhi in vehicular accidents. Although majority of the fatalities were on the spot quite a number of such victims survived for a varied period. A multipronged approach including the preventive and curative measures for this man-made calamity is the need of the hour. Amongst various preventive measure are wearing helmet be made compulsory for both the driver and pillion riders of two-wheelers, development of a safe traffic sense amongst the road users. Traffic Police should be honest, exemplary punishment for the reckless driver, decongestion of Delhi roads etc. The curative measures are facilities of CT scanners and neurosurgeons in all the major hospitals, provision of first-aid in the PCR (Police control room) vans, to institute immediate treatment to the victim without waiting for the medico-legal formalities, opening of more fully equipped Accident and trauma centres. It is high time the concerned authorities gave some serious thought towards this avoidable epidemic on Delhi roads.

Biomechanical analysis of a new external fixation system and its clinical significance.

The present study was conducted to analyze the biomechanical properties of a new external fixation system. Each frame was tested in compression, tension, and four-point bending and compared under similar conditions with the performance of the standard configurations of the Orthofix, Hoffmann, and AO models. With the new model we are able to create frames of variable rigidity, both higher and lower than that of the commercially available models. It was also possible to change the rigidity of the new frame as the fracture healed without losing the stability of the fracture. This study demonstrated that the new design, with its simplicity, versatility, and modularity, has sound biomechanical properties and these compare well with the clinical success that the model has achieved during trials.
