Choice of gastroepiploic vessels for gastrocystoplasty.

Use of vascularized gastric segments for augmentation cystoplasty and for construction of a continent urinary diversion depends on adequate blood supply via the right or left gastroepiploic vessels. Inadequate gastroepiploic vessels on 1 side may not be apparent without intraoperative testing for their patency. We recommend routine testing for the patency of the left and right gastroepiploic vessels before selecting the side to be used as a pedicle for the gastric segment.

Conservative extirpative treatment of cancer of the penis.

Carcinoma in situ, erythroplasia of Queyrat, and Bowen's disease have been managed successfully with a variety of conservative techniques. The management of invasive carcinoma remains controversial. The goal of surgical treatment in these patients is complete excision of the tumor with adequate tumor-free margins. While partial or total penile amputation is necessary for complete eradication of some primary penile tumors, many patients can be managed adequately with optimal preservation of function. Young and reliable patients with well-differentiated tumors are the most suitable candidates for conservative surgical extirpation.

Post-circumcision carcinoma of the penis: II. Surgical management.

Post-circumcision penile carcinoma is a unique clinical entity that occurs in circumcision scars on the penile shaft. Excision is the only effective treatment. Surgical excision must be tailored to the wide spectrum of disease extension. While some primary tumors can be controlled by standard penectomy, others may require much more extensive resection while some can be adequately controlled without sacrificing the urethra or the corpora. Those who had a conservative operation for less extensive disease had the best results.

Urologic manifestations of drug therapy.

Numerous drugs used for a variety of medical conditions may be involved in urologic disease or dysfunction. Manifestations include sexual dysfunction, voiding disturbances, incontinence, renal impairment, urolithiasis, obstructive uropathy, urothelial tumors, and others. The office urologist thus must be aware of the possibilities, which are listed and discussed briefly.

Epidemiology and clinical characteristics of testicular tumors in Saudi Arabia: King Faisal Specialist Hospital and Research Centre experience.

The data on 62 patients with germinal testicular tumors seen at King Faisal Specialist Hospital and Research Centre between January 1977 and June 1983 were analyzed to determine the epidemiologic and clinical characteristics of the disease in Saudi Arabia. Fifty-seven patients were Saudis. The geographic distribution of Saudi patients seemed to coincide with population concentrations. Testicular seminomas (TS) and non-seminomatous testicular tumors (NSTT) comprised 50% each. The mean age was 41 and 27.8 years for TS and NSTT, respectively. Fifteen patients had cryptorchidism of the involved testicle. Three patients with NSTT had a history of trauma to the involved testicle. The most common presentations were painless testicular swelling (51.6%), painful swelling (16%), and abdominal or inguinal swelling (21%). The delay between the onset of symptoms and referral (mean 15 months) was considerable. Eighty percent of patients with NSTT and 45% of those with TS had advanced disease at referral.

One-stage complete genital reconstruction for patients with congenital adrenal hyperplasia.

We performed 1-stage complete genital reconstruction on 20 female subjects with congenital adrenal hyperplasia. The blood and nerve supply to the glans was retained by preserving the neurovascular bundle, as well as the ventral mucosal strip. Preservation of the neurovascular bundle was simplified. The phallic skin was used to form labia minora and a flap vaginoplasty was performed simultaneously. There were no operative complications and all 20 patients had satisfactory results.

Penile reconstruction after total penectomy or urethra-sparing total penectomy.

A technique for 1-stage penile reconstruction is described in which several innovations were introduced to improve the functional results. Two flexible rod penile prostheses are wrapped in synthetic mesh, which serves as a scaffold for the growing collagen fibers to replace the tunica albuginea. The proximal portions of the prostheses are placed in the corporeal stumps to provide added stability and the mesh-wrapped prostheses are covered with a scrotal flap. In patients who previously underwent a urethra-sparing penectomy the urethra is included in the reconstructed penis to provide a meatal location at the tip of the penis without the added risks of standard urethral reconstruction.

Testicular seminomas in Saudi Arabia: clinical characteristics, prognostic indicators, and recommendations for management.

Thirty Saudi patients with pure testicular seminoma were treated at King Faisal Specialist Hospital and Research Centre in Riyadh, Saudi Arabia, between January 1977 and June 1983. Disease characteristics in Saudi Arabia including clinical findings, response to therapy, and prognosis are described and compared to those in other populations reported in the literature. Symptom durations were 3 to 42 months. Many of the patients presented with an extensive tumor burden and a poor performance status. There was a higher incidence of anaplastic seminoma and of cryptorchidism than in other series and a relatively high incidence of elevated betahuman chorionic gonadotropin (B-HCG). Patients initially underwent funiculo-orchiectomy. Twenty-two patients received radiation therapy and four received chemotherapy. Patients with limited disease responded well to orchiectomy and radiation therapy. However, those with extensive tumor burden had an unsatisfactory response to radiation therapy. Preradiation chemotherapy is recommended for patients with massive retroperitoneal metastases, nodal disease above the diaphragm, or extranodal disease and patients with minimal or moderate sized retroperitoneal nodal disease associated with an elevated B-HCG.

Post-circumcision carcinoma of the penis. I. Clinical aspects.

Fifteen patients with post-circumcision squamous cell carcinoma of the penis are discussed (13 were referred from the southwestern region of Saudi Arabia). Carcinoma developed in circumcision scars on the penile shaft. Excision was the only effective treatment and resulted in disease control in approximately 64 per cent of those treated surgically. Radiation therapy and chemotherapy did not seem to be effective in these patients.

Hernias after transpubic urethroplasty.

We report 2 cases of herniation following repair of posterior urethral strictures. Both patients underwent transpubic bulboprostatic urethral anastomosis and omentoplasty, which resulted in a perineal hernia in one and a pubic hernia in the other.

Pattern of calcium metabolism in normo- and hypercalciuric patients with calcium urolithiasis in Saudi Arabia.

Urinary calcium excretion was measured in 100 consecutive normocalcemic patients with calcium urolithiasis and 12 Saudi Arabian controls while the patients and the controls were eating their usual diet of unknown calcium content. Only 16 patients were hypercalciuric using the definition of twenty-four-hour urinary calcium of more than 300 mg for males and more than 250 mg for females, and one of these patients was subsequently found to have hyperparathyroidism. The twenty-four-hour urine calcium was less than 200 mg in 60 per cent of male patients and in all the male controls. Since the dietary intake of calcium during the twenty-four-hour urinary collection was unknown, a 1,000-mg calcium loading test was performed in an attempt to differentiate various patterns of abnormal calcium excretion. Of the twenty-four-hour normocalciuric patients 10 (18%) had "absorptive hypercalciuria" and 16 patients (29%) demonstrated a "renal hypercalciuria" pattern. Thirty-nine patients (57%) and all 12 controls had normocalciuria before and after calcium loading.

In vitro study of phenothiazine effects on urinary bladder.

Clinical observations suggest that long-term phenothiazine therapy can cause urinary retention and stress incontinence. The phenothiazines, chlorpromazine and perchlorperazine, depress bladder smooth muscle activity in vitro by depressing bladder smooth muscle activity directly and by a peripheral cholinergic blocking action.
