Lymphoepithelial carcinoma of the minor salivary gland.

Undifferentiated carcinoma of the minor salivary glands has been rarely reported in the world literature. Lymphoepithelial carcinoma, which is a variant of undifferentiated carcinoma, is distinguished from small cell and large cell undifferentiated carcinoma by its association with benign lymphoepithelial lesions. We report a case of a lymphoepithelial carcinoma developing in a minor salivary gland of the oral cavity in a 69-year-old woman. To our knowledge, this is the first reported case of a lymphoepithelial carcinoma arising from a minor salivary gland.

Pediatric tympanoplasty: the role of adenoidectomy.

A chronic dry perforation in a child presents a dilemma with regard to timing of intervention or whether intervention is appropriate at all. Many studies have looked at elements associated with eustachian tube function in hopes of finding prognostic factors. Adenoidectomy has been shown to be very effective in the treatment of chronic and recurrent otitis media. Intuitively, it would seem to play a role in pediatric tympanoplasty; however, no study has thoroughly investigated this issue to date. To evaluate the role of adenoidectomy in pediatric tympanoplasty, we performed a retrospective review of all patients younger than 18 years who had a simple dry perforation and underwent a Wullstein's type I tympanoplasty. Thirty-six patients were identified for review during the 7-year study period. The patients were grouped into those who had previous adenotonsillectomy (n = 12), those who had prior adenoidectomy alone (n = 10), and those who had neither (n = 14). Initial success of tympanoplasty was noted to be high in all three groups. However, at 6-month follow-up, the success for the group who had not had prior adenoidectomy or tonsillectomy dropped dramatically (14.3%), whereas the other two groups maintained success rates in excess of 75% (p = 0.002). This relationship remained fairly constant throughout the 2 years of follow-up. Although our population under study is somewhat small, the results support a potentially advantageous role of adenoidectomy for pediatric tympanoplasty. These results and their implications will be discussed.

Improving the physical health-mental health interface for the chronically mentally ill: could nurse case managers make a difference?

Although the increased incidence and prevalence of medical illnesses among the chronically mentally ill has been well documented, access and use of general health care services by this population remains problematic despite the implementation of case management. This article describes the problems experienced in meeting physical health care needs through the five core case-management components of client identification and outreach, assessment, service planning, monitoring of service delivery, and advocacy. A model of case management is proposed that uses psychiatric nurse clinical specialists in supervisory and consultative rôles, and nurse practitioners to deliver primary care.

Independent living for the chronically mentally ill.

1. Many of the systems problems faced by the chronically mentally ill and their caregivers could be ameliorated by applying the principles of primary nursing. 2. Psychiatric nurses as primary caregivers of the chronically mentally ill can intervene at the client level (assessment, education, medication monitoring, family counseling) and at the systems level (networking, boundary-spanning, advocacy). 3. Psychiatric nurses are a natural and largely untapped resource for improving the care of the chronically mentally ill in the community.

Psychiatric nursing in a rural outreach program.

TOPIC: A method of delivering mental health services to clients in rural areas PURPOSE: To describe the opportunities available for nurses to provide mental health services in rural areas SOURCE: Preliminary outcome data from program statistics and case studies CONCLUSION: Nurses were able to significantly decrease client hospitalizations and increase longevity in the community.

Linkages between community mental health centers and public mental hospitals.

Directors of community mental health centers and superintendents of public mental health hospitals in one state were surveyed to gather data on interagency linkages. Implementation of affiliation agreements, exchange of staff training, and exchange of patient information were investigated. Affiliation agreements tended to be implemented with little difficulty and there was more interagency cooperation than that reported in earlier research. However, exchange of training and staff were still areas of minimal interaction. Geographic proximity was found to have a positive influence and competition a negative influence on cooperation. Further attempts at interagency linkages in the interest of continuity of patient care are recommended.
