Juvenile ECG pattern in adult black Arabs.

A juvenile electrocardiographic (ECG) pattern is compatible with T wave inversion in the precordial leads V1 and V2 or in additional precordial leads to the left of V2. Electrocardiographic studies were performed in 48 Semitic Arabs, including 26 men and 22 women, and in 50 black Arabs (negroid Bedouins), including 22 men and 28 women. The subjects ranged in age from 20 to 46 years. The juvenile ECG pattern was found in 6 of the 28 black Arab women (21%), but it was not detected in the 22 black Arab men or in any Semitic Arabs of either sex. The difference between the two female groups was significant (P < .05). Careful interpretation of ECG recordings in black Arab women is recommended in order to prevent a misdiagnosis of myocardial ischemia or a misreading of T wave inversions related to a previous coronary event.

[Sonographic imaging of Ascaris lumbricoides].

The round worm, Ascaris lumbricoides, is one of the largest of the parasites that infest the human bowel. The worms usually develop in the jejunum and can reach several thousand in number, causing bowel obstruction, volvulus, intussusception, appendicitis and even bowel perforation with penetration into the peritoneal cavity. They tend to invade the bile and pancreatic ducts and may cause acute cholecystitis and pancreatitis. Ascaris lumbricoides can be detected by sonography. This imaging modality can be helpful in diagnosing the presence of the worms and in evaluating response to treatment. We present an 18-month-old girl in whom bowel worms were detected by sonography.

Ritual female genital surgery among Bedouin in Israel.

Ritual female genital operations are common in many parts of the world, with varying degrees of mutilation from clitoridectomy and removal of the labia to removal of the clitoral prepuce. Interviews of 21 Bedouin women in southern Israel revealed the practice to be normative in several tribes. However, physical examination of 37 young women from those tribes at a gynecological clinic revealed only small scars on the labia in each woman. Bedouin in southern Israel may offer a model of evolution of female circumcision into a nonmutilative ritual incision.

Marked increase in plasma high-density-lipoprotein cholesterol after prolonged fasting during Ramadan.

We evaluated the effect of the Ramadan fasting on plasma lipids and lipoproteins in normal individuals. Twenty-four healthy subjects were studied before the end of the Ramadan month (Ram) and for 1 mo thereafter. Plasma total cholesterol (TC), triglycerides, low-density-lipoprotein cholesterol (LDL-C), and very-low-density-lipoprotein cholesterol (VLDL-C) did not change. High-density-lipoprotein cholesterol (HDL-C) was 30% higher (P < 0.005) at the end of Ram; apolipoprotein A-I also increased (P < 0.0001). Both the ratios of TC to HDL-C and LDL-C to HDL-C (P < 0.001) decreased at Ram. There was a striking nonpharmacologic improvement in plasma HDL-C and ratios of TC to HDL-C and LDL-C to HDL-C, which were most probably induced by eating one large evening meal a day. Further studies to determine the mechanism of this observation are underway.
