"Warning card" as a prevention of prolonged apnea in children.

Human butyrylcholinesterase (EC 3.1.1.8) (BChE) is present in serum mainly as "usual UU" form, but it has also been found in variant forms known as "atypical" BChE. The most important predictive value of BChE phenotype is for anesthetist to prevent prolonged apnea. BChE has an important role in the hydrolysis of neuromuscular relaxant succinylcholine (suxamethonium, scoline) used during anesthesia. In order to detect atypical variants of BChE and give these findings to the anesthetists-surgeons before an operation to avoid the prolonged apnea, we phenotyped 542 sera of children before tonsillectomies. Total BChE activity was measured using butyrylthiocholine as substrate and dibucaine, fluoride, urea and dimethylcarbamate Ro 02-0683 were used as inhibitors. The frequencies of phenotypes in 542 children were: UU, UA, US, SS, AS and AA--92.25%, 7.01%, 0.18%, 0.18%, 0.18%, 0.18% respectively. Once established phenotype of BChE does not change during the lifetime. Therefore the carriers of atypical phenotype of BChE received a "Warning card", which is a permanent warning for succinylcholine application, as well as a sign to the members of the families to test their own phenotype of BChE. In our study three "Warning cards" were given: two to the carriers of atypical phenotype and third to a child presented as SS phenotype with low total activity of BChE. The present study is the first clinical evaluation of this genetic abnormality in the Republic of Croatia.

Polymethyl-methacrylate implants in forehead and supraorbital arches reconstruction: retrospective study.

Frontobasal injuries and some other diseases of frontal sinuses sometimes require radical surgery with the obliteration of the sinus. A cosmetic correction of the forehead and supraorbital arches has to be performed after such a procedure. Nowadays, there is a wide choice of alloplastic materials on the market. We tried polymethyl methacrylate implants in correction of the exterior appearance of the forehead and supraorbital arches and followed the behaviour of the implants up to 25 years after surgery. The result was 18 (94.7%) successful implantations and one failure (5.3%) with a mild sagging of the borders of the implant. Polymethyl-methacrylate proved pliable for work and modelling, not invasive for the organism and stable. In conclusion, the cure of some frontobasal injuries and diseases with the first surgical step followed by cosmetic correction of the forehead and supraorbital arches (2nd surgical step) using this alloplastic material provides a safe, durable and aesthetically satisfactory solution for the patient.

Some morphological, histological, cytological and histochemical aspects of Reinke's oedema.

Using morphological criteria we divided Reinke's oedema into a pale (transparent) and a livid type. The latter is more difficult to treat because of its prolonged postoperative recovery. Ten samples of pale Reinke's oedema and 10 of livid oedema were selected for cytological, histological and histochemical analysis in order to determine some of their fundamental characteristics and differences. Tissue samples were stained by hemalaun-eosin, Mallory-azan and Paf-Halmi method (specific for visualization of elastic versus collagen fibres). Cytology (May-Grünwald Giemsa) of cellular elements was undertaken by imprint method of the excised oedematous tissue. Also, the silver-staining nuclear organizer regions (Ag NOR) were identified and quantified. We identified intracytoplasmic granula and silver-staining nuclear organizing regions (Ag NOR) in stromal cells of pale type Reinke's oedema. Thus, it may be supposed that smoke irritation provokes higher activity of stromal cells producing a special substance in Reinke's space-resulting finally in Reinke's oedema.

[Benign paroxysmal vertigo in children].

Based on our examination of 176 patients, we first selected, according to the general standards of diagnosing benign paroxysmal vertigo of childhood (children aged up to ten years, paroxysmal vertigo, ataxia) a group of 78 patients. Using multidisciplinary approach we have thus performed in these children the following examinations: an exhaustive anamnesis, laboratory examinations, clinical ORL (ENT), ophthalmological, neuropediatric examinations, x-ray of the cranium and the temporal bone, audiometric, vestibulogenic and EEG examinations. In the following stage we have, on the basis of the obtained data, selected 22 patients who met the restrictive standards for the diagnosis of a benign paroxysmal vertigo of childhood (typical clinical picture, isolated vestibular symptomatology, exclusion of the etiological factors, benign course of the disease). In our material the age range was between the second and the sixth year. After the sixth year of age we have not had any case of benign paroxysmal vertigo. We have not arrived at any significant fact which would indicate an etiological factor. There is no significant difference in the frequency of the disease between the two sexes, but we may assert that small girls slightly prevail. Normal psychomotoric development, normal intellectual faculties, absence of risk factors (only a twin had lower birth weight), absence of neurological abnormalities, regular EEG findings, pathological findings of the vestibular excitability together with absence of cochlear symptomatology, and evident clinical symptoms obviously indicate the right diagnosis. The evolution of the disease is exceptionally favorable.(ABSTRACT TRUNCATED AT 250 WORDS)

Technical note: simple vibroacoustic foetal stimulator.

PRIMARY OBJECTIVE: To build and test a simple vibroacoustic foetal stimulator. RESEARCH DESIGN: A vibroacoustic foetal stimulator was constructed and subjected to bench testing. Clinical trial was carried out in comparison to an electro laynx. MAIN OUTCOME AND RESULTS: Screening specificity of 98% and 100% sensitivity was achieved, with no decrease in specificity due to user's maladjustments. The device was characterized by simple use, maintenance and disinfection. CONCLUSIONS: The device is a good screening instrument for foetal health in prenatal care, despite its simplicity and low price.
