[Study on the surgical treatment for pulmonary metastasis from renal cell carcinoma].

BACKGROUND: Pulmonary resection for metastatic renal cell carcinoma were studied to assess the indication of surgical management. METHODS: Between January, 1981 and December 1994, 17 consecutive patients (14 men and 3 women) underwent complete pulmonary resection for metastatic renal carcinoma. Median age was 61 years (range, 45 to 73 years). Eleven were appeared lung metastasis after resection of primary tumor. Median time between nephrectomy and pulmonary resection was 32 months (range, 0 to 127 months). RESULTS: There were no operative deaths. One patient had solitary metastasis, 4 had two, 2 had three, 2 had four, one had seven, one had eight and 6 had more than twenty-two. Segmental resection was performed in 12 patients, lobectomy in 2, lobectomy and segmentectomy in 3 and segmentectomy for total lobes in 3. Four patients had another site operation of renal metastasis, brain tumor resection, chest wall and ribs resection, contra-lateral adrenalectomy and contralateral partial nephrectomy. Median follow-up was 44 months (range, 10 to 129 months). The cause specific survival rate and disease free survival after pulmonary resection was 55 and 48 percent at 5 years and 27 and 14 percent at 10 years, respectively. CONCLUSION: Pulmonary resection for metastatic renal cell carcinoma was considered effective in some selected slow-growing cases. Multiple and both lungs metastases is not contraindication and the patients under 10 metastatic focuses had good prognosis.

[Ureteritis cystica--report of a case diagnosed by biopsy under ureteroscopy].

A 62-year-old male with left ureteritis cystica was reported. Extracorporeal shock wave lithotripsy (ESWL) was performed for his left renal stone 14 and 17 months earlier. However, small fragments remained in the lower calyx and pyuria, microhematuria and bacteriuria continued. Drip infusion pyelography (DIP) showed a small filling defect of the left dilated ureter. Retrograde pyelography revealed multiple, small filling defects of the left middle through the lower ureter. Multiple small submucosal cysts were observed mostly in the pelvic ureter by ureteroscopic examination. Ureteroscopic cold punch biopsy proved ureteritis cystica. The ureteral dilatation improved and filling defects disappeared after the treatment with antibiotics.

Role of urinary alpha1-globulin fraction in formation of urinary calculi. With special reference to aggregating effect of serum alpha1-globulin on calcium carbonate suspension.

The present study was undertaken to determine if urinary protein can produce urinary stones and what types of protein have such an effect. Normal human serum was fractionated first with ammonium sulfate and then with DEAE-cellulose. The stone-forming action of each fraction was estimated by calculating their ability to alter the sedimentation rate and zeta-potential of a calcium carbonate suspension. Finally, the proteins in each fraction were identified by cellulose acetate electrophoresis. The most marked aggregating and precipitating effect on a CaCO3 suspension was observed with a mixture of proteins of alpha1-globulin and albumin fractions. However, beta- and gamma-fractions had little aggregating effect on the CaCO3 suspension.

The role of the matrix substance in formation of urinary stones.

This study deals with the role of the stone matrix substance in formation of urinary stones. A series of coagulation experiments were conducted by determining the sedimentation rate of calcium carbonate suspension with respect to the following substances: sodium alginate, stone matrix substance, bovine albumin and chondroitin sulfate C, these were each added to a separate fine particle suspension of calcium carbonate. Changes in zeta-potential of the suspension particles after addition of the above materials were determined microscopically using Usui's method. The results obtained in this study indicate that sodium alginate, the matrix substance, bovine albumin and chondroitin sulfate C act on the suspension particles, and as a result, coagulation is produced by a decrease of the zeta-potential of the particles. Application of kinetic energy to the coagulate caused by sodium alginate resulted in the formation of a solid. From these results it is considered that the matrix substance is not incorporated by accident in urinary stones, but it participates in the formation of them.

On the transportation of spermatozoa in the vas deferens.

In this report the intraluminal pressure of the vas deferens was measured in vivo in male cross-bred dogs. The measurement of the pressure elucidated that the vas deferens repeated autonomic contraction. This contraction was not the so-called peristalsis but the entire portion of the vas deferens contracted simultaneously. Stimulation of the hypogastric nerves resulted in a prominent elevation of the pressure in the proximal portion of the vas deferens, i.e., the portion approximate to the epididymis, followed 5-10 seconds later by a rise in the distal portion, i.e., the portion approximate to the prostate. These results suggest that the spermatozoa present near the proximal end of the vas deferens are transported littly by little by the autonomic movements of the vas deferens and that on ejaculation the spermatozoa are pressed forward from the proximal end of the vas deferens to its peripheral end by the stimuli through the hypogastric nerves.

Peripheral nerves controlling the closure of internal urethral orifice during ejaculation.

The peripheral nervous pathways controlling seminal emission and closure of the internal urethral orifice during ejaculation were examined experimentally using male adolescent dogs. The experimental methods were measurement of the contraction of the internal urethral orifice and measurement of the change of the posterior urethral pressure induced by selective stimulation of the nerves related to the hypogastric nerve. Further, the change of actual ejaculation caused by manual stimulation of the penis was observed after excision of the nerves. Results included the following: (1) The seminal emission and the closure of the internal urethral orifice on ejaculation were controlled by the hypogastric nerve. (2) The nerve fibers which controlled these two phenomena took different paths above the lower mesenteric plexus. (3) Seminal emission was controlled by the splanchnic nerves which originated from the lower thoracic and upper lumbar ganglia of the sympathetic trunk, and the closure of the internal urethral orifice by those which originated from the lower lumbar ganglia of the trunk.

Role of alpha-adrenergic receptor mechanism in closure of the internal urethral orifice during ejaculation.

The effects of autonomic drugs on closure of the internal urethral orifice during ejaculation were investigated in dogs. The closure of the internal urethral orifice was induced by the electric stimulation of the lateral fibers of the lower mesenteric plexus. The closing state of the orifice was measured with a latex balloon inserted into the orifice. The autonomic drugs were given into the aorta just above its bifurcation. From the results obtained in this study it was concluded that the closure of the internal urethral orifice during ejaculation is predominantly under the influence of the adrenergic nervous system, particularly through an alpha-adrenergic receptor mechanism.

On the brain monoaminergic systems relatint to ejaculation. II. Brain serotonin and ejaculation.

The effects of 5-HTP and pCPA on manually induced ejaculation in dogs and on brain monoamines in dogs and rats were investigated. From the results obtained in this study it was concluded that the serotonergic system in the brain is the mechanism which inhibits the occurrence of ejaculation. Considered together with the results obtained in our previous paper, it is though that ejaculation occurs with the reconciliation of the dopaminergic and serotonergic systems.
