[The contribution of endoscopy in the diagnosis of esophago-gastro-duodenal disorders in a tropical milieu. Experience in Benin with 930 examinations].

The authors analyse the results of esophago-gastro-duodenal fibroscopy in 930 symptomatic patients. Ninety one per cent of them had lesions. Inflammatory pathology was predominant: esophagitis, gastritis and duodenitis were seen in 21.5%, 47% and 29.08% respectively of the patients investigated. Gastritis accompanied 75.13% of cases of esophagitis and 76.4% of duodenitis, and was associated with the demonstration of the presence of Helicobater pylori in gastric biopsies in 56.41% of patients with that lesion. The relatively high incidence of carcinoma of the esophagus (2.7%) is a particular feature of this study, while that of carcinoma of the stomach (1%) was in accordance with classical data. Duodenal ulcer was found in 18% of patients as compared with 5.16% for gastric ulcer. From a pathophysiological standpoint, mention is made of traditional diet (hot, highly spiced), self-medication and intestinal parasite infestation in causing inflammatory lesions. Finally, emphasis is placed upon the role of Helicobacter infection in the development of chronic gastritis. The high rate of infection with this organism and its involvement in the mechanisms of duodenal ulcerogenesis could explain the high incidence of duodenal ulcers in our group and in studies emanating from developing countries.

[Digestive manifestations of malaria in adults in an endemic zone].

The retrospective analysis, on a twelve months' period, of 146 files of patients suffering from malaria at the National University Hospital in Cotonou helps conclude what follows: the gastro-intestinal manifestations are frequent and represent 41% of adult malaria cases. Most of the time, they are characterized by vomiting (32.9%), followed by abdominal pains (20.5%). Diarrhoea is less frequent and represents only 6.8% of the cases. Those signs may be unusually acute and be mistaken for a possible surgery emergency or have serious consequences on the general health of the patients. They can, for instance, involve shock. The anoxaemia of tissues involved by microcirculation troubles is the main factor of the pathogenesis of those manifestations. A specific cure only can entail the disappearance of those signs and restore normal blood circulation conditions.

[Malabsorption in alcoholic cirrhosis].

21 patients with alcoholic cirrhosis were worked up for malabsorption. In three patients, the fecal weight was over 200 g/24 h; three had a steatorrhea over 6 g/24 h and in four the creatorrhea was over 2 g/24 h. The D-xylose test was abnormal 4 times out of 18, but these 4 patients presented an ascites. Alpha-1-antitrypsin clearance was increased in 1 out of 9 patients. The Lundh test demonstrated in 5 out of 8 cases an external pancreatic insufficiency, but without any relation with the fecal losses. The 4 patients with malabsorption showed signs of malnutrition (anthropometric criteria). In the course of an alcoholic cirrhosis, malabsorption seems therefore infrequent, dissociated, and only observed in patients with signs of malnutrition.

[Diabetic nephropathy: an epidemiological study based on proteinuria in a population of black African diabetics in Cotonou, Benin].

We evaluated proteinuria to determine the frequency of diabetic nephropathy and to study epidemiological aspects of this disease. We measured 24-hour urinary protein excretion in 152 diabetic patients. We recorded the age and sex of each patient, the duration of diabetes and blood glucose concentration. Proteinuria was diagnosed in 28% of the diabetic patients. The frequency of proteinuria was higher in men (33%) than in women (19%). The highest frequency (37.5%) was observed in subjects aged 70 years or over. In these patients, the higher frequency of proteinuria was associated with a longer duration of diabetes. However, proteinuria was detected in 28% of patients with diabetes diagnosed less than one year previously, suggesting a long period of undiagnosed diabetes in these subjects. Finally, proteinuria was more frequent in patients treated with insulin (42%) than in those treated orally (25%). Thus, diabetic nephropathy is a frequent complication in black diabetic patients in Cotonou. As hemodialysis and kidney transplantation are very expensive and access to these treatments is limited in developing countries, preventive measures based on optimizing patient management and the early diagnosis of diabetes and its complications are required.

[Duodenal tuberculosis: a case report].

The authors relate the case of a duodenal tuberculosis shown by a pyloro-duodenal stenosis. They emphasize the absence of specific clinical, radiological endoscopic and histological manifestations. Laparotomy is in most cases necessary. The diagnosis is therefore affirmed out of the findings of peritoneal granulations or the extemporaneous anatomo-pathological investigation of lymphatic nodules.

[Ultrasonography and amebic abscess of the liver at the CNHU at Cotonou: apropos of 33 cases].

From the 1st of March, 1991 to the 31st of December, 1995, 577 patients had abdominal echographic examinations which enabled the diagnosis of 33 cases of liver amebic abscesses. This study evaluated to what extent echography can facilitate the diagnosis and surveillance of patients during treatment in a region where immunological diagnosis of amebiasis is not yet performed and where percutaneous puncture is just beginning. Patients were suffering from chronic hepatic pain and fever in 28 cases. Seventeen patients had hepatomegaly and 14 patients had lost weight. Echographic images changed with time. In the beginning, there were diverse blurred structures which were 4 to 6 cm in diameter. Subsequently, the content of the abscesses became more homogeneous and in 3 cases the posterior area intensified. Also, the evolution of the disease during treatment by metronidazole could be followed by progressive echographic imaging. All the patients recovered without after-effects and only four of them showed hyperechogenic cicatrization. Two patients had an evacuation of their abscess which resulted in distinct improvement. The authors recommend generalization of that technique in all the hospitals across the country. Echography can thus improve both the diagnosis of liver amebic abscesses and the surveillance during the treatment.
