Role of radionuclide perfusion study in cold solitary thyroid nodule for diagnosis of malignancy: a complimentary diagnostic modality to fine needle aspiration cytology.

One hundred and forty eight subjects with euthyroid solitary thyroid nodules (STN) were taken up for radionuclide perfusion study. They were found to have a cold STN on 99mTc thyroid static scan. All had fine needle aspiration cytology (FNAC), and except for subjects with chronic lymphocytic thyroiditis, were subjected to surgery for tissue diagnosis by histopathology. The diagnostic findings in these patients of solitary thyroid nodules were correlated with the histopathology. Radionuclide perfusion study is considered useful to differentiate benign from malignant cold thyroid solitary nodules with high degree of sensitivity (95%) and specificity (87.9%).

Renal imaging with 99Tc(m)-dextran.

Significant uptake and retention of 99Tc(m)-dextran (molecular weight: 81,000) in renal parenchyma was discovered during evaluation of its intravascular use. Renal SPET images confirmed this. This study was designed to evaluate 99Tc(m)-dextran as a renal cortex imaging agent. Stability of parenchymal retention was shown by insignificant outflow at 24 h and by frusemide intervention. Evaluation of the renal parameters of intravenous 99Tc(m)-dextran (n = 71 normal kidneys) and its comparison with 99Tc(m)-DTPA n = 10) and 99Tc(m)-DMSA(III) (n = 23) was undertaken. The early glomerular extraction phase of the renograms of 99Tc(m)-DTPA and 99Tc(m)-dextran appeared identical; parenchymal uptake of 99Tc(m)-dextran continued to increase and reached a near-plateau by 40-60 min. The mean cortex-to-background and cortex-to-liver ratios at 2 h with 99Tc(m)-dextran and 99Tc(m)-DMSA(III) were 14.9 and 9.2, and 16.0 and 8.9, respectively. The target-to-nontarget ratios were similar despite different absolute renal uptake values (12 vs 20% at 2 h) because of faster background clearance of 99Tc(m)-dextran. The mechanism of parenchymal retention of 99Tc(m)-dextran appears to be trapping at the endothelial-epithelial interphase of the glomerulus. Our initial experience suggests 99Tc(m)-dextran is a viable renal parenchyma imaging agent.

Differentiation of malignant and degenerative bone lesions using dexamethasone interventional 3- and 24-hour bone scintigraphy.

Seventy-seven adult patients with suspected skeletal metastases were divided into two groups. In group A (n = 30), following intravenous administration of 20 mCi (740 MBq) of technetium-99m methylene diphosphonate (99mTc-MDP), 3- and 24-h scintigraphy of bone lesions was performed. The 24/3 h lesion to bone background radiouptake ratio (RUR) was calculated for each lesion. In group B (n = 47), the same procedure was followed with dexamethasone intervention (10 mg in 24 h) following the 3-h acquisition. In group A, after determination of the critical point, malignant and degenerative bone lesions could be separated with a sensitivity, specificity and accuracy of 0.76, 0.72 and 0.73, respectively. The mean RUR of the malignant lesions was 1.20 +/- 0.23, and that of the benign lesions, 0.95 +/- 0.15. In group B cases, significantly increased sensitivity, specificity and accuracy of 0.87, 0.94 and 0.92, respectively, were found (P < 0.001). The mean RUR of the malignant lesions was 1.48 +/- 0.34, and that of degenerative lesions, 0.88 +/- 0.19. Dexamethasone interventional bone scintigraphy seems to be a new cost-effective method for differentiating malignant from degenerative bone lesions using the RUR.

Tc-99m(V) DMSA imaging. A new approach to studying metastases from breast carcinoma.

Combined Tc-99m MDP skeletal imaging and Tc-99m(V) DMSA whole body scans to detect metastases were performed during the follow-up of 30 patients who underwent surgery for breast carcinoma. Eight patients had normal Tc-99m MDP and Tc-99m(V) DMSA scans and were declared free of metastatic disease, further confirmed by no change in symptomatology over a 1-year follow-up period. Twenty-two patients had positive Tc-99m MDP scans with varied skeletal involvement. Tc-99m(V) DMSA scans showed matched areas of increased radiotracer concentration in bony metastases in 20 of these patients. Tc-99m(V) DMSA concentration was not seen in traumatic vertebral collapse or in coexistent osteoarthritic disease in vertebral metastatic involvement. Interestingly, Tc-99m(V) DMSA showed increased concentration in brain and liver metastases. Pentavalent Tc-99m(V) DMSA appears useful for detecting skeletal and soft-tissue metastases in breast carcinoma, and can improve the specificity of Tc-99m MDP bone scans in screening for bone metastases.

Evaluation of functional parameters and left ventricular size by radionuclide ventriculography (RNV) in myocardial infarction patients.

Functional parameters like left ventricular ejection fraction (LVEF), peak ejection rate (PER), peak filling rate (PFR), regional wall motion and left ventricular size index were evaluated in 68 patients with myocardial infarction and in 50 normal subjects, by radionuclide ventriculography (RNV). Patients were divided into four groups (I to IV) according to increasing left ventricular size and the parameters were evaluated in each group. LVEF proved to be the most sensitive parameter of overall left ventricular performance. PFR decreased significantly from group I to group III but not from group III to group IV, suggesting that in extreme degrees of left ventricular enlargement some compensatory mechanism acts to prevent a greater fall in left ventricular compliance. LV size and performance were least affected in inferior wall myocardial infarction. LVEF was however a better predictor of LV size than the site of the myocardial infarction.

Non-invasive evaluation of global ventricular functions in coronary artery disease by exercise radionuclide angiocardiography.

Rest and exercise radionuclide angiocardiographic measurement of left ventricular functions were obtained in 12 normal subjects (group I) and 42 patients with well documented coronary artery disease (group II). Additionally thirty-five patients had an exercise tread mill test. Resting left ventricular ejection fraction less than 50% provided the greatest diagnostic information. Exercise was induced by supine bicycle exercise ergometer. In group II 32 patients of coronary artery disease with normal global ventricular function at rest, new regions of dysfunction developed during exercise and global ejection fraction fell by 8 to 15 percent while in group I there was an increase in LVEF and no areas of any ventricular wall motion abnormalities were detected. Exercise radionuclide angiocardiography is a useful non invasive technique which permits accurate assessment of the presence and functional severity of coronary artery disease.

Cervical mass lesion presenting as reflex sympathetic dystrophy of the hand.

A case with multiple small joint swelling and skin dystrophic changes in the right hand was initially treated as seronegative non-specific arthritis. Later, a triple phase bone scan indicated reflex sympathetic dystrophy (RSD) and the primary lesion was shown on MRI scan to be a cervical cord mass with associated sub-acute haematoma. The case underscores the need to consider RSD in the differential diagnosis of such conditions and the importance of a detailed neurological examination in RSD with no local causative factor.
