The 3' end termini of the Tacaribe arenavirus subgenomic RNAs.

Tacaribe virus (TV), a member of the Arenaviridae family, contains two single-stranded RNA genome segments called S and L. Two proteins, in an ambisense coding strategy, are encoded in both the S RNA and the L RNA. The 3' ends of the TV four putative mRNAs have been characterized using S1 nuclease mapping. The experiments revealed that the transcripts terminate within the intergenic region in each RNA segment. No special sequences that might function as termination signals were evident. The 3' end sequences of the four putative mRNAs can be predicted to adopt GC-rich stable hairpin configurations (delta G greater than or equal to -25 kcal). These observations suggest that the transcript structure rather than particular sequences might be the signal involved in the termination of arenavirus transcription.

The 5' region of Tacaribe virus L RNA encodes a protein with a potential metal binding domain.

We have just completed the Tacaribe arenavirus (TV) genome structure by sequencing the 5' region of the L RNA. Analysis of the sequence has indicated the existence of an open reading frame (ORF) in the viral sense RNA encoding a 95 amino acid polypeptide. The first in phase AUG codon is in positions 70-72 from the 5' end of the viral RNA surrounded by a sequence favorable for the initiation of protein synthesis. The ORF ends at positions 355-357. The predicted polypeptide (P11) contains a cysteine-rich sequence bearing a remarkable similarity to the "zinc finger" sequences found in a number of proteins. We have recently reported that the 3' region of the TV L RNA encodes a polypeptide comprising 2210 amino acids in the viral-complementary sequence. This latter gene, i.e., the L gene, terminates at positions 442-440 from the 5' end of the viral RNA. The two genes encoded by the L RNA (L and P11) are in opposite strands of the RNA in sequences that do not overlap, but are separated by a noncoding intergenic region of 82 nucleotides. The nucleotide sequence of the intergenic region leads to the prediction of a strong secondary structure.

Marrow aplasia during high dose mebendazole treatment.

A patient with chronic liver disease was treated with large doses of mebendazole for a hepatic hydatid cyst. Eighteen days after beginning treatment he developed marrow aplasia which reverted to normal after the drug was stopped. This is the marrow aplasia which reverted to normal after the drug was stopped. This is the sixth patient described as developing marrow aplasia when treated with large doses of mebendazole. We suggest that the aplasia is related to the dose of the drug, and that the patient's chronic liver disease was an important factor in its genesis. Patients treated with large doses of mebendazole should have their blood counts monitored during treatment.

Immunoglobulin A level in human milk from mothers delivering preterm.

Little information is available on immunoglobulin content of milk from mothers of preterm babies. In this study we have measured IgA levels in samples from preterm and term mothers during the 1st month postpartum. The concentration of IgA tended to be constant in the preterm group (about 251 mg/dl) while it showed a decreasing trend in the term group (from 212.8 +/- 19.9 to 171.6 +/- 12.8 mg/dl). From day 10 onward IgA levels in preterm milk were significantly higher than in term milk (p less than 0.002). These findings support the idea that premature infants fed their own mothers' milk would benefit from more stable levels of IgA, at a time when their mucosal immune system is poorly developed.

Allergy to local anaesthetics in dentistry. Myth or reality?

BACKGROUND: Local anesthetics (LA) are frequently used in dentistry. Although these drugs are usually well-tolerated, they can sometimes provoke adverse reactions of various types and severity. The true incidence of LA allergic reactions is unknown. The objectives of this study were (i) to evaluate the incidence of immediate adverse events in subjects requiring local anesthetic injection in order to receive dental treatment; (ii) to assess the incidence of anaphylactic allergic reactions among those recorded as adverse events and (iii) to analyze the relationship between the atopic antecedents of these patients and documented allergic reactions. MATERIALS AND METHODS: A prospective, open-label, non-comparative study including 5,018 subjects who received LA during dental treatment, despite their age, was carried out in 7 private or public odontological centers. All the possible reactions that could appear during the first hour of anesthetic administration were assessed. RESULTS: Twenty-five adverse reactions were diagnosed, representing 0.5 % of the study population. None of these reactions was due to an allergic cause. Most (22/25) were mild, quickly reversible psychogenic or vasovagal reactions. One case was related to defects in the anesthetic technique. In two further cases, allergic etiology was ruled out after skin and dose provocative challenge tests with the anesthetic. In conclusion, allergic reactions to LA are very rare. Most adverse reactions are psychogenic or vasovagal. Physicians and dentists should be aware of these facts in order to minimize the frequent fears and myths concerning the use of LA in the dentist's office.

Genetic and environmental influences on atopic immune response in early life.

The purpose of our study was to carry out a prospective follow-up of 114 newborns at term (including three pairs of twins), regarding clinical manifestations for atopy during the first year of life. Their IgE levels in cord blood samples, at 3, 6, 9 and 12 months of age were measured and the influence of race, sex, breast-feeding, maternal smoking, family income, month of birth, family history and personal manifestations of atopic disease were evaluated. Total serum immunoglobulin E was quantified by microparticle enzyme immuno-assay (MEIA). The study group consisted of 60 (53%) male neonates, 67 (59%) Caucasians and 47 (41%) blacks. In the clinical follow-up, 32 (28.1%) infants developed obvious atopic disease: 29 infants presented recurrent wheezing, two had cow's milk allergy and one had atopic dermatitis. Probable atopic disease developed in 12 (10.5%) infants, whereas 70 (61.4%) infants showed no manifestations. Cord blood IgE levels in infants with obvious atopic disease was higher when compared to those without (p = 0.024), with 70.97% sensitivity and 46.2% specificity. IgE levels were also significantly different up to 12 months in these groups (p = 0.0001), when the sensitivity was 82.1% and the specificity 54.1%. At this age, the IgE levels were higher in infants with obvious atopy than nonatopic disease in relation to male sex (p = 0.015), black race (p = 0.009), breast-feeding for less than 6 months (p = 0.011) and when the family income was less than three times the minimum wage (about US $300) (p = 0.006). There was no association between IgE levels and family history of atopy. We concluded that immune response for atopy was in a large degree influenced by environmental factors and serum IgE at 12 months was a good marker for identifying infants with risk of atopic disease in early life.

[A model for evaluating internal accuracy].

OBJECTIVE: To provide an evaluation model of internal accuracy illustrating its use in a quality control program of clinical chemistry. METHODS: The model uses data transformed to percentage of assigned value (%AV) that allows the pooling of different controls. Precision is evaluated by the mean of the intracontrol coefficients of variation, and accuracy by the mean of means of the %VAs and its standard deviation. The model was used in 17,280 measurements of 23 analytes assayed in seven pairs of controls (medium and high concentration) during four years in two automated clinical chemistry analyzers. RESULTS: The model established inaccuracy of 5% to 25% in seven analytes, five of them enzymes. It also detected interanalyzer differences in accuracy of five analytes (22%) and an erroneous assigned value in 19 of 304 controls (6%). CONCLUSIONS: The model showed its capacity to dissect the sources of variation of accuracy. There was inaccuracy in 30% of the analytes but further studies are needed to validate these inaccuracies with controls of other sources. The model can be useful for control programs of any quantitative assay system. A summary description of the model's operation is given.

[Oral hairy leukoplakia. Early sign of AIDS].

Oral hairy leukoplakia (OHL) is a new lesion that appears specifically on the lateral borders of the tongue. It has basically been detected in immunosuppressed male homosexuals and it is associated with the later development of the acquired immunodeficiency syndrome (AIDS) or HIV infection. Epstein-Barr virus has been suggested to be the possible etiological agent. A review of the existing literature pointing out definition, clinical, histopathologic and ultrastructural features, prognostic and treatment is made. Moreover the characteristics of the individuals suffering from it are equally studied. The role of the dentist at detecting the lesion, as well as the importance of its definitive diagnostic are emphasized, in virtue of the effects that may have a bearing on patients.

[Obtention of Trypanosoma cruzi clones of thin and thick epimastigotes and trypomastigotes living in the blood on various agar media].

Trypanosoma cruzi (EP strain) presents clonal growth when it is incorporated into at 0.7% agar and at 0.8% in surface. The colonies were obtained from a single parasite and in a relatively short time. The efficiency of plating was variable and depended on the type of agar medium. A maximal efficiency was obtained for at 0.7 agar/LIT-BHI and for at 0.8% agar/LIT-BHI-Blood. The sanguicolous tripomastigotes, either thin or thick, were able to generate colonies from single parasites, with 100% plating efficiency, higher than 80% corresponding to the epimastigotes coming from LIT medium.
