[Frequency of falls in german hospitals].

PURPOSE: So far, there are no statistics on falls by patients in German hospitals. This study records data on the frequency and results of inpatient falls and compares patients who suffered from falls, with the other patients. METHODS: In a prevalence study, we collected data from 3012 patients in 11 hospitals in Berlin and surrounding areas. RESULTS: 3.5 % of all patients fell during their clinical stay. 13 % of the patients who fell suffered a fracture or a joint injury, 20 % another injury while 67 % had no any injuries. The patients who fell were older and more care-dependent than the other patients. Most of the patients fell on geriatric wards. CONCLUSION: Older patients fall more frequently than other patients while in hospital.

[Prophylaxis and therapy with human immunoglobulins].

The supply of human immunoglobulin preparations (Ig) is steadily increasing. On the one hand, the wide range of specificities of the so-called hyper-Ig is broadening, on the other hand, the structural variety of Ig contained in the preparations and their fragments is increasing (intact IgG, Fab, F(ab')2 fragments, desaggregation, inactivation of the complement binding site). Specificity and structure of Ig are important for the efficacy, kind of application, amount and intervals of dosage and for compatibility. Therefore, an exact knowledge of the preparation becomes more and more necessary for reasonably applying it. The development of modern IgG preparations with intravenous compatibility and intact Fc-part providing the possibility for the application of a high dosage require "old" indications to be re-examined by clinically controlled investigations, thus opening new ways to an improved efficacy in applying Ig in the prophylaxis and therapy of infections as well as to new areas of indication by recognising the immunomodulatory effects, e.g. autoimmunopathies.

[A further case of Tn-polyagglutination].

Another case of Tn-polyagglutination is described where for 6 years those responses have been observed which are typical of the acquired erythrocyte changes, viz. mixed-field polyagglutination by normal adult sera of all blood groups, no response with anti-TAh, agglutination of a certain part of erythrocytes by anti-TnSs, anti-ADb, anti-AHP, agglomeration of the other part by protamin sulfate and A-like specificity. Papain treatment eliminates polyagglutination by human sera of adults. Sera of new-borns did not agglutinate Tn-erythrocytes. Mrs. B. B. 38 years old belongs to blood group 0 and shows latent signs of haemolysis as well as permanently lowered leukocyte and thrombocyte numbers. The difficulties in blood group serology in connection with polyagglutination are referred too.

[External quality control for anti-D demonstration within the scope of Rh immunoprevention].

In the GDR the success and the dosage of IgG-anti-D in immune prophylaxis after birth of Rh-positive infants from Rh-negative mothers are controlled by the estimation of anti-D antibodies in maternal serum. For this purpose the papain method was introduced in centers for immune prophylaxis since 1970/71 at the time of general introduction of this immune prophylaxis in the GDR. The results if five external quality control experiments in the 16 to 18 such centers in the district of Magdeburg between 1972 and 1977 are recorded. The false positive and false negative reports are discussed and the demand for those regular external quality controls is carried ant.
