Improvement of clinical and MRI findings in a boy with adrenoleukodystrophy by dietary erucic acid therapy.

A 5-year-old boy with adrenoleukodystrophy, with clinical symptoms of visual, mental and motor disturbances which progressed rapidly, was treated with Lorenzo's oil consisting 1 volume of glyceryl trierucate and 4 volumes of glyceryl trioleate. Five months after initiation of this therapy, ability to swallow was enhanced and T2-weighted magnetic resonance imaging of the brain revealed regression of high intensity area of the parieto-occipital white matter.

Adrenoleukodystrophy

Different natures of supersensitivity of adenylate cyclase stimulated by calcitonin gene-related peptide and isoproterenol in rat diaphragm after denervation and reserpine treatment.

In skeletal muscles, calcitonin gene-related peptide (CGRP) released from motor nerve terminals and humoral catecholamines stimulate adenylate cyclase (AC) and enhance muscle contraction. The effects of denervation and treatment with reserpine on twitch contraction and the AC system in rat diaphragm were investigated. The basal levels of twitch contraction and AC activity of the diaphragm of rats were both increased 2 weeks after phrenic nerve denervation but were not altered by treatment with reserpine. Reserpine treatment provoked supersensitivity of AC to isoproterenol, without affecting the response to CGRP. On the other hand, denervation decreased the activation of AC and enhancement of twitch contraction by CGRP, without affecting the responses to isoproterenol. These data suggest that denervation causes up-regulation of AC as a result of loss of CGRP release from nerve terminal and that depletion of catecholamines by reserpine treatment supersensitizes the responses at the beta-adrenoceptor level. Thus, nervous and humoral factors regulate the AC system in striated muscle by different mechanisms.

Effects of sialagogues on ornithine decarboxylase induction and proto-oncogene expression in murine parotid gland.

The mechanism of a sialagogue-induced increase in ornithine decarboxylase (ODC) activity and the expressions of proto-oncogenes in murine parotid gland were investigated by use of isoproterenol (IPR), carbachol (CC), and methoxamine (MTX). The results were as follows: (1) The three sialagogues had similar effects on the parotid in vivo (mouse parotid after a single injection of IPR) and/or in vitro (rat parotid explants cultured on siliconized lens paper floating on 199 medium containing IPR, CC, or MTX), the order of their effectiveness being IPR > CC > MTX. (2) Northern/dot and Western blot analyses revealed that the sialagogues elevated the steady-state levels of ODC mRNA and ODC protein to maxima at two h and six h, respectively, after stimulation. The increases were roughly proportional to those in ODC activity, suggesting that sialagogue-dependent enzyme induction is regulated at the transcriptional level. (3) The mRNAs of four of nine proto-oncogenes examined showed sialagogue-dependent increases to maxima at 30 min (c-fos) or 60 min (c-jun, c-myc, and c-src) after the beginning of stimulation. These increases were all transient, with the levels returning to the control values (without sialagogue) within 60 min. (4) The IPR-dependent elevations of ODC activity and the mRNAs of ODC, c-fos, and c-jun were inhibited by monensin, but not by polymyxin B. On the other hand, the CC-dependent increases in these parameters were inhibited by polymyxin B but not by monensin.(ABSTRACT TRUNCATED AT 250 WORDS)

A case report of familial cyclic neutropenia.

A 34-year-old female with cyclic neutropenia is reported. Family studies showed that her three sons and her mother were also involved. Oscillations in the blood neutrophil counts were almost regular, with a periodicity of 21 days. Numbers of colony-forming unit--granulocyte macrophage (CFU-GM) formed from the bone marrow cells of normal volunteers co-cultured with the patient's serum or mononuclear cell-conditioned medium (MNC-CM) were examined. Her serum prepared during the neutropenic phase inhibited the growth of CFU-GM, while her MNC-CM stimulated it. Human granulocyte colony-stimulating factor (hG-CSF) level in her serum was persistently high, with the peak occurring during the neutropenic phase. These results suggest that some inhibitory factors in the serum may be pathophysiologically important for cyclic neutropenia. To control infections, a pharmacological dose of hG-CSF was administered for 7 days around the early neutropenic phase. Her peripheral neutrophil counts oscillated from 1,200/mm3 to 17,000/mm3 with G-CSF, and from 150/mm3 to 1,800/mm3 without G-CSF.

Conditioning stimulation of the central amygdaloid nucleus inhibits the jaw-opening reflex in the cat.

To elucidate a function of the central amygdaloid nucleus (ACE) in the trigeminal system, the ACE conditioning effect on the jaw-opening reflex (JOR) regarded as a nociceptive reflex was investigated in the cat anesthetized with pentobarbital sodium. The JOR to molar tooth pulp stimulation with an intensity 1.2-1.5 times the threshold was recorded in the ipsilateral digastric muscle. As conditioning stimulation, a train of 33 rectangular pulses (0.5 ms in duration) at 330 Hz with an intensity of 300 microA was applied to the ipsilateral ACE. The conditioning stimulation inhibited a JOR that had a latency of 7.90 +/- 0.88 ms (n = 36). The inhibition was maximum (83.1 +/- 11.2%) at a conditioning-test (C-T) interval of 110 ms and continued for C-T intervals of up to 1,000 ms. Likewise, microinjection of 0.5 M monosodium glutamate (10 microliters) into the ACE-inhibited the JOR for approximately 10 min. Additionally, the ACE conditioning stimulation inhibited the JOR induced by the stimulation of the sensory trigeminal nuclear complex in a similar manner, but not the jaw-opening response induced by the stimulation of the trigeminal motor nucleus (Mo V). Also, the conditioning stimulation influenced neither the evoked potentials induced by the tooth pulp stimulation at the main sensory and rostral nuclei nor the jaw-closing reflex induced by the stimulation of the mesencephalic trigeminal nucleus (Mes V). These results suggest that the excitation of the cell bodies in the ACE exerts an inhibitory modulation on the JOR with no effect on the non-nociceptive reflex such as the jaw-closing reflex at the level of Mo V.

[Improvement of respiratory function with weight reduction in obese elderly].

The patient was a 74-year-old woman who had been obese since age 18. Her obesity was refractory to dietary manipulation. She had been suffering from increasing dyspnea for several months and eventually could not even move. She was admitted to a hospital and diagnosed as having heart failure. Although her cardiac function recovered with medical treatment, her symptoms did not improve. The patient was then sent to our hospital. On admission, her height and weight were 149 cm and 81.9 kg, respectively, yielding a body mass index (BMI) of 36.6 kg/m2. Arterial blood gas analysis in room air revealed hypoxemia and an apnea index of 27 per hour. She was given a daily 500-1000 kcal diet. After four months of treatment, her weight decreased to 65 kg with a BMI of 29.3 kg/m2. Weight reduction together with the usage of progesterone-derivatives resulted in marked improvement of sleep apnea. The apnea index decreased to 3/h and arterial blood gas values normalized. This patient seemed to have suffered from both obesity hypoventilation syndrome and sleep apnea syndrome. Improvement of respiratory function was achieved through relief of airway obstruction and weight reduction, with activation of the respiratory center due to progesterone treatment.

[Assessment of fetal lung maturity using newly developed immunological measurement of fetal pulmonary surfactant apoprotein-A in amniotic fluid].

The level of immunoreactive lung surfactant apoprotein A (SP-A) was determined with a newly developed one-step ELISA kit (TDR-20) in 217 samples obtained by transabdominal or transvaginal amniocentesis from 217 pregnant women with high risk pregnancy. The lecithin/sphingomyelin ratio (L/S ratio) by two-dimensional thin layer chromatography, disaturated phosphatidylcholine (DSPC) values, shake test and the stable microbubble method were done simultaneously to compare diagnostic reliability in estimating fetal lung maturity. Amniotic fluid SP-A levels increased with advancing gestational age and correlated well with the values obtained by the other methods. When the cut-off SP-A value for the assessment of lung maturity was set at 1,700ng/ml in the amniotic fluid obtained within 24h before delivery, the true-negative rate for RDS was 71% and the true-positive rate for non-RDS was 83%. The sensitivity, specificity and accuracy were 100%, 83% and 88%, respectively. These results are comparable with those for the L/S ratio, DSPC determination and the stable microbubble method, and were slightly better than those for the shake test. In conclusion, this newly developed ELISA kit for the measurement of amniotic fluid SP-A is more effective than other methods currently available for the evaluation of fetal lung maturity, when it is considered that it requires only 0.2ml of amniotic fluid and provides results in only 2h without technical difficulties.

[A new hemodynamic diagram, using COP-PCWP and CI as parameters, after open heart surgery].

In 24 patients subjected to cardio-pulmonary bypass (CPB) operation, changes in colloid oncotic pressure (COP), cardiac index (CI), respiratory index (RI), and pulmonary capillary wedge pressure (PCWP) were measured within a 48 hour period. The patients were divided into two groups according to dose levels of catecholamine. In 18 patients received small catecholamine doses, a stable hemodynamic and respiratory status were maintained. However, 6 patients received catecholamine doses of more than 15 micrograms/kg/min exhibited low COP, low CI, high RI and high PCWP. In this study we designed a new hemodynamic diagram using COP-PCWP and CI as parameters of the cardiac function. We consider that these parameters are more suitable than Forrester's parameters to evaluate the patient's condition before and after open heart surgery.

[Clinicopathological evaluation of EVAL embolization in arteriovenous malformations].

The clinical and pathological aspects of two large arteriovenous malformations which were removed totally after preoperative embolization using ethylene vinyl alcohol copolymer (EVAL) were studied. The material, which is not adhesive, is handled easily during the procedure of embolization. However, it involves some risks because it might migrate to and occlude the normal branches of the brain and pass through the nidus to the venous system. Histopathological study of AVM nidus which was removed showed embolic materials within the vessels and inflammatory reaction of the vessel wall and its surrounding tissue. There were patchy hemorrhages within the AVM nidus and its surrounding brain tissue. Recanalization was also found within the occluded vessels. These findings suggest that preoperative embolization has some risk of causing intracerebral hemorrhage after the embolization. AVM should be removed surgically as a radical treatment if the patient is able to tolerate the operation.

[Effects of age on blood pressure, heart rate, and sensory blockade by spinal anesthesia with plain 0.5% bupivacaine].

To discover the influence of age on spinal blockade with plain bupivacaine, 23 patients were administered a subarachnoid injection of approximately 4.0 ml of 0.5% bupivacaine. The patients were divided into two groups. One group consists of patients with ages under 65 years (younger group, n = 13), and the other group consists of patients older than 65 years (elder group, n = 10). The following results were obtained. The time to onset of anesthetic effects (pinprick discrimination, temperature discrimination, and light touch discrimination) decreased with age. At 180 min after the administration, spinal blockade level did not have a significant difference. The degree of the decrease of systolic arterial pressure was greater in the elder group than in the younger group. However, the degree of the decrease of heart rate was greater in the elder group than in the younger group.

Sialagogue-stimulated protein phosphorylation related to ornithine decarboxylase induction in cultured rat parotid explants.

Both beta-adrenergic (isoproterenol) and cholinergic (carbachol) sialagogues increase amylase secretion, ornithine decarboxylase activity and DNA synthesis in murine parotid gland in vivo and in vitro. These agonists enhanced the incorporation of labelled inorganic orthophosphate into parotid proteins in rat parotid explants cultured on siliconized lens paper floating on serum-free 199 medium. Analysis of the labelled proteins by SDS-PAGE and autoradiography revealed that isoproterenol enhanced the phosphorylation of four proteins with apparent molecular weights of 17, 20, 31 and 32 kDa and carbachol stimulated the phosphorylation of 31 and 32 K proteins. Isoproterenol-dependent ornithine decarboxylase induction and phosphorylation of the proteins were selectively suppressed by monensin but not by polymyxin B, whereas carbachol-dependent ornithine decarboxylase induction and protein phosphorylation were inhibited by polymyxin B but not by monensin. Neither monensin nor polymyxin B suppressed isoproterenol- or carbachol-stimulated amylase secretion. Time course experiments showed that sialagogue-stimulated protein phosphorylation preceded the increase of ornithine decarboxylase activity and had almost disappeared when it was maximal. Propranolol and atropine, antagonists of isoproterenol and carbachol, respectively, completely inhibited not only amylase secretion and ornithine decarboxylase induction but also protein phosphorylation stimulated by the corresponding agonists. These findings suggest that increased phosphorylation of specific proteins is associated with sialagogue-stimulated ornithine decarboxylase induction but not amylase secretion.

The influence of soft lining materials on pressure distribution.

Several soft lining materials are available at present that differ in physical and mechanical properties. To examine the feasibility of soft lining materials used as so-called "tissue conditioners," the properties of soft lining materials were investigated by using a static creep test and a pressure distribution test. Six typical materials were used. The pressure on the testing plate was distributed uniformly on the soft lining materials. The soft lining material acted as a cushion for the external forces. However, the variation of pressures increased with time in the cases of Hydro-cast and Softone materials, but decreased with time in the case of Coe-comfort material.

Dental Materials

Effect of bupivacaine on muscle tissues and new bone formation induced by demineralized bone matrix gelatin.

Heterotopic bone formation induced by demineralized bone matrix gelatin (BMG) in bupivacaine-HCl-treated skeletal muscle was examined histologically. BMG was obtained by dehydrating diaphyseal shafts of femora and tibiae of male, 4-week-old Sprague-Dawley (SD) rats, cutting it into chips, and demineralizing and extracting the chips with various solutions. The BMG was implanted into the rectus abdominis muscle of male, 5-week-old SD rats, bupivacaine-HCl was injected at the same site, and the resulting plaques of tissues were examined histologically on days 5, 10, 15 and 20 after BMG implantation. Heterotopic bone formation occurred in all animals. The bupivacaine-treated group had more degenerated and injured muscle fibers, and more osteocytes than the control group. Electron microscopy showed that the basement membrane of muscle fibers was discontinuous and that many mononucleated cells resembling activated satellite cells were present on day 5. Many fibroblasts, undifferentiated mesenchymal cells and myogenic cells were seen in the area around the BMG. In new bones there were few osteocytes on day 10, but their numbers were increased on days 15 and 20 after implantation, especially in the bupivacaine-treated group. The population of osteocytes that increased rapidly may have included mononucleated cells similar to activated satellite cells.

Abdominal Muscles

[Mechanism of protective effects of ouren-gedoku-to and san'ou-syashin-to on the gastric mucosa].

Ouren-Gedoku-To (OGT) and San'ou-Syashin-To (SST), traditional Chinese prescriptions, have been used to treat peptic ulcer. In order to elucidate the mechanism of the protective effects of OGT and SST on the gastric mucosa, we studied their effects on gastric mucosal lesions, ion permeability and prostaglandin synthesis using ulcer experimentally induced in rats by saturation with acidic test solution after taurocholate treatment. Both OGT and SST inhibited taurocholate-induced gastric mucosal lesions in a dose-related manner. These prescriptions dose-dependently inhibited not only the increase of hydrogen and sodium ion net fluxes but also possible hydrogen ion back diffusion into the gastric mucosa during saturation with acidic test solution. SST induced a significant increase in gastric mucosal prostaglandins synthesized from 14C labeled arachidonate. These results suggest that both OGT and SST by strengthening the gastric mucosal barrier demonstrate their protective effects on the gastric mucosa and that this effect of SST is attributable to the increased ability of the gastric mucosa to synthesize prostaglandin.
