Exploring Allium species as a source of potential medicinal agents.

It has been shown that Allium species may help to prevent tumor promotion, cardiovascular diseases and aging; all processes that are associated with free radicals. Therefore the Allium species of both cultivated species (Allium nutans L., Allium fistulosum L., Allium vineale L., Allium psekemense B. Fedtsch, Allium cepa L., Allium sativum L.) and wild species (Allium flavum L., Allium sphaerocephalum L., Allium atroviolaceum Boiss, Allium schenoprasum L., Allium vineale L., Allium ursinum L., Allium scorodoprasum L.) from various locations were investigated for their antioxidative properties. The leaves were examined for activities of antioxidative enzymes (catalase, peroxidase, superoxide-dismutase, glutathione-peroxidase), non-enzymic antioxidants (reduced glutathione and total flavonoids), content of soluble proteins, vitamin C, carotenoids, chlorophylls a and b, as well as the quantities of malonyldialdehyde and *OH and O2*- radicals. Using a contemporary spectroscopic fluorescent method, lipofuscin, 'plant age pigments' were determined. ESR spectroscopy was used to follow the decrease of oxygen radicals in the presence of extracts of Allium species in phosphate buffer (pH 7). The results showed that all Allium species had strong antioxidative properties due to their high concentration of total flavonoids, high content of carotenoids and chlorophylls, and very low concentrations of toxic oxygen radicals. ESR signals of DMPO-OH radical adducts, in the presence of Allium extracts in phosphate buffer (pH 7), were reduced by up to 94.3%.

[Intrahepatic B3 cholangiojejunostomy in the palliative surgery of high unresectable malignant biliary obstruction].

Palliating the effects of biliary obstruction is a major goal of therapy in patients with unresectable cancer at the hepatic duct confluence. We reviewed our expirience with intrahepatic holangioenteric bypass to the segmental bile duct B3 as a palliative therapy in patients with unresectable malignant diseases involving the ductal confluence or the common hepatic duct. Since March 2001, we have performed intrahepatic segmental bile duct B3 cholangiojejunostomy by Roux-en-Y fashion utilizing a round ligament approach in 13 patients with malignant obstructive jaundice due to unresectable hilar holangiocarcinoma (8 cases) and gallbladder cancer (5 cases). Mean hospital stay was 123 days and mean blood loss was 25060 mL. Postoperative complications occurred in 3 patients (23%), but there was no surgical complications such as postoperative bleeding, bile leakage or abscess formation. 30-day mortality was 7.7% (1 patient). Late complications (37.5%) were observed in 3 of the 8 patients who survived for more than 5 months after the surgery. Median survival after B3 cholangiojejunostomy was 9 months (range, 10 days-22 months). Median survival time was significantly greater in patients with hilar cholangio-carcinoma (11.8 months; range: 2-22 months) compared with those with gallbladder cancer (4.6 months; range: 10 days-11.5 months) (P-0.032 log rank test; P-0.049 Tarone-Ware test). Intrahepatic B3 cholangiojejunostomy when combined with careful patient selection, can provide useful palliation from jaundice, pruritus and cholangitis with acceptable mortality and morbidity rates.

[Interleukin-8 serum levels in patients with various types of open heart surgical procedures performed under extracorporeal circulation].

Serum concentrations of proinflammatory cytokine interleukin-8 (IL-8) in 15 patients with surgically revascularized myocardium by triple venous cardiopulmonary bypass (CPB x 3) and in 10 patients with implanted artificial aortic valves (AV) were measured. Average IL-8 concentrations in (CPB x 3) patients and in those with artificial aortic valve were 7.3 +/- 11.6 pg/mL 24 hours before surgery, i.e. 3.3 +/- 3.4 pg/mL; six hours after surgical procedure 32.7 +/- 71.4 pg/mL, i.e. 8.9 +/- 9.9 pg/mL; and 24 hours after surgery 10.9 +/- 9.7 pg/mL, i.e. 8.3 +/- 4.6 pg/mL. Extracorporeal circulation (ECC) caused significant increase of IL-8 serum concentration in both investigated groups six hours after surgery. Comparing preoperative values of the both groups, as well as those of 6 and 24 hours after surgery, no significant values of IL-8 were found. Various types of open heart surgical procedures had no influence on the extent of the production and secretion of proinflammatory IL-8 cytokine measured in patients during 24 hours after surgery.

An investigation into the antioxidant activity of Allium nutans L.

Antioxidants are important species which possess the ability to protect the body from damage caused by free radical-induced oxidative stress. There is currently much interest in the antioxidant role of fruit, vegetables, wines and teas. In this study the antioxidant activity of leaf, bulb and root of Allium nutants L. was investigated. Biochemical parameters were also determined: activities of antioxidant enzymes (superoxide dismutase, catalase, peroxidase, glutathione peroxidase), quantities of malonyldialdehyde, superoxide and hydroxyl radicals and reduced glutathione and contents of total flavonoids, chlorophylls a and b carotenoids, vitamin C and soluble proteins. Our results indicated that Allium nutants L. exhibits antioxidant ability in all investigated plant organs. The highest antioxidant ability was observed in the leaves where all investigated antioxidant enzymes were active and quantities of malonyldialdehyde and OH. low. Reduced glutathione, pigments and carotenoids present in the leaves contribute to the high antioxidant activity. ESR investigation conducted with Allium nutans L. phosphate buffer (pH 7) extract showed that the signal DMPO-OH spin adducts in the presence of Allium nutans L. extract was reduced by 78.48%.

[The yentl syndrome in women with arterial hypertension and positive findings on the echocardiography dipyridamole stress test].

UNLABELLED: The aim of this study was to assess the possible gender differences between positive (new wall motion abnormality) combined dipyridamole stress echocardiography and coronary angiography or myocardial revascularization procedures. We evaluated 127 consecutive patients (82 males, 53.5 years old and 45 females, 56.1 years old) with systemic hypertension and chest pain (angina or atypical chest pain) who underwent pharmacological stress echocardiography (dipyridamole-atropine or dipyridamole-dobutamine) for detection of myocardial ischaemia. Coronary angiography was carried out in 84 patients (58 males and 26 females). Diagnostic accuracy of the test was the same in women as in men (92.3 vs 89.1%, females vs males, respectively). Coronary angiography was done in 73.6% of male and 66.7% of female patients with positive test; the difference was not significant. Over a two-year follow-up there were 31 myocardial revascularization procedures (20 coronary surgery and 11 angioplasty), 25 in men and 6 in women (p = 0.028). There was a trend in women with positive test to have less surgery or angioplasty (28.6%) than in men (45.3%), but the difference was not significant. Cox multivariate regression analysis revealed that women were less likely to receive revascularization (relative risk for females RR = 0.1457, 95% CI 0.0416-0.5101). CONCLUSION: Although diagnostic accuracy of third generation dipyridamole stress echocardiography was not different between men and women, in patients with positive test there was a trend in women to undergo coronary angiography and coronary surgery or angioplasty in lesser degree than in men. Cox multivariate model showed low relative risk of females for revascularization procedures and possible Yentl's syndrome in studied population.
