Photoradiation therapy of bladder tumors.

Photoradiation therapy, in which hematoporphyrin derivative is activated by an argon-dye laser, was performed on 46 superficial bladder tumors in 9 patients. A fluorospectrophotometric study of biopsies from these tumors and normal mucosa revealed preferential hematoporphyrin derivative localization in malignant tissues. The tumor response was confirmed ultrasonographically in 36 stages Ta to Tl tumors. Treatment with 100 to 250 J./cm.2 obtained complete remission in 5 of 6 patients with tumors 1 cm. in size or less. For those with tumors up to 2 cm. in size the light intensity should be 300 mw./cm.2 for 5 to 10 minutes or more and the accumulated energy intensity of light dose should be 100 J./cm.2 or more. No patient with tumors more than 2 cm. in size had a complete remission.

[Fundamental and clinical evaluations of 9, 3"-diacetylmidecamycin in the field of surgery (author's transl)].

The distribution of sensitivity to 9,3"-diacetylmidecamycin (MOM) was examined of 10 strains of Staphylococcus aureus isolated from foci in our hospital. A level of less than 3.13 mcg/ml was seen distributed in the majority of 7 cases, and it could be assumed that MOM would be sufficiently effective against infections by Staphylococci in clinical practice. In the comparison with other antibiotics popularly used, the sensitivity of the 10 strains to them was comparable to those against MDM, but were slightly inferior to cephalexin (CEX) and lincomycin (LCM), and slightly better than ampicillin (ABPC). The evaluation of clinical results revealed that, of the 20 cases with surgical and superficial infections, 6 and 10 were markedly and moderately effective, respectively, while 3 and 1 were fairly and poorly effective, respectively. It was therefore concluded to be an effective remedy for treatment of surgical and superficial infections in which Gram-positive cocci share the majority of the etiological factors. Of the side effects, there was none showing symptoms in digestive tracts or abnormalities such as rash, nor was there any patient for whom further administration of the agent had to be suspended due to side effects. No abnormality was seen in the test results between the premedication and postmedication findings on the hemogram, or hepatic and renal functions. It was therefore conjectured that this agent has a large margin of safety in its application.

Flow control in prolonged ventricular bypass and clinical application in 15 patients.

Long-term experiments employing left ventricular or biventricular bypass with local heparinization and blood filtration for up to 48 days was conducted in 51 calves at different flow rates. The principle of flow control obtained was applied to 15 patients in critical cardiac states. An avcothanized extracorporeal circuit consisted of heparin and protamine microperfusion lines, Pall filter, and nonpulsatile pump. Bypass with flow rates at 30--40% of cardiac output was conducted between 3 and 48 days. Survival was obtained in 34 of 36 calves. Bypass with flow rates at 60--80% of cardiac output was associated with 6 deaths in the 10 calves who underwent the procedure. Bypass with flow rates over 90% of cardiac output was often technically difficult and only 2 of 5 calves survived. Hemodynamic changes correlated well with the above results. In clinical cases flow rate was adjusted up to a maximum of 100% of left ventricular blood flow initially and thereafter to 30--40% of cardiac output for chronic support. A total of 15 patients with myocardial infarction, myocarditis and failing cardiac condition post-surgically were supported for 9--172 hrs. Six of the 15 survived. Adequate flow control in ventricular bypass support is important and the regimen developed in experimental studies proved effective clinically.
