Hip arthroplasty in Jutland, Denmark. Age and sex-specific incidences of primary operations.

During the 10-year period 1981-90, 1752 primary total hip arthroplasties were performed in the County of South Jutland, Denmark. The annual number increased to a steady state during 1988-90. In this period, the incidence was highest in the age group 70-79 years for both women and men, with 485 and 410 arthroplasties per 100,000 inhabitants, respectively; the overall incidence was 82 per 100,000 inhabitants. During the next 30 years, the demand for primary hip arthroplasties in Denmark is expected to increase because of demographic changes.

[The value of 1-year follow-up after total cemented hip alloplasty. Clinical and economic consequences of a routine check-up].

The need for, expenses of and consequences of one year follow-up after total cemented hip replacements with posterior access are reviewed. A total of 150 patients were registered prospectively and consecutively with Charnley scores preoperatively, three months and one year after operation. The Charnley scores increased significantly from the preoperative values till the follow-up at three months and from the follow-up at three months till follow-up at one year (p less than 0.001). In five patients, (3%) follow-up examination after one year was of significance. Anisomelia was corrected in all three by means of adjustment of footwear. The cost of follow-up examination after one year is estimated to be 630 Danish crowns (approximately 53 pounds) per patient. It is concluded that follow-up examination after total cemented hip replacement is not necessary. Alteration of follow-up examination after one year to follow-up examination after five years for patients who were under 60 years at the time of operation in order to reveal aseptic loosening would imply that only 10% of the patients in this material should be followed-up.

Aged

Unstable trochanteric fractures treated with the sliding screw-plate system. A biomechanical study of unstable trochanteric fractures. III.

Eighty unstable trochanteric fractures were treated with the sliding screw-plate system. Early weight-bearing was encouraged and mobilization was obtained within the first week in 47 per cent of the cases (37/78). Technical complications were encountered in 5 per cent of the patients (4/76) but none required re-operation. Telescoping of the screw occurred in 49 per cent (37/76). Through this secondary fracture impaction a stable load transmission system was established. Fracture union in the postoperative position was obtained in 49 per cent of the patients (37/76) and non-union did not occur.

Adult

Isolated lesions of the radio-ulnar disk treated with excision.

Twelve patients with isolated lesions in the radio-ulnar disk are reviewed. In the traumatic cases, where peripheral ruptures were found at operation, the results after excision were satisfactory, whereas patients with no evident traumatic etiology, where a degenerated disk was found, did not benefit from the excision. Resection of capitulum ulnae is recommended in such cases.

Adolescent

Surgical treatment of axillary hyperhidrosis in 123 patients.

123 patients suffering from axillary hyperhidrosis have been operated on as outpatients under local anaesthesia, with radical ablation of sweat glands. All except 7 patients were satisfied with the result. There were few complications. A group of patients and controls were extensively studied including quantitative sweat testing and axillary biopsis. Apart from increased axillary sweating during the test, no differences could be found. The basic etiology of the condition is still unknown.

Adolescent
