New equipment for stereophlebography.

Discussed in the present article is the complex of the equipment intended for roentgenodiagnosis of the lower limb varicosity and for use in planning the operations on the vessels. The complex includes the automatic stereocassette, constructed as an attachment to the standard roentgenodiagnostic apparatus, and the stereoscope for analyzing the stereophlebograms visually. The article provides a description of this equipment and methods of its employment.

Luminescent screens for general X-ray diagnostics.

This article gives the main parameters of amplifying screens manufactured in the USSR: the sensitivity, the resolution, the modulation depth, the noise. The technical level of Soviet screens is compared with the level of those manufactured by certain foreign companies. The measures taken to increase the technical level of today's products and the further evolution directions are discussed. Data are given on screen production in the USSR during the last 5 years and their supposed production up to 2000.

Radiation methods and means for testing radiodiagnostic equipment.

Discussed are the requirements for the methods and means of checking the electrical and radiation characteristics of radiodiagnostic equipment at three test levels, i.e. on release from the manufacturer, during the periodic test of the parameters and adjustment and repair in the clinic. Discussed are the description and construction of the simple and contact measuring devices for checking parameters at each level. The capabilities of the devices are compared to those of the series-produced analogs of various firms.

[Psychological component of fight against smoking in the framework of primary prevention of pulmonary cancer].

The study discusses certain aspects related to primary prophylaxis of lung cancer. These include objective parameters of smoking and psychological aspects of tobacco dependence of smokers as well as their awareness of health hazard of tobacco and motivations for cessation of smoking. Social and psychological reasons and patterns of smoking were studied in 400 workers and clerks of industrial enterprises. The role of psychological factors in cessation of smoking was shown. The need for certain psychologic correction aimed at formation of healthy life style is emphasized.

[The under- and overdiagnosis of primary skin melanomas].

The analysis of clinico-morphological data on 10123 cases of tumor and malformation of the skin seen at The N. N. Petrov Research Institute of Oncology of the USSR Ministry of Health showed primary skin melanoma to be underdiagnosed in 30.6% of cases and overdiagnosed in 60.1%. Complex pigmented nevus, pigmented basal-cell papilloma, basalioma, hemangioma and angiofibroma may simulate primary skin melanoma which accounts for misdiagnosis.

[An economic assessment of check-up fluorographic studies in detecting lung cancer].

The diagnostic value and cost-effectiveness of photoroentgenography were assessed on the basis of the data for 1977-1986 provided by 41 health care establishments of Leningrad. The mean overall cost of a single examination including the procedure and analysis was 0.29 +/- 0.01 rubles. Identification of a lesion in the lung, mediastinum or thoracic wall, whatever its nature, costed 18.8 rubles.

[Principles of improvement in the early diagnosis of malignant neoformations].

Early diagnosis of the most frequently--occurring cancers cannot be improved unless integrated approach to the problem is used. This should include application on a wider scale of regular medical check-ups in population older than 40 years with more extensive but justified use of modern laboratory and instrumental methods and computer--based follow-up programs.

Screening of average-risk individuals for colorectal cancer. WHO Collaborating Centre for the Prevention of Colorectal Cancer.

Recent developments in screening, diagnosis and treatment of colon cancer could lead to a reduction in mortality from this disease. Removal of adenomas, identification of risk factors, appropriate application of accurate diagnostic tests, and aggressive anatomic-surgical resection of colon cancers may already be having a favourable impact. Screening of average-risk populations over the age of 50 also offers promise in the control of this important cancer. The disease is of sufficient magnitude to deserve detection at an early stage with better prospects of patient survival, since screening tests with moderate sensitivity and high specificity are available. Flexible sigmoidoscopy and faecal occult blood tests are sufficiently acceptable to be included in case-finding among patients who are in the health care system. The results of current controlled trials involving more than 300,000 individuals for evaluating the impact of screening on mortality from colon cancer are needed before this approach can be recommended for general public health screening of the population. Further research is required to develop better screening tests, improve patient and physician compliance, and answer more definitively critical questions on cost-effectiveness. Mathematical modelling using current and new data can be used to determine the effectiveness of screening in conjunction with recommendations for primary prevention.

Colorectal Neoplasms

[The attitude of cancer patients to their own diagnosis].

The wide spectrum of viewpoints on how fully should a cancer patient be informed of his or her disease made it interesting to evaluate patients' own attitude to the issue. A questionnaire was offered to 280 cancer patients to assess the degree of awareness of their disease as well as psychologic preparedness to face the true diagnosis. As many as 93.6% of patients expressed a certain degree of desire to know diagnosis and details of their disease.

[The readaptation problems of cancer patients].

The paper discusses certain problems involved in communication between physician, cancer patient and society. The authors suggest that information given to a cancer patient on his or her diagnosis should be determined by the patient's willingness to know and prognosis. Also, suggestions on improvement of treatment of cancer patients of clinical group 4 are made.
