Psychiatric nursing for the 21st century. The PACED model.

Health care reform has had a profound effect on the way psychiatric treatment occurs in this country. Decreasing length of stay, increasing acuity, and reducing staff levels are making traditional approaches to mental health care infeasible. Resources already in place may still be focusing on long-term treatment issues and do not facilitate rapid stabilization and discharge planning that includes continued care within an integrated system. Research supports the feasibility of quality mental health care, which can be accomplished in shortened lengths of stay, as long as clinical managers plan inpatient programs focused on short-term goals followed by appropriate aftercare. In addition to recommendations for clinical managers, this article provides a proposal for executives redesigning a mental health care delivery system, which includes the goals of rapid assessment and stabilization, as well as discharge planning and appropriate follow up within an integrated system.

Discharges against medical advice. Provider accountability and psychiatric patients' rights.

If clinicians decide to prevent a voluntary patient from leaving a facility, proper protocols must be followed within the time limits defined in that state's laws on detentions. AMA discharges can sometimes be prevented by expedient treatment that includes involving the patient in goal-setting, adequate patient teaching, and appropriate discharge planning. Hospital management personnel should be familiar with individual state laws on detention protocols and should assist staff in resolving irregular discharge questions.

Writing as a therapeutic modality.

1. Most psychiatric and mental health professionals are experienced with writing in some form as a tool for encouraging patients to express feelings and identify stressors. 2. Narrative writing, or journal writing, can also help patients identify areas of conflict in their lives and to clarify complicated issues. 3. Writing can be used effectively with patients who are reluctant or embarrassed to speak openly in one-on-one interactions.

Health care reform: clinical implications for inpatient psychiatric nursing.

1. Shorter hospitalizations and increasing acuity have had an impact on all areas of inpatient services, making it necessary for health care providers to adapt quickly in order to remain competitive and continue to be able to provide quality services. 2. New modalities are being incorporated rapidly into existing units in an attempt to meet the changing needs of today's health care environment. These new programs greatly alter the traditional milieu and require a different treatment approach. 3. Inpatient hospitalization requires a rapid progression through three phases of treatment: assessment, stabilization, and discharge planning. The psychiatric nurse is instrumental in all three phases.
