[Immunocyte subpopulations in histologic sections of breast tumors].

A study of tumor infiltrating lymphocytes was carried out on cryostat sections of breast tumors in 27 patients using 24 monoclonal antibodies. Two types of infiltration--stromal and intraparenchymal--were identified. Certain correlations characteristic of subpopulation levels of immunocompetent cells identifiable by monoclonal antibody clusters CD8, CD22, CD38 and CD7; CD74 and CD4; CD11b and CD22, CD37 were established. The study also established a correlation between levels of immunocompetent cells infiltration (CD4+, CD7+, CD8+ and CD11b+) and tumor size and regional lymph node involvement.

[The clinicoimmunological characteristics of blast transformation in lymphosarcomas].

The authors studied a blast cell immunological phenotype in 50 adults with lymphosarcoma undergoing leukemization following the pattern of acute leukemia. Among the patients there were 12 females and 38 males aged 14-61. Immunological phenotyping of tumor cells was performed using a panel of monoclonal antibodies to T- and B-lymphocyte antigens, to myelomonocytic cells, some nonlinear and activation antigens. T, B and zero variants of blast cells were identified. Occasionally, blast cells carried nonlymphoid antigens and those corresponding to the common lymphosarcoma subvariant. Leukemization in the direction of lymphoblastic leukemia is associated with greater frequency of lymphosarcoma T subvariant (46%). B-cell and zero subvariants occurred in 28% and 20% of the patients, respectively. The number of complete remissions in the group of patients with T-cell subvariant was greater than in the group with B-cell subvariant. The survival in these two groups, however, was almost similar (median up to 8-12 months). Further studies into lymphoblastic leukemization immunophenotyping can help design programs of differentiated polychemotherapy.

[Hairy-cell leukemia (a review of the literature and the authors' own data)].

Gene engineering interferon made in Russia (reaferon, realdiron) was given to 40 patients with hairy-cell leukemia. Complete and partial remissions were achieved in 56% and 28% of the patients, respectively. Minimum effect was reported in 3% of the cases. In 5 cases with primary resistance or recurrences the authors used pentostatin which produced complete remissions in all the five patients.

[The current diagnosis of acute nonlymphoblastic leukemias].

The paper presents the results of the retrospective study of blast cells from 377 patients with acute nonlymphoblastic leukemia (ANLL). Morphocytochemical, immunological, cytogenetic assessments and cloning in semisolid agar were employed. The authors specify blast characteristics with reference to ANLL variants according to the FAB classification, define diagnostic criteria for poorly differentiated myeloblastic (MO), erythroblastic (M6) and megakaryoblastic (M7) leukemia, hybrid variants.

[Histiocytic proliferative diseases in children].

The course of histioproliferative diseases was analyzed in 308 children treated at the All-Union Cancer Research Center, USSR AMS. Of these, there were 8 children with benign proliferation (Rosai-Dorfman disease). Histiocytosis-X was revealed in 232 children. The authors describe the regularities of histiocytic proliferation manifestations depending on the patients' age. Provide morphological (cytological) criteria for different types of histioproliferative diseases and characterize the treatment data.

Local activation of lymphoid cells in rheumatoid synovium.

Lymphoid cell phenotype was investigated in the peripheral blood, synovial fluid, and synovial tissue of sixteen patients with rheumatoid arthritis (RA). In the peripheral blood of RA patients the proportion of cells expressing HLA-DR and beta 2-microglobulin receptors was higher than in normal controls, whereas the proportion of cells that were CD5+ (i.e. were T lymphocytes) was lower. Expression of the other cell surface antigens studied remained in the normal range. In the synovial tissue and synovial fluid of RA patients there was an increased percentage of cells expressing HLA-DR, beta 2-microglobulin receptors, CD25, CD5, CD4, and Thy-1, but the proportion of CD8+ cells was significantly decreased compared with that seen in peripheral blood. The CD4+/CD8+ ratio in RA joints was therefore significantly higher than that in peripheral blood. The proportion of cells expressing HLA-DR correlated with disease activity.

[Monoclonal antibodies of the ICO series in immunologic classification of lymphoblastic tumors in children].

The specificity of monoclonal antibodies (MoAbs) ICO in normal T and B cell differentiation has been described. The use of standard MoAbs CD19, CD5, CD7 was taken into account in modification of immunologic classification of lymphoblastic tumors. Analysis in 336 patients with ALL has been done to compare the use of E-rosettes, xenogeneic antisera and MoAbs in T-cell leukemia and lymphoma diagnosis. There are evident differences in the frequencies of some immunological subvariants of lymphoblastic leukemia as compared to lymphoma.

[Effects of immunologic markers on prognosis in acute lymphoblastic leukemia in young children].

Altogether 77 children aged up to 3 years with acute lymphoblastic leukemia (ALL) were examined for the immunological phenotype of blast cells. L1, L1/L2, L2/L1, L2 variants of ALL and the undifferentiated one were established in accordance with criteria of the FAB classification. T1, zero and Ia immunosubvariants were recorded most frequently. Mature cell T2 and pre-B variants (3 and 2 cases, respectively) were rare; B-cellular acute lymphoblastic leukemia was lacking; the "common" subvariant was revealed in 28 patients. The patients' age produced the highest effect on the prognosis: the significantly least disease standing and remissions were noted in a group of children under 2 years as compared to those aged 2 to 3 years. As for immunological markers, expression of Thy1-antigen exerted an unfavourable effect on the prognosis and duration of the first remission.

[Clinico-cytological parallels in acute myeloid leukemia in children].

Based on a comprehensive study of the morphofunctional parameters of blast cells by means of morphological, cytochemical, immunological and cytogenetic research methods, 8 types of blast cells and leukemic variants were identified in 200 children suffering from acute myeloid leukemia. In these children, the clinico-hematological parameters differed depending on the leukemic variant. Verification of the diagnosis of acute myeloid leukemia variants in children will favour further perfection of the individualized treatment programs.

[Glucocorticoid receptors in acute lymphoblastic leukemia in children].

Glucocorticoid receptor (GR) levels were quantitated in leukemic blasts from bone marrow aspirates of 53 children with acute lymphoblastic leukemia (ALL) and compared to the clinical and immunological features of the disease and its prognosis. GR levels did not correlate with the patients' sex, peripheral white blood cell count and cellular immunophenotype, but depended on the patients' age and histological type of leukemia. 43 patients were monitored during 3 or more years. The relapse-free and overall survival of ALL patients receiving standard courses of polychemotherapy including glucocorticoids was highly dependent on the GR level in bone marrow blasts. The best prognosis (67% relapse-free and 73% overall 3-years' survival) was observed in patients with GR levels over 6,000 sites/cell and patients with GR levels under 3,000 sites/cell had poor prognosis (31% relapse-free and 50% overall survival; median 1st remission duration 12 months). These correlations were most pronounced in ALL patients with "common" antigen.

[Treatment and prognosis of lymphoblastic lymphosarcoma in children].

Analysis was made of the results of the treatment of 18 children afflicted with lymphoblastic lymphosarcoma, with estimation of predictors influencing the patients' survival (from 1980 to 1986). The most important predictors influencing the 2-year relapse-free survival were found to be the stage, localization of the process, the presence of the symptoms of intoxication and biological activity. The data obtained were used later in the treatment of 160 children during 1986 to 1989. The results of the 2-year survival of this patients' group appreciably differ from those derived before.

[The results of using differentiated programs of therapy in acute lymphoblastic leukemia in adults].

Results of the treatment of 30 adult patients with acute lymphoblastic leukemia are analyzed. Before the treatment was commenced, the patients were distributed into groups with a favourable, intermediate and unfavourable prognosis depending on the signs previously defined by the authors. In the phase of induction and consolidation of a remission, the treatment intensity in these groups varied. The rate of complete remissions (CR) reached 70% in the whole group; provided Ph-positive ALLs were excluded from the analysis, it was 75%, the median duration of the CR amounted to 21 months, the projected 4-year relapse-free survival was 41%. In the group of historic control (31 patients treated in accordance with the unified program), these indicators were 58 and 69%, 18 months, and 21%, respectively. All the differences appeared statistically insignificant. In spite of the total treatment intensification, the rate of lethal outcomes did not rise during remission induction.

K20 and ICO-10 monoclonal antibodies (gp120/200; Thy-1): immunophenotyping of human solid tumours.

Solid tumour cells were shown to express VLA-beta and Thy-1 antigens. For identification of these molecules two monoclonal antibodies, K-20 and ICO-10, characterised in detail previously, were used. Four groups of solid tumours have been identified according to their immunophenotype: VLA-beta+ and Thy-1-; VLA-beta+ and Thy-1+; VLA-beta- and Thy-1+; VLA-beta- and Thy-1-. To a certain extent these groups have been shown to reflect tumour histogenesis: tumours of epithelial origin never expressed an ICO-10+, K20-phenotype while soft tissue sarcomas and neuroblastoma cells never expressed the beta-chain of VLA molecular complexes.

[The expression of leukocytic and related antigens on the cells of human breast cancer cells].

Certain expression regularities of different markers in human breast cancer cells are demonstrated. Monomorphic determinants of HLA class II antigen are detected only in case of HLA class I expression and cancer-embryonal antigen (CEA); the latter is combined with HLA class I and ICO-84 antigens at a significantly higher rate. The expression effect of CEA and HLA-DR on the metastatic spreading rate of regional lymph nodes is marked. A heterogeneity of the single tumour and in patients with malignant breast cancer is determined by the presence of the "leucocytic" antigens and CEA. In parallel studies monoclonal antibodies (MAB) HMFG-1 and a common epithelial antigen are assayed. The antigen interacting with MAB HEA-125 is characterized by the monomorphic expression.

[An immunologic study of serous exudate cells in oncologic patients].

Fitness of immunofluorescence and enzyme immunoassay for tests with centrifuged acetone-fixed cell preparations was assessed. All the 11 monoclonal antibodies (MCA) used reacted with cellular antigens in centrifuged cell preparations. HEA-125 and IKO-25 MCA proved to be the most informative; they permitted detection of individual tumor cells in serous exudation and specify the cytologic diagnosis.
