Two cases of catastrophic antiphospholipid syndrome.

Catastrophic antiphospholipid syndrome (CAPS) is a rare complication of antiphospholipid syndrome. It is a disseminated severe vascular pathology which presents with multi-organic dysfunction that progresses rapidly. Prognosis ends up being fatal in half of the cases. It may appear during pregnancy, surgery, infection, or after suspension of anticoagulation therapy. We studied two female patients with CAPS who survived after treatment with anticoagulation therapy and intravenous corticosteroids. The evolution of our two patients, after 17 months of follow-up under oral anticoagulant treatment was favourable and they are currently asymptomatic. In these patients the early diagnosis and treatment were essential to enhance their possibilities of survival.

[Intervention of Campylobacter jejuni subsp. jejuni flagella in the adhesion to cellular cultures: bacteriological and immunological evidence].

The participation of the flagella of a virulent strain (O52) of Campylobacter jejuni subsp. jejuni in the adhesion to HEp-2 cells and their inhibition by means of homologous polyclonal antibodies, monoclonal anti-flagella antibodies and colostral natural antibodies (IgA) was studied. An aflagellated strain (T1) was used as negative control. Adhesion was observed in higher rates with O52 strain (72%) than with T1 strain (27.5%). Polyclonal, monoclonal and colostral antibodies inhibited O52 strain adhesion in more than 70% (p < 0.001). T1 strain adhesion was inhibited only by polyclonal and colostral natural antibodies. Our results suggest that the flagella of C. jejuni subsp. jejuni could participate effectively in the adhesion process. However, the inhibition of T1 strain by polyclonal and colostral antibodies suggests the existence of other binds of adhesins in the bacterial surface.

[Helicobacter pylori diagnosis in children with recurrent abdominal pain].

We studied 40 children with recurrent abdominal pain during a period of six months (january to june 1993) entered at the gastroenterology service of the Instituto Nacional de Pediatria in México City. In all of then we performed in all of them an upper gastrointestinal endoscopy with 3 biopsies from gastric mucosa (antrum) for histological examination and urease tests. We also did an ELISA test looking for an H. Pylori specific serum IgG response. Our control group were 40 healthy children in whom we did only the ELISA test. We did not find any statistics differences between the histologic findings (gold standard) and the urease and ELISA tests. With IgG antibodies prevalence for H. Pylori in children with recurrent abdominal pain were 57% in contrast with our control group in whom the prevalence was 5%.

[Parasitic helminths infection in Coho salmón, Oncorhynchus kisutch, during their return to Simpson river, Chile].

Between may and july 1994, 17 adult returning salmons, Oncorhynchus kisutch, were collected in the River Simpson, Chile. All fishes showed infection by plerocercoids of Diphyllobothrium sp. in different locations: stomach, spleen, liver, mesenteries and gonads. Infection with larval cestodes of an unidentified species of Phillobothriidae was determined in the intestine of seven (41.2%) salmons and its prevalences of infection showed significant differences between female and male salmons. The 94.4% of total plerocercoids of Diphyllobothrium were isolated from the stomach wall. Prevalence and mean intensity of infection by Diphyllobothrium sp. did not show significant differences between fishes of different sex.

[Parasitism in fishes and human riverside communities of the Huillinco and Natri lakes (Great Island of Chiloé), Chile].

In April and December 1989, 35 fish from Lake Huillinco (42 degrees 48'S, 74 degrees 02'W) and 36 fish from Lake Natri (42 degrees 48'S, 73 degrees 50'W), in the Great Island of Chiloé (Chile) were examined. Coprological samples from 159 persons, 17 dogs, 19 pigs and 4 cats from around both lakes were examined for Diphyllobothrium spp. infection. In the Lake Huillinco the following helminths of fishes were determined: Contracaecum sp. and Hysterothylacium sp. in Salmo trutta, Cauque mauleanum and Eleginops maclovinus; Dichelyne (Cucullanellus) dichelyneformis in S. trutta and E. maclovinus and Scolex pleuronectis in S. trutta. One specimen of Mugil cephalus did not show helminth parasites. Prevalence of infection were greater for Contracaecum sp. in S. trutta (75.0%) and C. mauleanum (76.0%); and Hysterothylacium sp. in E. maclovinus (75.0%). Mean intensity was higher for D. (C.) dichelyneformis in E. maclovinus. Contracaecum sp. in S. trutta, Oncorhynchus mykiss, Oncorhynchus kisutch and Galaxias maculatus; Acanthocephalus sp. in S. trutta and G. maculatus, S. pleuronectis in O. mykiss and Cystidicoloides sp. in G. maculatus were determined at Lake Natri. Prevalence and intensity of infection were higher for Contracaecum sp. in S. trutta and O. kisutch. Infection by Diphyllobothrium sp. was determined in one domestic cat. Prevalence of infection by intestinal protozoan and helminths in human population only showed significative differences for Ascaris lumbricoides and Trichuris trichiura that were higher in the Lake Huillinco. Importance of natural infection by helminth parasites for fish in cultured condition and possible mechanisms of infections in relation to the diet of fishes are discussed.

[Prevalence of infection by intestinal helminths and protozoa in school children from a coastal locality in the province of Valdivia, Chile].

During July-August 1989, 219 coprological samples from primary school children from Niebla and Los Molinos localities (39 degrees 52'S, 73 degrees 26'W) in the coast of province of Valdivia, Chile, were analysed. Prevalence % (in parentheses) of infection by intestinal protozoa and helminths were the followings: Entamoeba histolytica: (18.0), Entamoeba coli (34.0), Endolimax nana (34.4), Iodamoeba buetschlii (7.4), Blastocystis hominis (64.3), Giardia intestinalis (27.9) Chilomastix mesnili (0.8), Ascaris lumbricoides (12.7), Trichuris trichiura (32.0), Trichostronqylidea gen. sp. (0.8), Enterobius vermicularis (1.6), Hymenolepis nana (0.4) Diphyllobothrium sp. (0.4). No sanitary conditions, water, faeces and garbage disposal were detected in the 55.5%, 86.4% and 52.7% of 110 houses from the sector, respectively. The high prevalence of protozoa and helminths intestinal infections in the sector are related to no sanitary conditions of houses and fecal contamination of the estuary of the Valdivia river.

[Comparison of 3 techniques in the serological diagnosis of human hydatidosis].

Three serological techniques for hydatid disease were compared using 60 human serum samples obtained from 29 patients with hydatidosis previously demonstrated by surgery, 15 apparently healthy individuals and 16 persons with different pathologies. The results obtained with these 3 techniques showed some differences that could associate with the anatomical location of the parasite and the biological stage of the hydatid cysts. The global sensitivity of the techniques was 96.6% for ELISA, 86.2% for IHAT and 79.3 for DD5. False positive reactions were observed with IHAT and ELISA in sera from patients with infection by Taenia saginata. ELISA also showed cross reactions in the sera of four patients with intestinal infection by Ascaris lumbricoides. No false positive reactions were observed with DD5 test.

[Viruses precipitating asthmatic crises].

Study made with 45 patients in the allergy service of the Instituto Nacional de Pediatría, with the purpose of analyzing the virus participation in the precipitation of asthmatic crisis in children who lived in the southeast of Mexico City. The subjects were included in the study if they presented signs of atopy or infection of the superior air channels. Pollen allergics, rhinosinusitis, smokers relatives and patients who own pets were excluded. In this population group, the participation of the virus were RSV, influenza B and parainfluenza.
